Rak dróg żółciowych (cholangiocarcinoma)
Objawy
Pięcioletnie przeżycie w przypadku raka ograniczonego do dróg żółciowych wynosi 18-23%, natomiast w zaawansowanych stadiach spada do 2-3%, z wyjątkiem wczesnych wykryć, gdzie wskaźnik przeżycia może sięgać 80-100%.

- Zmniejszony apetyt
Zmiany w kolorze stolca i moczu
Utrata masy ciała i apetytu
Inne objawy systemowe
Do innych objawów zaawansowanego raka dróg żółciowych należą:
- Zmęczenie – przewlekłe zmęczenie jest powszechnym objawem wynikającym z kombinacji stanu zapalnego związanego z nowotworem, niedoborów żywieniowych i niezdolności organizmu do efektywnego metabolizowania składników odżywczych26
- Poty nocne2728
- Niezamierzonej utraty masy ciała i zmęczenia
- Powiększenia wątroby (hepatomegalia)
- Łagodnej żółtaczki (w późniejszych stadiach)
- Wodobrzusza z powodu rozsiewu nowotworowego do otrzewnej w zaawansowanych przypadkach
Rak wnękowo-zewnątrzwątrobowy
Ten typ raka występuje w miejscu, gdzie przewody żółciowe wychodzą z wątroby i łączą się, tworząc wspólny przewód żółciowy4. Objawy obejmują:
- Wczesną żółtaczkę – jest to często pierwszy objaw
- Świąd skóry
- Ciemny mocz i jasne stolce
- Jadłowstręt i niedobory żywieniowe spowodowane cholestazą i zaburzeniami wchłaniania
- Wyczuwalny pęcherzyk żółciowy (objaw Courvoisiera) – może wystąpić w przypadku guzów zlokalizowanych dystalnie do przewodu pęcherzykowego
Progresja choroby
Etapy progresji
Progresja raka dróg żółciowych obejmuje następujące etapy:
- Wczesne stadium – guz ograniczony do dróg żółciowych, zwykle bezobjawowy3
- Przerzuty regionalne – zajęcie okolicznych narządów (wątroba, jelita, żołądek, przepona) i węzłów chłonnych6
Objawy progresji choroby
W miarę postępu choroby objawy stają się bardziej nasilone i mogą obejmować9:
- Nasilającą się żółtaczkę
- Uporczywy ból brzucha
- Znaczną utratę masy ciała
- Wodobrzusze (gromadzenie się płynu w jamie brzusznej)10
- Powiększenie wątroby, śledziony lub pęcherzyka żółciowego11
- Niewydolność wątroby w zaawansowanych stadiach12
- Ból kości lub powikłania oddechowe w przypadku przerzutów13
- Hiperkalcemię i kacheksję w zaawansowanym stadium choroby14
Rokowanie i przeżycie
Rokowanie w raku dróg żółciowych jest zwykle niekorzystne, szczególnie w przypadkach zaawansowanej choroby1. Czynniki wpływające na rokowanie obejmują:
- Stadium nowotworu w momencie rozpoznania
- Lokalizację guza (nowotwory wewnątrzwątrobowe mają gorsze rokowanie)2
- Możliwość całkowitego usunięcia chirurgicznego3
- Wiek i ogólny stan zdrowia pacjenta
- Żółtaczkę (zażółcenie skóry i białkówek oczu)
- Intensywny świąd skóry
- Jasne, białawe stolce
- Ciemny mocz

Cholangiocarcinoma, a cancer of the bile ducts, is a rare and aggressive tumor. […] The actual number of cases is probably underestimated, since this form of cancer is difficult to diagnose. It is sometimes wrongly diagnosed as another type of cancer. […] Several factors increase the risk of developing a bile duct cancer. Frequently, they are related with a history of previous disease or inflammation of the bile ducts. Other risk factors also include age (patients are on average 70 years old), obesity, diabetes, and exposure to certain types of chemicals. […] Bile duct cancer is often diagnosed late because many patients display no signs or symptoms. The first signs of the disease are often non-specific and appear when the tumor is large and compresses the large bile ducts, which can cause jaundice (yellowing of the skin) in extrahepatic cholangiocarcinoma. Other symptoms may appear, such as itching, pale stools, dark urine, abdominal pain, weight loss, and fever. However, these symptoms are not specific to the diseases and may lead to a delayed cancer diagnosis. […] Surgical resection is the only potential option for curing cholangiocarcinoma. Targeted therapies and immunotherapy represent new hope for patients.

