Etiologia i przyczyny

Hematospermia, definiowana jako obecność krwi w ejakulacie, najczęściej ma etiologię infekcyjną lub zapalną, stanowiącą około 40% przypadków, zwłaszcza u mężczyzn poniżej 40. roku życia. Do najczęstszych patogenów należą bakterie (np. Escherichia coli), wirusy (CMV, HIV) oraz patogeny przenoszone drogą płciową (rzeżączka, chlamydia). Jatrogenne przyczyny, takie jak biopsja prostaty (występująca u 80% pacjentów po zabiegu) czy TURP, również często prowadzą do hematospermii, która może utrzymywać się do 4 tygodni. U mężczyzn powyżej 40. roku życia dominują przyczyny związane z gruczołem krokowym, takie jak BPH, zapalenie prostaty, kamienie czy torbiele, a także rzadziej nowotwory (około 3,5% przypadków), w tym rak prostaty, jądra i pęcherza moczowego. Diagnostyka powinna uwzględniać także czynniki ogólnoustrojowe, takie jak nadciśnienie tętnicze, zaburzenia krzepnięcia oraz stosowanie leków przeciwkrzepliwych.

Krwawienie w nasieniu – Etiologia, przyczyny, czynniki wywołujące

Krwawienie w nasieniu (hematospermia) to obecność krwi w ejakulacie. Stan ten może być alarmujący dla pacjenta, jednak w większości przypadków nie stanowi poważnego problemu zdrowotnego i często ustępuje samoistnie12. Hematospermia może występować jako niewielka ilość krwi widoczna tylko pod mikroskopem lub jako wyraźnie widoczna krew w ejakulacie3.

Brak ustalonej przyczyny

W około 15-70% przypadków nie udaje się ustalić jednoznacznej przyczyny krwawienia w nasieniu45. Wiele z tych przypadków jest samoograniczających się, co oznacza, że krew w nasieniu ustępuje samoistnie bez interwencji medycznej6. W przypadkach idiopatycznych krwawienie zwykle ustępuje w ciągu 1-2 miesięcy7.

Infekcje i stany zapalne

Infekcje i stan zapalny są najczęstszymi zidentyfikowanymi przyczynami krwawienia w nasieniu, odpowiadając za około 40% przypadków89. Szczególnie u mężczyzn poniżej 40. roku życia, infekcje układu moczowo-płciowego są najczęstszą przyczyną1011. Do najczęstszych infekcji prowadzących do hematospermii należą:

Stan zapalny może dotyczyć gruczołu krokowego, pęcherzyków nasiennych, cewki moczowej lub innych narządów układu rozrodczego i moczowego22. Proces zapalny powoduje przekrwienie i obrzęk przewodów, pęcherzyków nasiennych, prostaty i cewki moczowej, prowadząc do krwawienia23.

Przyczyny jatrogenne i urazy

Procedury medyczne są jedną z najczęstszych przyczyn krwawienia w nasieniu24. Do najczęstszych przyczyn jatrogennych należą:

  • Biopsja gruczołu krokowego – najczęstsza przyczyna jatrogenna, może prowadzić do hematospermii u nawet 80% pacjentów; krew może utrzymywać się w nasieniu przez okres do 4 tygodni2526

  • Rak gruczołu krokowego – najczęstszy nowotwór mogący powodować hematospermię, choć rzadko jest jej pierwszym objawem5354
