Drżenie esencjalne (DE) to najczęstsze zaburzenie ruchu, charakteryzujące się mimowolnymi, rytmicznymi drganiami, głównie kończyn górnych, o średnim rocznym wzroście nasilenia od 1,5% do 5%. Około 50% przypadków ma podłoże autosomalnie dominujące. DE jest często mylone z chorobą Parkinsona, jednak różni się charakterystyką drżenia. Czynniki ryzyka, takie jak wiek i genetyka, są niemodyfikowalne, a profilaktyka rozwoju choroby jest niemożliwa. Zarządzanie obejmuje ograniczenie kofeiny, unikanie alkoholu i nikotyny, kontrolę stresu, odpowiednią higienę snu oraz regularną aktywność fizyczną i dietę. Monitorowanie funkcji tarczycy jest istotne, gdyż jej dysfunkcje mogą nasilać objawy. Leki przeciwdepresyjne i nadmierna terapia zastępcza tarczycy mogą pogarszać drżenie, dlatego konieczna jest konsultacja lekarska przed zmianą terapii.

Profilaktyka drżenia esencjalnego

Drżenie esencjalne (DE) to najczęściej występujące zaburzenie ruchu, które charakteryzuje się mimowolnymi, rytmicznymi drganiami, najczęściej dotyczącymi kończyn górnych. Około 50% przypadków ma charakter dziedziczny i jest przekazywanych jako cecha autosomalnie dominująca 1. Choroba jest często mylnie utożsamiana z chorobą Parkinsona, choć charakterystyka drżenia jest inna 2.

Możliwości zapobiegania

Drżenie esencjalne pojawia się nieprzewidywalnie, dlatego nie można mu zapobiec ani zmniejszyć ryzyka jego wystąpienia 34. Jest to stan trwały i przewlekły, który zwykle postępuje z czasem (średni roczny wzrost nasilenia drżenia kończyn górnych wynosi od 1,5% do 5%) 5. Większość czynników ryzyka, takich jak wiek czy historia rodzinna/mutacje genetyczne, nie podlega modyfikacji 6.

Minimalizacja objawów

Mimo że nie można zapobiec rozwojowi drżenia esencjalnego, można podjąć działania mające na celu złagodzenie jego objawów i spowolnienie progresji 7. Zalecane są następujące strategie:

Unikanie czynników nasilających
  • Kofeina – ograniczenie spożycia napojów i produktów zawierających kofeinę (kawa, herbata, cola, czekolada) 8910
  • Alkohol – choć w małych ilościach może tymczasowo zmniejszać drżenie (na około godzinę), jego spożywanie nie jest zalecaną metodą leczenia ze względu na ryzyko uzależnienia i efekt odbicia (nasilenie drżenia po ustąpieniu działania alkoholu) 111213
  • Nikotyna – unikanie palenia papierosów 1415
  • Stres i niepokój – drżenie może nasilać się w sytuacjach stresowych 16
  • Niedobór snu i zmęczenie – dbanie o odpowiednią ilość snu i unikanie nadmiernego wysiłku fizycznego 1718
  • Ekstremalne temperatury – unikanie przegrzania lub wyziębienia organizmu 19
Zdrowy styl życia
  • Regularna aktywność fizyczna – ćwiczenia oporowe, ćwiczenia równoważne i rozciągające pod nadzorem specjalisty mogą pomóc w utrzymaniu sprawności manualnej i zmniejszeniu objawów 2021
  • Zbalansowana dieta – bogata w owoce, warzywa, pełne ziarna i chude białka 22
  • Regularne kontrole tarczycyzaburzenia czynności tarczycy mogą nasilać drżenie 23
  • Techniki relaksacyjne – joga, głębokie oddychanie, medytacja, tai chi czy biofeedback mogą pomóc w radzeniu sobie ze stresem i potencjalnie zmniejszyć nasilenie drżenia 2425
Przegląd przyjmowanych leków

Niektóre leki mogą nasilać drżenie, w tym leki przeciwdepresyjne czy nadmierna ilość leków stosowanych w terapii zastępczej tarczycy 26. Konsultacja z lekarzem jest konieczna przed zaprzestaniem lub zmianą dawkowania jakichkolwiek przepisanych leków 2728.

Farmakologiczne metody profilaktyki

Leczenie farmakologiczne drżenia esencjalnego może być stosowane zarówno doraźnie, jak i przewlekle, w zależności od nasilenia objawów i wpływu na jakość życia pacjenta 29.

Leczenie doraźne

Terapia doraźna może być stosowana przed sytuacjami, które mogą nasilać drżenie (np. wydarzenia społeczne, stresujące sytuacje) 30:

  • Propranolol (Inderal) – 0,5-2 tabletek (20 mg) można podać 30 minut do 1 godziny przed aktywnością społeczną lub sytuacją wywołującą niepokój 31
  • Benzodiazepiny – mogą być stosowane doraźnie, choć ze względu na potencjał uzależniający i działania niepożądane ze strony ośrodkowego układu nerwowego, należy je stosować z ostrożnością 32

Leczenie przewlekłe

W przypadku pacjentów wymagających długoterminowej terapii, dostępne są następujące opcje leczenia 33:

  • Propranolol (Inderal) – lek pierwszego wyboru, beta-bloker, który może zmniejszyć drżenie nawet o 50% 3435
  • Prymidon (Mysoline) – lek przeciwpadaczkowy, równie skuteczny jak propranolol, może zmniejszyć drżenie o około 50% 36
  • Terapia skojarzona – w przypadku gdy monoterapia propranololem lub prymidonem nie zapewnia odpowiedniej kontroli drżenia kończyn, można zastosować oba leki jednocześnie 37
  • Inne leki – w przypadkach opornych na standardową terapię:

Toksyna botulinowa

Iniekcje toksyny botulinowej typu A w mięśnie wywołujące drżenie mogą być użyteczne w przypadkach opornych na leczenie farmakologiczne 41. Ta metoda jest szczególnie skuteczna w leczeniu drżenia głowy i głosu 42.

