Zapobieganie i profilaktyka

Rak kory nadnerczy (adrenocortical carcinoma) to rzadki, złośliwy nowotwór o nie do końca poznanej etiologii, z około 50% przypadków związanych z mutacjami genetycznymi. Brak jednoznacznych, modyfikowalnych czynników ryzyka uniemożliwia opracowanie specyficznych metod zapobiegania. W profilaktyce zaleca się stosowanie ogólnych zasad zmniejszających ryzyko nowotworów, takich jak unikanie używek (tytoń, alkohol – zalecane ograniczenie do maksymalnie 2 drinków dziennie u mężczyzn i 1 u kobiet, choć nie ma bezpiecznej dawki alkoholu), utrzymanie BMI w zakresie 18,5-29,4, regularna aktywność fizyczna (minimum 150 minut tygodniowo) oraz zdrowa dieta bogata w owoce i warzywa. Osoby z obciążeniem rodzinnym powinny rozważyć badania genetyczne i poradnictwo genetyczne w celu oceny ryzyka i monitorowania.

Rak nadnerczy (Adrenal cancer) – Profilaktyka i zapobieganie

Rak nadnerczy (rak kory nadnerczy, adrenocortical carcinoma) to rzadki nowotwór złośliwy wywodzący się z kory nadnerczy. Ze względu na swoją rzadkość, nie opracowano dotychczas jednoznacznych metod zapobiegania temu schorzeniu. Obecnie naukowcy nadal badają dokładne przyczyny powstawania raka nadnerczy, co utrudnia sformułowanie konkretnych wytycznych dotyczących profilaktyki.12

Brak jednoznacznych metod zapobiegania

Obecnie nie istnieją naukowo potwierdzone metody zapobiegania rakowi nadnerczy. Wynika to z faktu, że nie zidentyfikowano jednoznacznych, modyfikowalnych czynników ryzyka dla tego nowotworu. Badacze odkryli, że około połowa przypadków raka kory nadnerczy jest związana z mutacjami genetycznymi, jednak nie wiedzą, co wywołuje te mutacje, co uniemożliwia opracowanie skutecznych strategii prewencyjnych.12

Amerykański Narodowy Instytut Raka (NCI) także nie posiada opartych na dowodach informacji dotyczących zapobiegania rakowi kory nadnerczy, co podkreśla lukę w wiedzy naukowej w tym zakresie.1

Zalecenia ogólne dotyczące zdrowego stylu życia

Pomimo braku specyficznych metod profilaktyki raka nadnerczy, eksperci zalecają przestrzeganie ogólnych zasad zmniejszających ryzyko zachorowania na nowotwory. Działania te mogą potencjalnie wpłynąć na ogólne zmniejszenie ryzyka nowotworowego, w tym raka nadnerczy:12

  • Unikanie używek – całkowite powstrzymanie się od używania wyrobów tytoniowych oraz ograniczenie spożycia alkoholu. Zgodnie z zaleceniami CDC, mężczyźni powinni ograniczyć konsumpcję alkoholu do maksymalnie dwóch drinków dziennie, a kobiety do jednego. Warto jednak zaznaczyć, że nie istnieje bezpieczna ilość alkoholu jeśli chodzi o ryzyko rozwoju chorób nowotworowych – nawet małe ilości mogą zwiększać ryzyko.12
  • Utrzymanie prawidłowej masy ciała – dążenie do utrzymania BMI w przedziale 18,5-29,4, który jest uznawany za zdrowy dla większości dorosłych (choć może się różnić w zależności od składu ciała).12
  • Regularna aktywność fizyczna – zgodnie z zaleceniami Amerykańskiego Towarzystwa Kardiologicznego (AHA), dorosłe osoby powinny wykonywać co najmniej 150 minut umiarkowanej do intensywnej aktywności fizycznej tygodniowo.1
  • Zdrowa dieta – spożywanie posiłków bogatych w owoce i warzywa, a ubogich w przetworzone mięsa.12
  • Unikanie ekspozycji na karcynogeny – ograniczenie kontaktu z substancjami rakotwórczymi w miejscu pracy i środowisku, przestrzeganie zasad bezpieczeństwa przy pracy z takimi substancjami.1

Poradnictwo genetyczne i badania genetyczne

Osoby z obciążonym wywiadem rodzinnym w kierunku raka nadnerczy lub powiązanych zespołów genetycznych powinny rozważyć następujące działania:12

  • Badania genetyczne – rozważenie testów wykrywających mutacje genetyczne związane z zespołami dziedzicznymi zwiększającymi ryzyko raka nadnerczy.1
  • Poradnictwo genetyczne – konsultacja ze specjalistą genetyki klinicznej, która pomoże ocenić indywidualne ryzyko i zaplanować odpowiednie działania monitorujące.12

