Postępowanie obejmuje:

Nadciśnienie płucne

Nadciśnienie płucne jest częstym powikłaniem zespołu POEMS i wiąże się z gorszym rokowaniem47. Wszyscy pacjenci z zespołem POEMS powinni być oceniani lub badani w kierunku nadciśnienia płucnego za pomocą echokardiografii, szczególnie ci z objawami przewodnienia48.

Aktywne leczenie, w tym MDex (melfalan, deksametazon), ASCT i nowe leki, może poprawić rokowanie w tej grupie pacjentów49.

Zespoł przecieku włośniczkowego

Większość pacjentów z zespołem POEMS prezentuje objawy przewodnienia i anasarka, głównie przypisywane aktywności VEGF w surowicy i jego wkładowi do przecieku włośniczkowego5051.

Postępowanie obejmuje:

Po zakończeniu aktywnego leczenia, pacjenci wymagają regularnej, długoterminowej obserwacji57. Może to obejmować:

  • Regularne wizyty kontrolne (co 3 miesiące przez pierwsze kilka lat)
  • Ocenę funkcji neurologicznych
  • Monitorowanie parametrów laboratoryjnych (białko monoklonalne, VEGF)

Rokowanie

Rokowanie w zespole POEMS zależy od wielu czynników, w tym od czasu od wystąpienia objawów do rozpoznania, stopnia zaawansowania choroby w momencie rozpoznania oraz odpowiedzi na leczenie58.

Przewlekłe i nieodwracalne upośledzenie funkcjonalne po leczeniu jest częste i wymaga ciągłego monitorowania i wsparcia62.

Przypadki atypowe i wyzwania diagnostyczne

Zespół POEMS jest chorobą o bardzo zróżnicowanym obrazie klinicznym, co może prowadzić do opóźnień w diagnostyce i leczeniu63.

Atypowe manifestacje kliniczne

W literaturze opisywane są przypadki atypowych manifestacji zespołu POEMS, które mogą stanowić wyzwanie diagnostyczne64:

  • Dysfunkcja autonomiczna – mimo że zachowanie funkcji autonomicznych jest typowe dla zespołu POEMS, opisywano przypadki z nasilonymi objawami dysautonomii, w tym zaburzeniami funkcji seksualnych, nietolerancją ciepła, ciężkim niedociśnieniem tętniczym, tachykardią, zatrzymaniem moczu, przewlekłymi zaparciami i kserostomią6566
  • Obrzęk tarczy nerwu wzrokowego (tarcza zastoinowa) – obecny u co najmniej jednej trzeciej pacjentów i może być związany ze zwiększonym ciśnieniem wewnątrzczaszkowym6768

Wyzwania diagnostyczne

Zespół POEMS jest często błędnie diagnozowany jako przewlekła zapalna polineuropatia demielinizacyjna (CIDP) w około 60% przypadków6970. Opóźnienie w diagnozie może prowadzić do progresji choroby i gorszego rokowania71.

Aby poprawić wczesną diagnostykę, zaleca się72:

  • Badanie poziomu VEGF u pacjentów z nabytą neuropatią demielinizacyjną
  • Dokładną diagnostykę obrazową (nie tylko przeglądowe zdjęcia rentgenowskie, ale także badania CT lub MRI) w poszukiwaniu zmian kostnych
  • Szczegółową ocenę endokrynologiczną
  • Badanie immunoelektroforetyczne w kierunku obecności białka monoklonalnego

    The POEMS syndrome treatment guidelines proposed by Drs. Dispenzieri and Buadi, based on the number of osteosclerotic bone lesions and the presence or absence of bone marrow infiltration, are very easy to use. Examination of the bone marrow is critical to determine whether infiltration of bone has occurred, given that irradiation is essential if a bone lesion is found. In the case of localized disease, irradiation may induce resolution of symptoms and, eventually, cure. In contrast, when irradiation is not possible, high-dose chemotherapy with stem cell support is the treatment of choice in fit patients, and achieves a nearly 100% response. Alternatively, drugs developed for the treatment of plasmacytic proliferation may be used. Lenalidomide seems to be the more promising therapy, because of its anti-proliferative and anti-angiogenic effects and lack of major neurotoxicity. In small series, a significant proportion of patients have responded to this treatment.

    POEMS syndrome is a rare, but treatable cause of neuropathy. Further work is required to establish its exact underlying pathophysiology. Current treatment approaches afford good prognosis. Moving forward, randomised-controlled studies, though difficult given the rarity of POEMS syndrome, and the development of prognostic tools, will be important in establishing individualised approaches to patient care.