Metody niefarmakologiczne profilaktyki i łagodzenia objawów

Techniki behawioralne i fizjoterapia

Kompleksowe podejście do zarządzania drżeniem esencjalnym obejmuje również metody niefarmakologiczne 43:

  • Fizjoterapia – ukierunkowana na poprawę siły mięśniowej, funkcji oraz redukcję drżenia 44
  • Terapia zajęciowa – pomaga w adaptacji środowiska i stylu życia, aby zminimalizować wpływ drżenia na codzienne funkcjonowanie 45

Dostępne są specjalne urządzenia, które mogą pomóc w codziennym funkcjonowaniu 49:

  • Cięższe sztućce, metalowe kubki – dodatkowa waga może zmniejszyć drżenie 50
  • Obciążniki nadgarstkowe – dodanie niewielkiego ciężaru do ręki może zredukować drżenie 51
  • Łyżki ze stabilizującymi żyroskopami – choć są kosztowne i nie posiadają formalnej aprobaty FDA 52
  • Urządzenia ortotyczne – reagujące na drżenie 53
  • Specjalistyczne urządzenia – jak np. Steadi-3, zaprojektowane specjalnie dla osób z drżeniem esencjalnym, zapewniające kontrolę drżenia w czasie rzeczywistym 54

Leczenie chirurgiczne w profilaktyce progresji objawów

Leczenie chirurgiczne drżenia esencjalnego jest zarezerwowane dla wyselekcjonowanych pacjentów z ciężkim drżeniem, które nie jest odpowiednio kontrolowane przez terapię farmakologiczną 55. Metody chirurgiczne obejmują:

Głęboka stymulacja mózgu (DBS)

Ta metoda jest uznawana za złoty standard leczenia chirurgicznego 56. Polega na implantacji elektrody do mózgu, która łączy się z urządzeniem podobnym do rozrusznika serca, wszczepionym pod obojczykiem 57. Elektroda wysyła łagodne impulsy elektryczne do mózgu, aby kontrolować skurcze mięśni 58. Jest to wysoce skuteczna metoda dla osób z drżeniem esencjalnym 59.

Talamotomia

Zabieg polega na wytworzeniu chirurgicznej zmiany w brzuszno-pośrednim jądrze wzgórza 60. Istnieją różne metody wykonania talamotomii:

  • Zogniskowana ultradźwiękowa talamotomia (HIFU) – beznacięciowa procedura, wykorzystująca obrazowanie MRI do kierowania fal dźwiękowych przez skórę i czaszkę, które są skupiane na wzgórzu. Fale generują ciepło, które niszczy tkankę mózgową powodującą drżenie 61. Procedura odbywa się całkowicie w skanerze MRI w ramach jednej dwugodzinnej, ambulatoryjnej sesji 62. Jest to opcja dla pacjentów, u których objawy drżenia są gorsze lub postępują szybciej po jednej stronie ciała 63.
  • Radiochirurgia stereotaktyczna (nóż gamma) – kolejna metoda beznacięciowa tworzenia zmiany 64.

Znaczenie wczesnej interwencji

Bardzo ważne jest, aby pacjenci z drżeniem esencjalnym jak najwcześniej trafili pod opiekę wykwalifikowanego specjalisty zaburzeń ruchu 65. Badania wielokrotnie wykazały, że im wcześniej pacjent zostanie objęty specjalistyczną opieką, tym szybciej poprawia się jego jakość życia i tym dłużej utrzymują się korzyści z leczenia 66.

Pacjenci powinni rozważyć konsultację ze specjalistą drżenia esencjalnego, jeśli objawy nie poprawiają się przy obecnej terapii lub jeśli chcieliby zasięgnąć drugiej opinii na temat dostępnych opcji leczenia 67.

Regularne monitorowanie

Regularne monitorowanie stanu zdrowia i omawianie wszelkich zmian w nasileniu drżenia z lekarzem prowadzącym jest istotnym elementem długoterminowego zarządzania tym schorzeniem 68. Pozwala to na wczesne wykrycie progresji choroby i dostosowanie leczenia odpowiednio do zmieniających się potrzeb pacjenta.

Należy zauważyć, że choć opisane strategie mogą pomóc w zarządzaniu objawami drżenia esencjalnego, nie ma gwarantowanego sposobu na zapobieżenie rozwojowi lub progresji tego schorzenia 69. Dodatkowo, osoby z rodzinną historią drżenia esencjalnego mogą mieć wyższe ryzyko rozwoju tego schorzenia, a działania profilaktyczne mogą mieć ograniczony wpływ w tych przypadkach 70.
