Opcje niefarmakologiczne w leczeniu drżenia esencjalnego
Metody fizyczne i terapie zajęciowe
U niektórych pacjentów drżenie można zmniejszyć poprzez obciążenie kończyny, zazwyczaj stosując obciążniki nadgarstkowe. U niewielkiej części pacjentów może to wystarczająco stłumić drżenie, aby zapewnić ulgę lub poprawić funkcjonowanie7. Dostępnych jest kilka rozwiązań technologicznych, które mogą pomóc stabilizować używanie przyborów, takich jak ważone sztućce lub technologia aktywnego tłumienia drżenia8.

Leczenie farmakologiczne drżenia esencjalnego
Leki pierwszego rzutu
Podejście terapeutyczne do drżenia esencjalnego często opiera się na metodzie prób i błędów, a pacjenci powinni wypróbować kilka leków, jeśli pierwszy wybór jest nieskuteczny lub wiąże się z osłabiającymi działaniami niepożądanymi19. Primidone (Mysoline) jest lekiem przeciwdrgawkowym, który wykazuje porównywalną skuteczność do propranololu25. Został zbadany w 8 badaniach poziomu I, które obejmowały łącznie 274 pacjentów z drżeniem esencjalnym, porównując primidone z placebo i został sklasyfikowany jako skuteczny i klinicznie użyteczny26. Leki drugiego rzutu

Topiramat (Topamax) jest lekiem przeciwdrgawkowym, który okazał się skuteczny w leczeniu drżenia esencjalnego29.
Gabapentyna (Neurontin) jest lekiem przeciwdrgawkowym, który wykazał mieszane wyniki w leczeniu drżenia esencjalnego, zarówno jako monoterapia, jak i terapia dodana30.
Alprazolam (Xanax) jest krótkodziałającą benzodiazepina, która wykazała skuteczność w leczeniu drżenia esencjalnego w 2 badaniach poziomu I31.
Leki trzeciego rzutu
Terapie te opierają się na badaniach otwartych lub seriach przypadków. Leki z tej klasy obejmują nimodypinę i klozapinę32.
Nimodypina, bloker kanału wapniowego stosowany w leczeniu skurczów naczyniowych wtórnych do krwawienia podpajęczynówkowego, była badana w leczeniu drżenia esencjalnego33.
Leczenie skojarzone
Gdy monoterapia propranololem lub primidone nie kontroluje odpowiednio drżenia kończyn, te dwa leki można stosować w połączeniu34. U pacjentów, którzy niedostatecznie reagują na monoterapię, rozważenie politerapii może przynieść dodatkowe korzyści. Łączenie leków może działać na różne mechanizmy i potencjalnie zwiększać ogólną kontrolę drżenia35.
Inne opcje farmakologiczne
Niektórzy pacjenci z drżeniem esencjalnym zgłaszają złagodzenie objawów po wypiciu kieliszka wina lub koktajlu36. Około połowa wszystkich osób z drżeniem esencjalnym stwierdza, że alkohol jest pomocny w zmniejszaniu drżenia. Dwie jednostki alkoholu mogą stłumić drżenie na około cztery godziny. Jednakże zbyt duża ilość alkoholu może nasilić drżenie i musi być stosowana z ostrożnością, aby uniknąć rozwoju problemu z alkoholem37.
Jeśli drżenie nasila się pod wpływem napięcia lub lęku, niefarmakologiczne techniki relaksacyjne i biofeedback mogą być skuteczne u niektórych pacjentów38.
Leczenie iniekcjami toksyny botulinowej
U niektórych pacjentów z ciężkim drżeniem głowy lub rąk, iniekcje z toksyny botulinowej mogą być pomocne39. Toksyna botulinowa (BoNT) jest wprowadzana do drżących mięśni, aby tymczasowo osłabić mięśnie i zmniejszyć nasilenie drżenia40.
BoNT należy rozważyć jako opcję leczenia drżenia rąk u pacjentów, którzy nie reagują na leki doustne. Niedawny przegląd oparty na dowodach wykazał niewystarczające dowody, aby stwierdzić użyteczność BoNT w leczeniu drżenia głowy i głosu41.
Zastrzyki z toksyny botulinowej mogą poprawić drżenie nawet na trzy miesiące42. Iniekcje toksyny botulinowej do ramienia powodują również osłabienie ramienia, co zwykle nie jest tolerowane. Jednak zastrzyki z toksyny botulinowej mogą działać naprawdę dobrze, jeśli drżenie esencjalne dotyczy głowy i szyi43.
