Objawy
Cerwicyt to zapalenie szyjki macicy, które może mieć charakter ostry (zwykle infekcyjny) lub przewlekły (często nieinfekcyjny). Występuje u ponad 50% kobiet w ciągu życia, często bezobjawowo (do 80% przypadków). Objawy, gdy występują, obejmują nieprawidłową wydzielinę o zmienionej barwie (żółta, biała, szara, zielonkawa), krwawienia między miesiączkami lub po stosunku, dyspareunię, dyskomfort w miednicy oraz objawy zapalenia dróg moczowych. W badaniu ginekologicznym obserwuje się zaczerwienienie, obrzęk, śluzowo-ropną wydzielinę, kruchość szyjki i bolesność przy ruchach szyjki (cervical motion tenderness). Leczenie infekcyjnego cerwicytu bakteryjnego (np. chlamydia, rzeżączka) antybiotykami zwykle prowadzi do ustąpienia objawów w ciągu kilku dni i całkowitego wyleczenia w 1-2 tygodnie. W przypadku wirusowego zapalenia (HSV, HPV, CMV) infekcja może być kontrolowana, ale nie zawsze całkowicie eliminowana.
Definicja cerwicytu
Cerwicyt (łac. Cervicitis) to stan zapalny szyjki macicy, dolnej, wąskiej części macicy, która łączy się z pochwą. Stan ten może być ostry (najczęściej spowodowany infekcją) lub przewlekły (zwykle związany z czynnikami nieinfekcyjnymi). Jest to bardzo powszechny problem zdrowotny – badania pokazują, że dotyka ponad połowę dorosłych kobiet na pewnym etapie ich życia. 123
Objawy cerwicytu
Cerwicyt często przebiega bezobjawowo i wiele kobiet nie wie o jego istnieniu aż do momentu, gdy zostanie zdiagnozowany podczas rutynowego badania ginekologicznego przeprowadzonego z innego powodu. Szacuje się, że nawet do 80% pacjentek z cerwicytem nie wykazuje żadnych objawów. 123
Podstawowe objawy
Jeśli objawy występują, mogą obejmować:
- Nieprawidłową wydzielinę z pochwy – często o zwiększonej ilości, barwie żółtej, białej, szarej lub zielonkawej, czasem z nieprzyjemnym zapachem 12

Ciężkie objawy
W ciężkich przypadkach cerwicytu może wystąpić:
- Obfita ropna wydzielina z pochwy o żółtej lub zielonej barwie 12
- Otwarte owrzodzenie szyjki macicy 12
- Znaczny ból w dole brzucha 12
- Gorączka (rzadko, zwykle przy rozwinięciu się zapalenia narządów miednicy mniejszej) 12
Cechy charakterystyczne w badaniu lekarskim
Podczas badania ginekologicznego, lekarz może zaobserwować:
- Zaczerwienie, obrzęk i podrażnienie szyjki macicy 12
- Śluzowo-ropną wydzielinę widoczną w kanale szyjki macicy 12
- Łatwą krwawość szyjki macicy przy delikatnym dotknięciu wymazówką (tzw. kruchość szyjki) 12
- Bolesność przy poruszaniu szyjką macicy (cervical motion tenderness) – może wskazywać na zapalenie narządów miednicy mniejszej 12
Przebieg cerwicytu
Ostre i przewlekłe zapalenie
Cerwicyt może przebiegać jako:
- Ostre zapalenie – rozwija się szybko, zwykle spowodowane przez infekcję. Objawy mogą być nasilone, ale przy właściwym leczeniu szybko ustępują. 12
- Przewlekłe zapalenie – trwa miesiące lub lata, zwykle spowodowane czynnikami nieinfekcyjnymi. Objawy mogą być łagodniejsze, ale bardziej długotrwałe. Termin przewlekłego cerwicytu stosuje się do kobiet z utrzymującą się wydzieliną przez co najmniej 3 miesiące pomimo ustąpienia/wykluczenia infekcji. 123
Czas trwania i odpowiedź na leczenie
Czas trwania cerwicytu zależy od przyczyny, nasilenia stanu i zastosowanego leczenia:
- Przy infekcyjnym cerwicycie bakteryjnym (np. spowodowanym chlamydią lub rzeżączką), objawy zwykle zaczynają ustępować w ciągu kilku dni po rozpoczęciu leczenia antybiotykami. Całkowite wyleczenie następuje zwykle w ciągu 1-2 tygodni przy prawidłowym stosowaniu antybiotyków. 123
- W przypadku wirusowego zapalenia (np. HSV, HPV, CMV), infekcja może nie zostać całkowicie wyleczona, ale objawy można kontrolować, a układ odpornościowy może z czasem opanować infekcję. 12
- Nieinfekcyjny cerwicyt spowodowany alergenami lub środkami drażniącymi może ustąpić szybko po usunięciu czynnika drażniącego, ale pełne wygojenie może potrwać kilka tygodni. 12
- Bez odpowiedniego leczenia, cerwicyt może utrzymywać się przez miesiące, a nawet lata, powodując przewlekły ból podczas stosunku i pogarszające się objawy. 123
Nawroty cerwicytu
Cerwicyt może nawracać, szczególnie jeśli:
- Leczenie nie zostało dokończone zgodnie z zaleceniami 12
- Partner seksualny nie został odpowiednio leczony (w przypadku infekcji przenoszonych drogą płciową) 12
- Doszło do ponownej ekspozycji na czynnik wywołujący zapalenie 1
Badania pokazują, że cerwicyt nawraca u 8-25% kobiet, które już go miały. 12
Potencjalne powikłania
Zapalenie narządów miednicy mniejszej
Najpoważniejszym powikłaniem cerwicytu jest zapalenie narządów miednicy mniejszej (PID), gdy infekcja rozprzestrzenia się z szyjki macicy do górnych części układu rozrodczego, w tym do wyściółki macicy (endometrium), jajowodów i jajników. 123
