Dermografizm, czyli dermatographic urticaria, to najczęstsza forma pokrzywki indukowanej, dotykająca 2-5% populacji. Charakteryzuje się potrójną odpowiedzią Lewisa: po drażnieniu skóry pojawia się czerwona linia (rozszerzenie naczyń włosowatych), rumień (rozszerzenie tętniczek) oraz liniowy bąbel (obrzęk i przesięk płynu). Objawy, takie jak wypukłe, czerwone linie, bąble, świąd, pieczenie czy mrowienie, zwykle ustępują samoistnie w ciągu 30 minut. Diagnostyka opiera się na teście klinicznym polegającym na delikatnym przeciągnięciu narzędziem po skórze i obserwacji reakcji. Należy różnicować dermografizm z innymi typami pokrzywki, w tym z białym dermografizmem, który objawia się blednięciem skóry po potarciu. W diagnostyce różnicowej uwzględnia się także mastocytozę, w której biopsja skóry może być pomocna.

Dermografizm – definicja

Dermografizm (znany również jako dermatographia lub dermatographic urticaria) to łagodna choroba skóry, w której nawet delikatne podrapanie lub pocieranie skóry powoduje powstawanie wypukłych, czerwonych linii lub bąbli w miejscu dotyku. Termin „dermografizm” dosłownie oznacza „pisanie na skórze”. Jest to najczęstsza postać pokrzywki indukowanej, dotykająca około 2-5% populacji ogólnej.123

W przypadku dermografizmu, pocieranie, drapanie lub nacisk na skórę powoduje pojawienie się reakcji podobnej do pokrzywki. Po zadrapaniu lub potarciu skóry, początkowo pojawia się czerwona linia (rozszerzenie naczyń włosowatych), następnie rumień (rozszerzenie tętniczek) i powstanie liniowego bąbla (przesięk płynu/obrzęk) – zdarzenia te określane są łącznie jako potrójna odpowiedź Lewisa.34

Objawy dermografizmu

Objawy dermografizmu są zazwyczaj łagodne i ustępują samoistnie w ciągu 30 minut. Typowe objawy obejmują:56

  • Wypukłe, czerwone linie w miejscu zadrapania lub potarcia skóry
  • Obrzęk/stan zapalny
  • Bąble podobne do pokrzywki
  • Świąd (w niektórych przypadkach)
  • Pieczenie lub mrowienie skóry

Objawy dermografizmu mogą być wywoływane przez proste czynniki lub działania. Tarcie ubrania lub pościeli o skórę może wywołać reakcję. Również nacisk wywierany przez opieranie się lub spoczywanie na twardej powierzchni może wyzwolić bąble.9

Czynniki wyzwalające

Do najczęstszych czynników wyzwalających dermografizm należą:1011

  • Pocieranie lub nacisk na skórę
  • Wszelkie podrażnienia powodujące świąd
  • Ciepło (gorące prysznice lub sauna)
  • Stres
  • Formy ćwiczeń wiążące się z tarciem skóry
  • Suche powietrze
  • Infekcje
  • Niektóre leki, w tym penicylina

U niektórych pacjentów dermografizm może być również związany z innymi schorzeniami, takimi jak zespół hipereozynofilowy.12

Diagnoza dermografizmu

Dermografizm jest diagnozą kliniczną, którą lekarz może potwierdzić przeprowadzając prosty test. Podczas badania, lekarz może użyć szpatułki lub innego narzędzia, aby delikatnie przeciągnąć po skórze ramienia lub pleców. Jeśli w ciągu kilku minut pojawi się wypukła linia lub bąbel, prawdopodobnie mamy do czynienia z dermografizmem.1314

W celu dokładnej diagnozy, lekarz może zadać następujące pytania:15

  • Kiedy zaczęły się objawy?
  • Czy byłeś/aś chory/a tuż przed wystąpieniem objawów?
  • Czy zacząłeś/ęłaś przyjmować nowy lek tuż przed wystąpieniem objawów?
  • Czy objawy są ciągłe, czy pojawiają się i znikają?
  • Jak poważne są objawy?
  • Czy objawy utrudniają codzienne czynności?
  • Czy masz alergie? Jeśli tak, na co jesteś uczulony/a?
  • Czy masz suchą skórę lub inne schorzenia skórne?
  • Co poprawia objawy?
  • Co pogarsza objawy?

