Wsteczny wytrysk (ejaculatio retrograda) to stan, w którym nasienie cofa się do pęcherza moczowego podczas ejakulacji, co skutkuje zmniejszoną lub całkowitym brakiem wydzielania nasienia przez cewkę moczową. Nie wpływa on na erekcję ani odczuwanie orgazmu, ale może prowadzić do niepłodności męskiej. Leczenie jest wskazane głównie u pacjentów planujących potomstwo i zależy od przyczyny, np. uszkodzenia nerwów spowodowanego cukrzycą, stwardnieniem rozsianym czy zabiegami chirurgicznymi. Farmakoterapia obejmuje stosowanie leków α1-adrenergicznych (np. imipramina, midodryna, pseudoefedryna, fenylefryna, metoksamina), które zwiększają napięcie mięśnia szyi pęcherza moczowego, zapobiegając cofaniu się nasienia. Efektywność leczenia farmakologicznego wynosi około 30-40%, jednak leki te mogą podnosić ciśnienie tętnicze i częstość akcji serca, co wymaga ostrożności u pacjentów z chorobami serca lub nadciśnieniem.

Definicja wstecznego wytrysku

Wsteczny wytrysk (łac. ejaculatio retrograda, ang. retrograde ejaculation) to stan, w którym nasienie zamiast być wyrzucane przez cewkę moczową i ujście zewnętrzne prącia, cofa się do pęcherza moczowego podczas ejakulacji. Zjawisko to często nazywane jest również „suchym orgazmem” i może być czynnikiem wpływającym na niepłodność męską.12 Wsteczny wytrysk nie wpływa na zdolność do osiągnięcia erekcji czy odczuwania orgazmu, lecz powoduje, że podczas wytrysku wydzielana jest bardzo mała ilość nasienia lub nie jest ono wydzielane wcale.34

Leczenie wstecznego wytrysku

Wsteczny wytrysk zazwyczaj nie wymaga leczenia, chyba że zaburza płodność. W takich przypadkach terapia zależy od przyczyny leżącej u podłoża schorzenia.56 Jeżeli pacjent nie planuje posiadania potomstwa, lekarz może zasugerować, że leczenie nie jest konieczne, ponieważ wsteczny wytrysk nie jest szkodliwy ani bolesny.78

Leczenie farmakologiczne

Farmakoterapia może być skuteczna w przypadku wstecznego wytrysku spowodowanego uszkodzeniem nerwów. Ten rodzaj uszkodzenia może być wywołany przez cukrzycę, stwardnienie rozsiane, niektóre zabiegi chirurgiczne oraz inne schorzenia i metody leczenia.910 Celem farmakoterapii jest wzmocnienie napięcia mięśnia szyi pęcherza moczowego, co zapobiega cofaniu się nasienia do pęcherza podczas ejakulacji.11

Najczęściej stosowane leki w leczeniu wstecznego wytrysku to:1213

W niektórych przypadkach skuteczne może być podanie czystego agonisty receptorów α1-adrenergicznych, metoksaminy. Lek ten może być samodzielnie podawany domięśniowo przez pacjentów 30 minut przed stosunkiem lub masturbacją. W badaniach nie obserwowano istotnych działań niepożądanych poza lekkim zjawiskiem piloreakcji (tzw. „gęsia skórka”), niewielkim wzrostem ciśnienia tętniczego i brakiem zmian w częstości akcji serca.22

Ważne jest, aby przed przepisaniem nowego leku lekarz rozważył ogólny stan zdrowia pacjenta, w tym inne schorzenia, na które może cierpieć.23 Należy zauważyć, że leki najczęściej przepisywane na wsteczny wytrysk mogą powodować podwyższone ciśnienie krwi i przyspieszoną częstość akcji serca, dlatego mogą nie być dobrą opcją dla osób z chorobami serca lub wysokim ciśnieniem krwi.24

Skuteczność leczenia farmakologicznego

Efektywność leczenia farmakologicznego wstecznego wytrysku zależy od przyczyny występowania tej dolegliwości:25

  • Jeśli wsteczny wytrysk jest spowodowany przyjmowaniem leków, zaprzestanie ich stosowania lub zmiana na inne zazwyczaj przywraca normalną ejakulację2627
  • W przypadku łagodnego uszkodzenia nerwów lub mięśni pęcherza, leczenie farmakologiczne może przynieść poprawę28
  • Około 40% mężczyzn poddanych leczeniu farmakologicznemu obserwuje poprawę29
  • Około jedna trzecia mężczyzn ze wstecznym wytryskiem doświadcza poprawy po leczeniu lekami, które zamykają szyję pęcherza (takimi jak pseudoefedryna lub imipramina)30