So far there are no routine screening tests to detect bile duct cancer (cholangiocarcinoma). The bile ducts are deep inside the body, so abnormalities are difficult to detect in a routine physical examination. Only a small number of these cancers are diagnosed before they have spread beyond the point where they can be removed surgically.

These cancers are often difficult to diagnose, their pathogenesis is poorly understood, and their dismal prognosis has resulted in a nihilistic approach to their management. The two major clinical phenotypes are intrahepatic, mass-forming tumors and large ductal tumors. Among the ductal cancers, lesions at the liver hilum are most prevalent. […] Patients with cholangiocarcinoma usually present at late stages of the disease, and symptoms might be nonspecific, such as painless jaundice, weight loss or cholangitis. Therefore, these cancers remain difficult to diagnose and treat and their prognosis is generally poor. Approximately half of untreated patients die within 34 months of presentation from the indirect effects of local tumor progression, bile duct obstruction, liver failure or sepsis from cholangitis and abscesses. The management of these patients is palliative, aimed at reducing obstructive cholestasis, pruritus and cholangitis.

Cholangiocarcinoma is a rare and often fatal cancer that affects the bile ducts. […] Your symptoms may vary depending on the location of your tumor, but they can include the following: Jaundice, which is yellowing of the skin, is the most common symptom. This may develop at an early or late stage, depending on the tumor site. […] You can have pain in your abdomen that penetrates to your back. This tends to occur as the cancer progresses. […] Additional rare but serious side effects may include enlargement of your liver, spleen, or gallbladder. […] Many people aren’t eligible for surgery that removes the tumor by removing all or part of the liver or bile duct. This can be because the cancer is too advanced, has already metastasized, or is in an inoperable location.

Presenting symptoms of patients with early bile duct cancer are characteristic, in that they rarely complain of abdominal pain or jaundice typically associated with advanced bile duct cancer patients. Thirty nine percent of early bile duct cancer patients were asymptomatic and 84% of patients were non-jaundiced. […] Early bile duct cancer patients should have markedly better surgical outcomes than advanced bile duct cancer patients. In this regard, the reported 5-year survival rate for patients with early bile duct cancer has been promising. Mizumoto et al reported a 5 year survival rate for early bile duct cancer of 100%, and Kurosaki et al reported a 5-year survival rate for seven patients with pT1 bile duct cancer of 86%. Recently, we also demonstrated a 5-year survival rate for early bile duct cancer patients of 80%. Therefore, we believe that detection of early bile duct cancer may provide the opportunity for better prognosis in bile duct cancer.

Unexplained weight loss may be an early sign of bile duct cancer. There are several reasons for a patient to have unintentional weight loss during bile duct cancer. First, patients with bile duct cancer have a low appetite, leading to reduced calorie intake. Further, the absence or reduced level of bile in the small intestine (because of bile duct obstruction) may interfere with fat absorption. […] Loss of appetite may also be a cholangiocarcinoma cancer symptom. It may occur both before and after the diagnosis. Before the diagnosis, patients do not feel hungry due to interference with liver-assisted metabolism and inadequate fat absorption. This interference may lead to difficulty in the absorption of various nutrients. After the cancer diagnosis, poor psychological health plays a role in losing appetite.
- #24 Cholangiocarcinoma – Wikipediahttps://en.wikipedia.org/wiki/Cholangiocarcinoma
Cholangiocarcinoma, also known as bile duct cancer, is a type of cancer that forms in the bile ducts. Symptoms of cholangiocarcinoma may include abdominal pain, yellowish skin, weight loss, generalized itching, and fever. Light colored stool or dark urine may also occur. The most common physical indications of cholangiocarcinoma are abnormal liver function tests, jaundice (yellowing of the eyes and skin occurring when bile ducts are blocked by tumor), abdominal pain (30-50%), generalized itching (66%), weight loss (30-50%), fever (up to 20%), and changes in the color of stool or urine. To some extent, the symptoms depend upon the location of the tumor: people with cholangiocarcinoma in the extrahepatic bile ducts (outside the liver) are more likely to have jaundice, while those with tumors of the bile ducts within the liver more often have pain without jaundice. Cholangiocarcinoma is typically incurable at diagnosis which is why early detection is ideal. In these cases palliative treatments may include surgical resection, chemotherapy, radiation therapy, and stenting procedures. In about a third of cases involving the common bile duct and less commonly with other locations the tumor can be completely removed by surgery offering a chance of a cure. Even when surgical removal is successful chemotherapy and radiation therapy are generally recommended. Even when surgery is successful the 5-year survival is typically less than 50%. The prognosis may be worse for people with primary sclerosing cholangitis who develop cholangiocarcinoma, likely because the cancer is not detected until it is advanced.