Biopsja skóry jest rzadko wymagana, z wyjątkiem przypadków, gdy konieczne jest odróżnienie dermografizmu od mastocytozy.14

Różnicowanie

Dermografizm należy odróżnić od innych typów pokrzywki, szczególnie od pokrzywki przewlekłej spontanicznej i innych form pokrzywki indukowanej.3 Istnieje również odmiana dermografizmu zwana „białym dermografizmem”, która występuje głównie u pacjentów z atopowym zapaleniem skóry. W białym dermografizmie, po potarciu skóry, początkowa czerwona linia jest zastępowana w ciągu 10-15 sekund przez blednięcie, które może utrzymywać się przez 60 sekund lub dłużej.16

Leczenie i postępowanie pielęgniarskie

Większość przypadków dermografizmu nie wymaga leczenia, ponieważ objawy zwykle ustępują samoistnie w ciągu 30 minut. Jednak jeśli objawy są poważne, długotrwałe lub dokuczliwe, dostępnych jest kilka opcji terapeutycznych.513

Leczenie farmakologiczne

W przypadku objawowego dermografizmu, najczęściej stosowanymi lekami są:1718

  • Leki przeciwhistaminowe H1 – są lekami pierwszego wyboru; jednak często wymagane są wyższe niż standardowe dawki (do 4-krotnie wyższe) w celu osiągnięcia kontroli objawów. Przykłady to cetryzyna, loratadyna, feksofenadyna.
  • Leki przeciwhistaminowe H2 – takie jak difenhydramina, hydroksyzyna są łączone w razie potrzeby.
  • Omalizumab – rekombinowane humanizowane przeciwciało monoklonalne przeciwko immunoglobulinie E (IgE), z powodzeniem stosowane u pacjentów z pokrzywką fizyczną, w tym objawowym dermografizmem.

Omalizumab jest generalnie dobrze tolerowany przez większość pacjentów, w tym dzieci. Niektórzy pacjenci zgłaszają całkowite ustąpienie objawów w ciągu kilku dni po pierwszym wstrzyknięciu.19

Fototerapia

Wąskopasmowa fototerapia UVB i doustny psoralen plus terapia światłem UVA były stosowane jako metody leczenia objawowego dermografizmu. Zgłaszano subiektywną ulgę w świądzie i obrzęku oraz obiektywne zmniejszenie bąbli.19

Postępowanie pielęgniarskie

W opiece nad pacjentem z dermografizmem, ważne jest stosowanie się do następujących zaleceń:420

  • Delikatne traktowanie skóry – używanie łagodnego mydła lub środka myjącego bez mydła i delikatne osuszanie skóry.
  • Unikanie drapania – to dobra wskazówka dla każdego schorzenia skóry.
  • Nawilżanie skóry – codzienne stosowanie kremów, balsamów lub maści. Kremy i maści są grubsze i zazwyczaj działają lepiej niż balsamy. Aplikacja produktu do pielęgnacji skóry, gdy skóra jest jeszcze wilgotna po myciu.
  • Noszenie odzieży z materiałów, które nie powodują swędzenia – unikanie wełny i materiałów syntetycznych.
  • Używanie letniej wody podczas kąpieli lub prysznica – unikanie gorących pryszniców, które mogą nasilić objawy.

Ponadto, ważne jest zarządzanie stresem, który może nasilać objawy dermografizmu. Aktywności relaksacyjne, takie jak medytacja, joga, tai chi, ćwiczenia oddechowe i relaksacja mięśni mogą pomóc zmniejszyć stres i zapobiec dermografizmowi.5

Postępowanie w jamie ustnej

U niektórych pacjentów z dermografizmem może występować zwiększona wrażliwość tkanek jamy ustnej na minimalne naciski. Może to negatywnie wpływać na procedury higieny jamy ustnej. Podczas leczenia takich pacjentów ważne jest, aby zmniejszyć objawy w jamie ustnej, co umożliwi pacjentowi przeprowadzenie rutynowej pielęgnacji jamy ustnej.21