Leki nie pomogą, jeśli wsteczny wytrysk wynika z operacji powodującej trwałe zmiany fizyczne w anatomii pacjenta. Przykładami takich zabiegów są operacja szyi pęcherza i przezcewkowa resekcja gruczołu krokowego (TURP).313233

Wskazania do leczenia

Głównym wskazaniem do leczenia wstecznego wytrysku jest chęć przywrócenia płodności. Jeśli mężczyzna nie planuje potomstwa, zazwyczaj nie ma potrzeby stosowania terapii.3435 Niektórzy pacjenci mogą jednak zdecydować się na leczenie ze względu na dyskomfort psychiczny związany z brakiem lub zmniejszoną objętością wytrysku.3637

W przypadku mężczyzn, którzy chcą mieć dzieci, a cierpią na wsteczny wytrysk, konieczne jest wdrożenie odpowiedniego leczenia, aby umożliwić zapłodnienie partnerki. Aby doszło do zapłodnienia, mężczyzna musi wydzielać wystarczającą ilość nasienia, które przeniesie plemniki do pochwy i macicy partnerki.38

Techniki wspomaganego rozrodu

Jeśli leczenie farmakologiczne nie pozwala na wydzielanie nasienia, pacjent najprawdopodobniej będzie potrzebował procedur leczenia niepłodności, znanych jako techniki wspomaganego rozrodu (ART – Assisted Reproductive Technology), aby umożliwić partnerze zajście w ciążę.3940

Pozyskiwanie plemników

W przypadku mężczyzn cierpiących na wsteczny wytrysk, plemniki można pozyskać różnymi metodami:4142

  • Odzyskiwanie plemników z moczu – plemniki mogą być pobrane z moczu po ejakulacji, a następnie odpowiednio przygotowane w laboratorium. Jest to najczęstsza, najbardziej efektywna i ekonomiczna niechirurgiczna metoda w technikach wspomaganego rozrodu.4344
  • Przezskórna aspiracja plemników z najądrza (PESA) – procedura pobrania plemników bezpośrednio z najądrza.45
  • Ekstrakcja plemników z jądra (TESE) – chirurgiczna procedura polegająca na pobraniu niewielkiej ilości tkanki jądra, z której następnie izoluje się plemniki.4647
  • Stymulacja wibracyjna prącia (PVS) – stosuje się specjalnie zaprojektowany wibrator przyłożony do żołędzi prącia. Nerwy czuciowe przenoszą wibracje do rdzenia kręgowego, wywołując ejakulację. Ta metoda jest skuteczna u około 60% mężczyzn z anejakulacją.48
  • Elektroejakulacja – jeśli PVS nie działa, lekarz może wprowadzić sondę do odbytnicy, aby bezpośrednio stymulować nerwy czuciowe w pobliżu prostaty. Sonda wysyła łagodne impulsy elektryczne, które powodują orgazm i wytrysk. Zabieg ten może odbywać się w znieczuleniu ogólnym. Około 90% mężczyzn doświadcza ejakulacji przy zastosowaniu tej metody, chociaż jakość plemników może być początkowo niska.49

Metody zapładniania

Po pomyślnym pobraniu plemników, lekarz może pomóc partnerze mężczyzny zajść w ciążę za pomocą jednej z następujących metod:5051

  • Inseminacja domaciczna (IUI) – przy użyciu małego cewnika wprowadza się oczyszczone plemniki do macicy partnerki w czasie owulacji. Ta metoda jest szczególnie przydatna, gdy udaje się odzyskać dostateczną ilość plemników o dobrej jakości.5253
  • Zapłodnienie in vitro (IVF) – proces, w którym komórki jajowe i plemniki inkubowane są razem w laboratorium w celu uzyskania zapłodnienia.5455
  • Docytoplazmatyczna iniekcja plemnika (ICSI) – polega na wstrzyknięciu pojedynczego plemnika bezpośrednio do komórki jajowej partnerki w celu zapłodnienia. Ta metoda jest szczególnie przydatna w przypadkach, gdy liczba lub jakość plemników jest bardzo niska.56

Wskaźniki powodzenia technik wspomaganego rozrodu w przypadku par, gdzie mężczyzna cierpi na wsteczny wytrysk, są porównywalne do wyników uzyskiwanych przy wykorzystaniu ejakulowanych plemników.57 W przypadku inseminacji domacicznej (IUI) wskaźnik ciąż na cykl może wzrosnąć nawet do 24%, co zbliża się do wskaźnika obserwowanego u par bez problemów z płodnością.58

Leczenie chirurgiczne

Leczenie chirurgiczne wstecznego wytrysku nie jest powszechnie stosowane i jest zalecane tylko wtedy, gdy inne metody nie przynoszą rezultatów.5960 Procedury chirurgiczne zazwyczaj obejmują rekonstrukcję mięśni zwieracza pęcherza.61