Wczesne wykrywanie i regularne badania

Choć nie istnieją standardowe badania przesiewowe w kierunku raka nadnerczy dla populacji ogólnej, pewne działania mogą przyczynić się do wczesnego wykrycia patologii nadnerczy:12

  • Regularne badania kontrolne – szczególnie istotne dla osób z czynnikami ryzyka, w tym obciążeniem rodzinnym.1
  • Ocena incydentaloma nadnerczy – przypadkowo wykrytych guzów nadnerczy podczas badań obrazowych jamy brzusznej lub klatki piersiowej wykonywanych z innych przyczyn. Luty jest Narodowym Miesiącem Profilaktyki Nowotworowej – jeśli kiedykolwiek wykonano u pacjenta tomografię komputerową lub rezonans magnetyczny jamy brzusznej lub klatki piersiowej, warto poprosić lekarza o sprawdzenie, czy nie występuje incydentaloma nadnerczy.1

Wszystkie krajowe i międzynarodowe wytyczne podkreślają, że u wszystkich pacjentów z incydentaloma nadnerczy należy przeprowadzić diagnostykę w kierunku nadprodukcji hormonów oraz wykonywać badania kontrolne w celu monitorowania ewentualnego wzrostu guza. Około 20% incydentaloma może mieć potencjał złośliwy, być nowotworami złośliwymi lub wydzielać nadmiar hormonów. Identyfikacja tych przypadków stanowi istotny element medycyny prewencyjnej i profilaktyki nowotworowej.1

Postępowanie w przypadku niepokojących objawów

Wczesne rozpoznanie i leczenie mogą zapobiec progresji nowotworu oraz zmniejszyć wskaźniki śmiertelności i chorobowości. Dlatego istotne jest, aby:12

  • Niezwłocznie zgłaszać się do lekarza w przypadku wystąpienia nietypowych objawów, takich jak: zwiększenie masy ciała, zmiany w dystrybucji tkanki tłuszczowej, objawy wirylizacji (maskulinizacji) lub inne niepokojące symptomy.1
  • Przeprowadzać szybką ocenę hormonalną i radiologiczną w przypadku podejrzenia patologii nadnerczy.1

Specjalistyczna opieka i postępowanie po leczeniu

W przypadku zdiagnozowania raka nadnerczy, kluczowe znaczenie ma skierowanie pacjenta do wyspecjalizowanego ośrodka, gdzie przypadek będzie omawiany interdyscyplinarnie, co pozwoli na określenie optymalnej diagnostyki, leczenia i strategii dalszej obserwacji dostosowanych indywidualnie do pacjenta.1

Najnowsze badania wnoszą coraz więcej informacji na temat optymalnego postępowania po leczeniu chirurgicznym raka kory nadnerczy. Według badania klinicznego opublikowanego w sierpniu 2023 roku w czasopiśmie The Lancet Diabetes Endocrinology, nie wszyscy pacjenci po całkowitej resekcji guza wymagają dotychczasowej standardowej terapii mitotanem. Leczenie uzupełniające mitotanem nie jest wskazane u pacjentów z rakiem kory nadnerczy o niskim stopniu złośliwości, zlokalizowanym (bez przerzutów), który został całkowicie usunięty, ponieważ ich rokowanie jest stosunkowo dobre, a leczenie mitotanem nie wykazuje statystycznie istotnej poprawy wskaźnika nawrotów, wiąże się natomiast z działaniami niepożądanymi.12

To odkrycie stanowi pierwszy krok w kierunku medycyny spersonalizowanej w leczeniu tej rzadkiej choroby. Badanie pokazuje, że możliwe jest zidentyfikowanie podgrupy pacjentów o znacznie lepszym rokowaniu niż oczekiwano, u których aktywny nadzór jest najwłaściwszym podejściem. Wykorzystuje się w tym celu prosty i szeroko dostępny schemat prognostyczny oparty na ocenie stadium zaawansowania guza, statusie resekcji i ocenie markera Ki-67.1

Kompleksowa opieka onkologiczna

Nowoczesne ośrodki onkologiczne oferują pełne spektrum opieki dla pacjentów z guzami nadnerczy – od profilaktyki i wykrywania, przez leczenie, aż po opiekę nad osobami, które przeżyły chorobę nowotworową. Specjalistyczna opieka obejmuje pomoc w ocenie i zmniejszaniu ryzyka wystąpienia guzów nadnerczy, badania przesiewowe w kierunku wczesnego wykrywania oraz kompleksowe leczenie.1