Rozpoznanie tego łagodnego zaburzenia i uspokojenie pacjenta jest ważne w radzeniu sobie z tym stanem. Leczenie dermografizmu objawowo, dopóki problem nie zostanie odpowiednio opanowany. Należy zwrócić szczególną uwagę podczas leczenia tych pacjentów ze względu na zwiększoną częstość występowania reakcji alergicznych oraz zwiększoną wrażliwość tkanek jamy ustnej i skóry.21

Prognoza i przebieg choroby

Dermografizm jest schorzeniem łagodnym, chociaż może być uciążliwy, zwłaszcza jeśli towarzyszy mu świąd, i może nawet powodować zakłopotanie. Jednak jest to stan powszechny i bardzo normalny, który zwykle nie wpływa na jakość życia.22

Naturalny przebieg objawowego dermografizmu jest nieprzewidywalny. Może trwać miesiące lub lata, lub występować okresowo. U wielu pacjentów stan stopniowo się poprawia i ustępuje po kilku latach.12

W porównaniu z innymi przewlekłymi postaciami pokrzywki, objawowy dermografizm wykazuje najszybsze ustępowanie, z 36% przypadków ustępujących po 5 latach i 51% po 10 latach.23

Edukacja pacjenta

Edukacja pacjenta jest kluczowym elementem w zarządzaniu dermografizmem. Ważne jest, aby informować pacjentów o:1223

  • Łagodnym charakterze schorzenia
  • Możliwym długotrwałym przebiegu
  • Działaniach niepożądanych terapii przeciwhistaminowej, szczególnie senności
  • Ostrzeżeniach dotyczących prowadzenia pojazdów lub obsługi maszyn podczas przyjmowania leków przeciwhistaminowych

Pacjenci powinni być również edukowani na temat czynników ryzyka związanych z pojawieniem się dermografizmu, aby unikać takich stresorów. Prawidłowa pielęgnacja skóry i stosowanie emolientów może zmniejszyć suchość skóry, tym samym zapobiegając nasileniu dermografizmu poprzez drapanie.24

Zalecenia dla pacjenta

Oto kilka wskazówek, które mogą pomóc złagodzić dyskomfort i zapobiec objawom dermografizmu:2511

  • Zmniejsz stres poprzez lekkie ćwiczenia, spacery na łonie natury oraz ćwiczenia uważności lub oddechowe.
  • Usuń bodźce fizyczne, takie jak ucisk spowodowany ciasnym ubraniem i swędzące materiały, takie jak wełna.
  • Kąpiel w letniej wodzie, używanie łagodnych mydeł i osuszanie skóry raczej poprzez delikatne dotykanie niż pocieranie.
  • Nakładanie środków nawilżających na dotknięte obszary.
  • Utrzymywanie odpowiedniego nawodnienia organizmu poprzez picie wystarczającej ilości wody.
  • Zdrowa dieta unikająca alkoholu, nabiału, kofeiny i cukru; oraz włączenie owoców, warzyw i pokarmów bogatych w kwasy omega, które wzmacniają zdrowie układu odpornościowego.

Jeśli objawy utrzymują się, leki przeciwhistaminowe, takie jak cetryzyna lub loratadyna, mogą pomóc w opanowaniu świądu. Leki przeciwhistaminowe zwykle zaczynają działać po około 30 minutach. Można przyjmować hydroksyzynę, sedatywny lek przeciwhistaminowy, przed snem.25

Wnioski

Dermografizm jest łagodnym schorzeniem skóry, które zazwyczaj nie wymaga leczenia. Większość osób z tym schorzeniem doświadcza jedynie łagodnych, krótkotrwałych objawów, które ustępują samoistnie w ciągu 30 minut. Jednak dla tych, których objawy są poważne lub dokuczliwe, dostępne są opcje leczenia, w tym leki przeciwhistaminowe i fototerapia.513

Kluczowe w zarządzaniu dermografizmem jest unikanie czynników wyzwalających, takich jak drapanie, wełniana odzież, gorące prysznice i stres. Właściwa pielęgnacja skóry, w tym regularne nawilżanie i łagodne mycie, może pomóc zminimalizować objawy.4