W niektórych przypadkach stosuje się następujące techniki chirurgiczne:6263

  • Rekonstrukcja szyi pęcherza – w przypadkach, gdy wsteczny wytrysk jest spowodowany uszkodzeniem szyi pęcherza w wyniku operacji gruczołu krokowego, rekonstrukcja szyi pęcherza może być rozważana jako opcja terapeutyczna.64
  • Przezskórną iniekcję kolagenu lub Defluxu do szyi pęcherza – metoda ta może przywrócić prawidłowy kierunek ejakulacji. Technika ta jest minimalnie inwazyjna i może umożliwić naturalne poczęcie, oszczędzając wydatki medyczne i unikając dyskomfortu związanego z technikami wspomaganego rozrodu.65

Warto zauważyć, że skuteczność leczenia chirurgicznego jest zmienna, a dowody na jego skuteczność są ograniczone do kilku małych serii przypadków z lat 80. XX wieku.66 Dlatego też przed rozważeniem interwencji chirurgicznej należy wyczerpać inne możliwości, takie jak pobranie plemników z moczu po ejakulacji lub z jąder do wykorzystania w technikach wspomaganego rozrodu.67

Indywidualizacja leczenia

Podejście do leczenia wstecznego wytrysku powinno być zindywidualizowane i dostosowane do potrzeb konkretnego pacjenta.6869 Leczenie zależy od różnych czynników, takich jak:70

  • Przyczyna wstecznego wytrysku
  • Nasilenie objawów
  • Występowanie zaburzeń erekcji
  • Plany prokreacyjne pacjenta
  • Ogólny stan zdrowia pacjenta

W przypadku wstecznego wytrysku spowodowanego przyjmowaniem leków (takich jak alfa-blokery lub niektóre leki przeciwdepresyjne), lekarz może dostosować dawkę, zmienić lek na inny lub zalecić odstawienie leku, aby przywrócić normalną ejakulację.7172 Ważne jest, aby nie przerywać przyjmowania przepisanych leków bez uprzedniej konsultacji z lekarzem.73

Jeśli wsteczny wytrysk jest spowodowany schorzeniem, takim jak cukrzyca, leczenie tego schorzenia może pomóc w złagodzeniu objawów.7475 W przypadku cukrzycy szczególnie ważna jest kontrola poziomu cukru we krwi, co może przyczynić się do poprawy funkcji nerwów i mięśni.76

Wsparcie psychologiczne

Wsparcie psychologiczne jest bardzo ważne dla wszystkich mężczyzn, niezależnie od rodzaju leczenia, jakiemu są poddawani. Brak skutecznej ejakulacji może znacznie obniżyć satysfakcję emocjonalną i fizyczną mężczyzny, co może być stresujące i wywoływać uczucia niepokoju.77

Problem wstecznego wytrysku może stanowić większy problem psychologiczny u par starających się o dziecko, dlatego wsparcie psychologiczne i emocjonalne jest bardzo istotne.78 Konsultacja z seksuologiem lub urologiem może pomóc w zrozumieniu i radzeniu sobie z tym schorzeniem, szczególnie dla mężczyzn starających się zostać ojcami.79

Zapobieganie wstecznemu wytruskowi

Chociaż nie zawsze można zapobiec wstecznemu wytrskowi, istnieją pewne środki ostrożności, które można podjąć, aby zmniejszyć ryzyko jego wystąpienia:8081

  • Jeśli przyjmujesz leki lub masz problemy zdrowotne, które narażają cię na ryzyko wstecznego wytrysku, zapytaj lekarza, co możesz zrobić, aby zmniejszyć to ryzyko

Podsumowanie leczenia wstecznego wytrysku

Wsteczny wytrysk, choć nie jest schorzeniem zagrażającym życiu, może znacząco wpływać na płodność mężczyzny i jego jakość życia. Leczenie tego zaburzenia powinno być dostosowane do indywidualnych potrzeb pacjenta, z uwzględnieniem przyczyny leżącej u podstaw problemu oraz celów prokreacyjnych.8485

Farmakoterapia może być skuteczna w przypadkach wstecznego wytrysku spowodowanego łagodnym uszkodzeniem nerwów, jednak jej skuteczność jest ograniczona, gdy wystąpiły trwałe zmiany anatomiczne.8687 Dla mężczyzn, którzy pragną mieć dzieci, a u których leczenie farmakologiczne nie przyniosło rezultatów, techniki wspomaganego rozrodu oferują realną szansę na ojcostwo.8889

Istotne jest, aby mężczyźni cierpiący na wsteczny wytrysk otrzymywali kompleksową opiekę, obejmującą zarówno aspekty medyczne, jak i psychologiczne.90 Dzięki odpowiedniemu leczeniu i wsparciu, wielu mężczyzn ze wstecznym wytryskiem może skutecznie poradzić sobie z tym schorzeniem i osiągnąć swoje cele prokreacyjne.9192
