Etiologia i przyczyny

Wsteczny wytrysk (retrograde ejaculation) to patologiczne cofanie się nasienia do pęcherza moczowego podczas orgazmu, spowodowane dysfunkcją zwieracza pęcherza moczowego. Najczęstszą etiologią są zabiegi chirurgiczne w obrębie miednicy, zwłaszcza przezcewkowa resekcja prostaty (TURP) z ryzykiem 10-15%, prostatektomia oraz operacje szyi pęcherza moczowego. Inne przyczyny to neuropatie autonomiczne związane z cukrzycą, stwardnieniem rozsianym, chorobą Parkinsona, urazami rdzenia kręgowego (częstość 8-37%) oraz dysautonomią. Leki alfa-adrenolityczne (np. tamsulosin, silodosin), SSRI, leki przeciwpsychotyczne i inne mogą indukować wsteczny wytrysk poprzez wpływ na mięśnie szyi pęcherza lub układ nerwowy. Wrodzone anomalie anatomiczne, takie jak szeroka szyja pęcherza czy zastawki cewki tylnej, również predysponują do tego zaburzenia.

Wsteczny wytrysk – Etiologia, przyczyny, uwarunkowania

Wsteczny wytrysk (retrograde ejaculation) to stan, w którym nasienie zamiast być wyrzucane przez cewkę moczową na zewnątrz penisa podczas orgazmu, cofa się do pęcherza moczowego. Mechanizm tego zjawiska związany jest z nieprawidłowym działaniem mięśnia okrężnego zwanego zwieraczem pęcherza, który podczas normalnego wytrysku powinien zamykać się, aby umożliwić nasienie wyrzut nasienia na zewnątrz i zatrzymać mocz w organizmie. Gdy zwieracz ten nie działa prawidłowo, szyja pęcherza pozostaje otwarta, co pozwala nasieniu przemieszczać się drogą najmniejszego oporu do pęcherza.123

Wsteczny wytrysk jest stosunkowo rzadkim zaburzeniem, jednak jego częstość wzrosła w ostatnich latach z powodu zwiększonej liczby zabiegów chirurgicznych w obszarze miednicy i narządów płciowych. Jest to najczęstsza przyczyna aspermii, czyli braku wytrysku nasienia podczas orgazmu.45 Szacuje się, że wsteczny wytrysk odpowiada za około 0,3-2% wszystkich przypadków niepłodności męskiej.67

Przyczyny chirurgiczne

Jedną z najczęstszych przyczyn wstecznego wytrysku są zabiegi chirurgiczne, szczególnie te dotyczące układu moczowo-płciowego oraz okolic miednicy:89

  • Zabiegi na prostacie – przede wszystkim przezcewkowa resekcja gruczołu krokowego (TURP) – u 10-15% mężczyzn poddanych temu zabiegowi występuje wsteczny wytrysk1011
  • Prostatektomia (usunięcie całej prostaty) – ryzyko wystąpienia wstecznego wytrysku jest jeszcze wyższe12
  • Operacje na szyi pęcherza moczowego1314
  • Limfadenektomia zaotrzewnowa (RPLND) w leczeniu raka jądra – może powodować czasowy lub trwały wsteczny wytrysk poprzez uszkodzenie dróg nerwowych biegnących do zwieracza pęcherza1516
  • Cystektomia (usunięcie pęcherza moczowego)17
  • Operacje na okrężnicy i odbytnicy18
  • Operacje na dolnym odcinku kręgosłupa19
  • Zabiegi chirurgiczne aorty i operacje naczyniowe w obrębie miednicy20

Klasyczne techniki chirurgiczne stosowane w leczeniu łagodnego rozrostu prostaty mogą prowadzić do trwałego wstecznego wytrysku u większości mężczyzn poddanych tym procedurom.2122 Radioterapia stosowana w leczeniu nowotworów w obszarze miednicy może również uszkadzać nerwy i mięśnie odpowiedzialne za prawidłowy wytrysk.2324

Przyczyny neurologiczne

Uszkodzenie układu nerwowego stanowi kolejną istotną przyczynę wstecznego wytrysku. Choroby i stany powodujące neuropatię mogą wpływać na prawidłowe funkcjonowanie zwieracza pęcherza:2526

  • Cukrzyca – szczególnie długotrwała, źle kontrolowana cukrzyca może prowadzić do neuropatii autonomicznej, która uszkadza nerwy kontrolujące mięśnie pęcherza i narządów rozrodczych272829
  • Stwardnienie rozsiane – może powodować dysfunkcję nerwów odpowiedzialnych za prawidłowy wytrysk3031
  • Choroba Parkinsona – wpływa na funkcje autonomicznego układu nerwowego3233
  • Urazy rdzenia kręgowego – mężczyźni z urazami rdzenia kręgowego mogą doświadczać wstecznego wytrysku z częstością od 8% do 37%3435
  • Dysautonomia – zaburzenie funkcji autonomicznego układu nerwowego3637
  • Udar mózgu – może wpływać na nerwy kontrolujące funkcje ejakulacyjne38