Zabiegi inwazyjne w leczeniu drżenia esencjalnego
Głęboka stymulacja mózgu (DBS)
Dla pacjentów z ciężkim drżeniem, którzy nie reagują na leczenie farmakologiczne lub nie tolerują działań niepożądanych, operacja może być opcją44. Głęboka stymulacja mózgu (DBS) jest najczęściej stosowanym leczeniem chirurgicznym drżenia esencjalnego i wykazuje wysoką skuteczność45.
DBS polega na umieszczeniu niewielkiego urządzenia zwanego neurostymulatorem, który dostarcza impulsy elektryczne do określonej części mózgu. Impulsy elektryczne przerywają sygnały, które powodują ruchy mięśni przy drżeniu esencjalnym. Może to poprawić drżenie po obu stronach ciała46.
Podczas DBS elektryczna stymulacja jest dostarczana do mózgu przez elektrodę wszczepioną głęboko do brzuszno-pośredniego jądra (VIM) wzgórza. Najczęściej odbywa się to poprzez wszczepienie 4 elektrod do VIM przy użyciu metod stereotaktycznych. Skomputeryzowane programowanie generatora impulsów jest najczęściej wykonywane za pomocą przenośnego urządzenia po opuszczeniu szpitala przez pacjenta w celu optymalizacji montażu elektrod, napięcia, częstotliwości impulsów i szerokości impulsu47.
Głęboka stymulacja mózgu może być wykonywana jednostronnie lub obustronnie, w zależności od objawów pacjenta. Istnieje zwiększone ryzyko trudności z mową i równowagą w przypadku zabiegów obustronnych. Jeśli drżenie znacząco wpływa na obie ręce, to preferowana jest strona dominująca, ale można rozważyć zabiegi obustronne48.
Większość serii donosi o 70% do 90% kontroli drżenia rąk po DBS49. Według badań, pacjenci, którzy przeszli DBS, wykazują poprawę drżenia o 60%-90%50. 98% z ponad 200 000 osób, które przeszły zabieg DBS, poleciłoby go innym51.
Główną zaletą tej terapii jest to, że nie powoduje trwałych uszkodzeń mózgu, z wyjątkiem niewielkiego obszaru, w którym umieszczona jest elektroda52. DBS jest odwracalne, regulowane i nie niszczy tkanki53.
Zogniskowany ultradźwięk (MRgFUS)
Zatwierdzony przez FDA w 2016 roku, kierowana rezonansem magnetycznym (MRI) wysokointensywna zogniskowana talamotomia ultradźwiękowa jest innowacyjną metodą leczenia drżenia esencjalnego54. Jest to zabieg nieinwazyjny, który wykorzystuje fale ultradźwiękowe skupione na pojedynczym punkcie w mózgu zaangażowanym w powstawanie drżenia, niszcząc celowaną tkankę. To zniszczenie może znacznie zmniejszyć nasilenie drżenia i poprawić stabilność ręki55.
Zogniskowany ultradźwięk jest leczeniem, w którym fale dźwiękowe są skupiane przez czaszkę do celu bez potrzeby nacięć, implantów mózgowych lub promieniowania56. Specjaliści mogą stosować zogniskowane ultradźwięki do leczenia drżenia po obu stronach ciała57.
Technika ta wykorzystuje fale ultradźwiękowe, które bezpiecznie przechodzą przez czaszkę do mózgu bez potrzeby nacięcia chirurgicznego58. Zogniskowany ultradźwięk kierowany MR (MRgFUS) używa wysokointensywnych fal ultradźwiękowych skupionych na określonym celu w mózgu. Cała procedura jest przeprowadzana wewnątrz skanera obrazowania MRI, dzięki czemu lekarz może łatwo planować, kierować i celować w dany obszar59.
Badania wykazują 73% poprawy drżenia w pięcioletniej obserwacji, pokazując nie tylko natychmiastowe, ale także trwałe wyniki60. Randomizowane, podwójnie zaślepione badanie wykazało, że 56 pacjentów, którzy otrzymali leczenie, doświadczyło prawie 50% poprawy drżenia i funkcji motorycznych po trzech miesiącach i utrzymało 40% poprawy po roku61.
Jako procedura niechirurgiczna, ambulatoryjna, pacjenci mogą spodziewać się powrotu do normalnej aktywności w ciągu kilku dni62. Większość pacjentów jest w stanie odstawić leki przeciwdrżeniowe po leczeniu63.
FDA zatwierdziła etapowe leczenie obustronne drżenia esencjalnego 20 grudnia 2022 r. Zabiegi są zatwierdzane dla drugiej strony co najmniej 9 miesięcy po początkowym leczeniu64. Ostatnie badanie wykazało, że etapowe obustronne leczenie zogniskowanym ultradźwiękiem drżenia esencjalnego jest bezpieczne i skuteczne65.
