Transwersalne zapalenie rdzenia (TZR) to poważne schorzenie neurologiczne o podłożu często infekcyjnym lub autoimmunologicznym, które może prowadzić do trwałych dysfunkcji neurologicznych. Profilaktyka obejmuje przede wszystkim minimalizację ryzyka zakażeń poprzez higienę osobistą, dezynfekcję powierzchni, szczepienia ochronne (np. przeciwko wirusowi ospy wietrznej i półpaśca) oraz unikanie kontaktu z osobami chorymi. Dodatkowo, istotne jest zarządzanie czynnikami ryzyka, takimi jak stres, palenie tytoniu oraz właściwe leczenie chorób przewlekłych, zwłaszcza autoimmunologicznych. U pacjentów z wysokim ryzykiem nawrotów stosuje się leczenie profilaktyczne, w tym leki immunomodulujące, immunosupresyjne oraz dożylne immunoglobuliny (IVIG), dostosowane do etiologii i ryzyka rozwoju stwardnienia rozsianego (SM) lub choroby Devica (NMO).

Profilaktyka i zapobieganie transwersalnemu zapaleniu rdzenia

Transwersalne zapalenie rdzenia (TZR) to poważne schorzenie neurologiczne, które może prowadzić do znaczących dysfunkcji neurologicznych. Mimo że nie istnieje jednoznaczna metoda zapobiegania temu schorzeniu, istnieją działania profilaktyczne, które mogą zmniejszyć ryzyko jego wystąpienia lub nawrotu, zwłaszcza w przypadkach, gdy choroba ma podłoże infekcyjne lub autoimmunologiczne.123

Profilaktyka pierwszorzędowa

Ponieważ transwersalne zapalenie rdzenia często rozwija się po ostrych infekcjach, podstawowym elementem profilaktyki jest minimalizacja ryzyka zakażeń bakteryjnych i wirusowych.45 Do kluczowych działań należą:

  • Utrzymywanie wysokiego poziomu higieny osobistej – regularne mycie rąk i unikanie dotykania twarzy nieumytymi rękami67
  • Dezynfekcja często dotykanych powierzchni w celu zmniejszenia ryzyka transmisji patogenów8
  • Szczepienia ochronne zgodnie z zaleceniami lekarza, które mogą zapobiec niektórym infekcjom wiązanym z TZR, np. wirus ospy wietrznej i półpaśca910
  • Unikanie bliskiego kontaktu z osobami chorymi11

Warto zaznaczyć, że chociaż szczepionki przeciwko chorobom zakaźnym rzadko mogą być potencjalnym czynnikiem wyzwalającym, nie oznacza to, że należy ich unikać, ponieważ jest to jedynie rzadka możliwość.12

Zarządzanie czynnikami ryzyka

Świadome zarządzanie czynnikami ryzyka może zmniejszyć prawdopodobieństwo wystąpienia transwersalnego zapalenia rdzenia.13 Kluczowe działania obejmują:

  • Regularna aktywność fizyczna, która pomaga utrzymać ogólne zdrowie rdzenia kręgowego14
  • Zarządzanie stresem poprzez techniki relaksacyjne, jogę, medytację lub aktywność fizyczną – długotrwały stres wiąże się z wyższym ryzykiem chorób autoimmunologicznych, w tym TZR15
  • Unikanie palenia tytoniu, które zwiększa ryzyko chorób autoimmunologicznych, w tym TZR16
  • Właściwe leczenie i monitorowanie chorób przewlekłych, szczególnie zaburzeń autoimmunologicznych1718

Profilaktyka wtórna u pacjentów wysokiego ryzyka

U pacjentów, u których zdiagnozowano schorzenie mogące prowadzić do TZR lub u których wystąpił już jeden epizod, stosuje się leczenie profilaktyczne mające na celu zapobieganie kolejnym atakom.1920

Leczenie immunomodulujące w profilaktyce

W zależności od podłoża TZR, stosowane są różne strategie terapeutyczne:21

Leczenie przeciwdrobnoustrojowe w profilaktyce

W przypadkach, gdy podejrzewa się infekcyjne podłoże TZR, istotne jest wdrożenie odpowiedniego leczenia przeciwdrobnoustrojowego:25

Ze względu na potencjalne korzyści z zalecanej terapii, u pacjentów z rozpoznaniem zapalenia poprzecznego rdzenia i historią kontaktu z wodą na obszarach endemicznych, należy rozpocząć leczenie przypuszczalne podczas oczekiwania na wyniki badań parazytologicznych lub serologicznych.28

Wczesna interwencja terapeutyczna

Szybkie rozpoznanie i wdrożenie odpowiedniego leczenia może znacząco zmniejszyć ryzyko trwałych powikłań neurologicznych w przebiegu TZR.2930 Standardowe postępowanie obejmuje:

  • Wysokie dawki kortykosteroidów – stosowane w celu stłumienia aktywności układu immunologicznego oraz zmniejszenia obrzęku i stanu zapalnego31
  • Plazmafereza (terapia wymiany osocza) – procedura polegająca na wymianie osocza pacjenta na specjalne płyny, które usuwają z krwi szkodliwe przeciwciała i białka powodujące stan zapalny; stosowana, gdy kortykosteroidy nie przynoszą ulgi w objawach32
  • Leki przeciwbólowe – takie jak ibuprofen lub acetaminofen33
  • Leki immunosupresyjne – hamujące nieprawidłową odpowiedź immunologiczną34
  • Specjalistyczne leki ukierunkowane na ból neuropatyczny35

W zależności od przyczyny TZR, kluczowe jest zidentyfikowanie i leczenie pierwotnego schorzenia.36 Gdy nie można ustalić przyczyny, należy rozważyć zastosowanie kortykosteroidów i plazmaferezy.37

Rehabilitacja jako element profilaktyki trzeciorzędowej

W zależności od rodzaju i nasilenia skutków transwersalnego zapalenia rdzenia, pacjent może wymagać krótko- lub długoterminowej fizjoterapii.38 W niektórych przypadkach konieczny może być okres specjalistycznej opieki w ośrodku rehabilitacyjnym.39 Kompleksowa rehabilitacja stanowi istotny element profilaktyki trzeciorzędowej, mającej na celu zapobieganie lub minimalizowanie długotrwałych powikłań neurologicznych.40

Podsumowanie działań profilaktycznych

Chociaż nie istnieje skuteczna metoda całkowitego zapobiegania transwersalnemu zapaleniu rdzenia, połączenie różnych strategii profilaktycznych może zmniejszyć ryzyko jego wystąpienia lub nawrotu oraz poprawić jakość życia pacjentów z tym schorzeniem.4142 Kluczowe znaczenie ma prawidłowa diagnoza i leczenie przez specjalistę, które są niezbędne do skutecznego zarządzania tą chorobą.43

Warto podkreślić, że leczenie zawsze obejmuje zarówno doraźne (krótkoterminowe) leczenie stanu zapalnego i jego przyczyny, jak i długoterminowe zarządzanie powikłaniami.44 Dzięki lepszemu zrozumieniu schorzenia oraz wykorzystaniu dostępnych zasobów i metod leczenia, osoby cierpiące na to zaburzenie mogą rozpocząć kroki w kierunku lepszego zarządzania swoim stanem i znalezienia fizycznej i emocjonalnej ulgi od objawów.45