Zaburzenia endokrynologiczne, takie jak niedoczynność tarczycy, mogą objawiać się jako mononeuropatia lub polineuropatia czuciowo-ruchowa. Jeśli stan ten wpływa na nerwy odpowiedzialne za kontrolę ejakulacji, może prowadzić do wstecznego wytrysku.3940

Przyczyny farmakologiczne

Liczne leki mogą powodować wsteczny wytrysk poprzez wpływ na mięśnie szyi pęcherza moczowego lub autonomiczny układ nerwowy:4142

  • Leki alfa-adrenolityczne (alfa-blokery) stosowane w leczeniu:
    • Łagodnego rozrostu prostaty (BPH) – tamsulosin (Flomax), alfuzosin (Uroxatral), terazosin (Cardura)434445
    • Nadciśnienia tętniczego4647
  • Leki przeciwdepresyjne:
    • Selektywne inhibitory wychwytu zwrotnego serotoniny (SSRI)48
    • Imipramina (Tofranil) – może być również używana w leczeniu wstecznego wytrysku49
    • Duloksetyna50
  • Leki przeciwpsychotyczne:
    • Chlorpromazyna, tiorydazyna, risperidon5152

Tamsulosin (Flomax) i silodosin (Rapaflo) należą do alfa-blokerów, które najczęściej powodują problemy z ejakulacją.53 Zauważalny jest również związek z atomoksetyną, która należy do inhibitorów wychwytu zwrotnego noradrenaliny (NRI).54 Pacjenci doświadczający wstecznego wytrysku jako efektu ubocznego leków często przerywają terapię z tego powodu.55

Przyczyny wrodzone i anatomiczne

W niektórych przypadkach wsteczny wytrysk może być spowodowany wrodzonymi lub anatomicznymi nieprawidłowościami:5657

  • Wrodzone nieprawidłowości szyi pęcherza lub cewki moczowej58
  • Wrodzona szeroka szyja pęcherza59
  • Zastawki cewki tylnej60
  • Torbiele macicy męskiej61
  • Wynicowanie pęcherza (ekstrofia pęcherza)6263

Te anatomiczne nieprawidłowości mogą utrudniać prawidłowe zamknięcie szyi pęcherza podczas ejakulacji, prowadząc do wstecznego wytrysku.64

Czynniki ryzyka

Zwiększone ryzyko wystąpienia wstecznego wytrysku występuje u mężczyzn, którzy:6566

  • Chorują na cukrzycę lub stwardnienie rozsiane67
  • Przeszli operację prostaty lub pęcherza moczowego68
  • Przyjmują leki na nadciśnienie tętnicze lub zaburzenia nastroju69
  • Doznali urazu rdzenia kręgowego70

Szacuje się, że u mężczyzn z cukrzycą i długotrwałą złą kontrolą glikemii ryzyko wystąpienia neuropatii wpływającej na funkcje ejakulacyjne jest znacząco wyższe.7172

Konsekwencje wstecznego wytrysku

Wsteczny wytrysk sam w sobie nie jest szkodliwy dla zdrowia, ale może powodować:7374

  • Niepłodność – główna komplikacja, istotna tylko dla mężczyzn planujących ojcostwo7576
  • Suchy orgazm – podczas orgazmu występuje niewielka ilość nasienia lub brak wytrysku, co może wpływać na satysfakcję seksualną7778
  • Problemy psychologiczne – związane z brakiem widocznego wytrysku podczas orgazmu7980

Warto podkreślić, że wsteczny wytrysk może być trwały lub tymczasowy, w zależności od przyczyny. W przypadku gdy jest spowodowany ciężkim uszkodzeniem nerwów lub mięśni, stan może być nieodwracalny.818283

Możliwości leczenia

Leczenie wstecznego wytrysku zależy od jego przyczyny. Typowo nie wymaga ono interwencji, chyba że wpływa na płodność:8485

  • Odstawienie leków wywołujących objawy – jeśli przyczyną jest działanie niepożądane leków, zazwyczaj powrót do normalnej ejakulacji następuje po zaprzestaniu ich stosowania8687
  • Farmakoterapia – w przypadku wstecznego wytrysku spowodowanego uszkodzeniem nerwów (np. w cukrzycy, stwardnieniu rozsianym):
  • Techniki wspomaganego rozrodu – gdy farmakoterapia jest nieskuteczna:
    • Pozyskiwanie plemników z moczu po ejakulacji9495
    • MESA (mikrochirurgiczna aspiracja nasienia z najądrza)96
    • Zapłodnienie in vitro (IVF) z wykorzystaniem pozyskanych plemników97

Leczenie farmakologiczne zazwyczaj nie przynosi rezultatów, jeśli wsteczny wytrysk jest spowodowany trwałymi zmianami anatomicznymi w wyniku zabiegów chirurgicznych, takich jak operacje szyi pęcherza moczowego czy przezcewkowa resekcja prostaty.9899

W przypadkach związanych z cukrzycą, odpowiednia kontrola poziomu glukozy we krwi może pomóc w zapobieganiu neuropatii prowadzącej do wstecznego wytrysku.100

U wielu mężczyzn z wstecznym wytryskiem możliwe jest uzyskanie ciąży przez partnerkę po zastosowaniu odpowiedniego leczenia, zwłaszcza technik wspomaganego rozrodu.101102