Inne zabiegi chirurgiczne
Talamotomia: Stereotaktyczne techniki chirurgiczne mogą wytworzyć uszkodzenie w brzuszno-pośrednim (VIM) jądrze wzgórza66. W tej procedurze wzgórze po jednej stronie mózgu jest niszczone. Tkanka mózgowa jest zwykle niszczona przy użyciu specjalnego ultradźwięku lub wiązek promieniowania. Wykazano, że jest bardzo skuteczna. Zatrzymuje lub znacznie zmniejsza drżenie u 9 z 10 osób z drżeniem esencjalnym67.
Radiochirurgia nożem gamma: Nóż gamma skupia wysokoenergetyczne promienie gamma na brzuszno-pośrednim jądrze, powodując śmierć neuronów. Jest to nieudowodnione leczenie, które nie zostało generalnie przyjęte ze względu na obawy dotyczące potencjalnych skutków ubocznych promieniowania, w tym teoretycznego, długoterminowego ryzyka powstawania wtórnych guzów68.
Pallidotomia: Komputerowo wspomagana neurochirurgia, mająca na celu zmniejszenie drżenia, sztywności i innych objawów poprzez zniszczenie obszarów mózgu, które powodowały te objawy69.
Wskazania do leczenia inwazyjnego drżenia esencjalnego
Pacjenci z drżeniem esencjalnym są uważani za kandydatów do głębokiej stymulacji mózgu, jeśli ich drżenia są niekontrolowane pomimo odpowiednich prób kilku leków70. Pacjent może być kandydatem do DBS, jeśli71:
- Leki nie kontrolują drżenia esencjalnego lub powodują poważne działania niepożądane
- Drżenie wpływa na jakość życia
- Objawy występują od co najmniej trzech lat
- Brak znaczących problemów z myśleniem i pamięcią
Zogniskowany ultradźwięk może być opcją leczenia dla pacjentów z drżeniem esencjalnym, którzy nie reagują na leki72. Leczenie jest przeznaczone dla osób z potwierdzonym rozpoznaniem drżenia esencjalnego, które nie reagowało na leki. Pacjenci muszą mieć ponad 22 lata73.
Kandydaci do MRgFUS muszą mieć co najmniej 22 lata, mieć potwierdzoną diagnozę drżenia esencjalnego lub drżenia dominującego w chorobie Parkinsona i nie reagować lub nie kwalifikować się do co najmniej dwóch standardowych leków74.
Pacjenci mogą nie kwalifikować się do zogniskowanego ultradźwięku, jeśli mają rozrusznik serca, chorobę nerek lub nie mogą mieć badania MRI75.
Porównanie metod leczenia drżenia esencjalnego
Amerykańska Akademia Neurologii (AAN) kategoryzuje leczenie drżenia esencjalnego zgodnie z skutecznością i bezpieczeństwem w następujące kategorie: Poziom A, skuteczne; Poziom B, prawdopodobnie skuteczne; Poziom C, możliwie skuteczne; Poziom U, niewystarczające dowody76.
Skuteczność leczenia farmakologicznego
Propranolol i primidone są kamieniami węgielnymi podtrzymującego leczenia medycznego drżenia esencjalnego. Leki te zapewniają dobrą korzyść, zmniejszając amplitudę drżenia u około 50-70% pacjentów77. Propranolol wykazuje największą poprawę w drżeniu kończyn górnych78.
Jeśli oba leki nie są skuteczne samodzielnie, kombinacje obu mogą zapewnić ulgę wybranym pacjentom79. Zarówno propranolol, jak i primidone wykazały swoją użyteczność w leczeniu drżenia esencjalnego80.
Skuteczność metod inwazyjnych
Ze względu na ograniczenia terapii farmakologicznych, głęboka stymulacja mózgu i zogniskowany ultradźwięk kierowany MRI stały się coraz bardziej atrakcyjnymi opcjami dla osób z drżeniem esencjalnym81.
Głęboka stymulacja mózgu (DBS) może znacznie zmniejszyć drżenie rąk u osób z drżeniem esencjalnym82. Do 90% pacjentów z drżeniem esencjalnym ma co najmniej umiarkowaną ulgę w objawach po DBS83.

Theres no cure for essential tremor, but there are ways to treat it. The main treatment options are medications, assistive devices, botulinum toxin, deep brain stimulation and focused ultrasound. […] Treatment of ET is symptomatic, and no disease-modifying therapies are available. The degree of tremor control provided by medications varies among patients and often depends on the dose of the agents used, tremor severity, and a patient’s individual response to the chosen agent. Patients with mild tremor may experience complete tremor suppression with available therapies. However, the goal of therapy for most patients is to reduce tremor severity enough to prevent disability while minimizing medication side effects.
