Badania przesiewowe CCHD u noworodków wykorzystują porównanie saturacji przedprzewodowej i poprzewodowej, z progiem nieprawidłowości poniżej 90% saturacji tlenu.

Epidemiologia sinych lub szarych ust i skóry (cyjanoza)

Cyjanoza, czyli sinienie skóry i błon śluzowych, jest objawem patologicznym, a nie chorobą samą w sobie. Ten charakterystyczny niebieski lub szary odcień pojawia się, gdy stężenie odtlenowanej hemoglobiny we krwi przekracza 5,0 g/dl. Termin „cyjanoza” pochodzi od greckiego słowa „cyan” oznaczającego kolor niebieski.12 Objaw ten można najlepiej zaobserwować w obszarach o bogatym unaczynieniu powierzchownym i cienkiej skórze, takich jak usta, nos, płatki uszu, jama ustna, kończyny oraz opuszki palców dłoni i stóp.1

Występowanie cyjanozy w populacji

Chociaż dane dotyczące częstości występowania cyjanozy jako objawu są ograniczone, pewne wskaźniki można uzyskać analizując częstość występowania schorzeń, które często powodują sinienie skóry i błon śluzowych. Wrodzone wady serca, które są jedną z głównych przyczyn cyjanozy, występują u około 1000 na 100 000 osób na całym świecie.1 Dane dotyczące częstości występowania sinej skóry i błon śluzowych związanej z wadami serca u noworodków są niewystarczające, jednak wiadomo, że sinica wrodzona dotyka w równym stopniu chłopców i dziewczynki, bez predyspozycji rasowych.1

W badaniu przeprowadzonym na noworodkach urodzonych o czasie i późnych wcześniakach, częstość występowania epizodów cyjanozy wymagających przyjęcia do oddziału intensywnej opieki neonatologicznej wynosiła 1,8 na 1000 żywych urodzeń, bez istotnych różnic między noworodkami urodzonymi o czasie i późnymi wcześniakami (p=0,167).12

Epidemiologia wad wrodzonych powodujących cyjanozę

Spośród wszystkich noworodków z wrodzonymi wadami serca, około 25% prezentuje cyjanozę jako objaw.1 Niektóre warianty hemoglobiny z niskim powinowactwem do tlenu mogą również powodować sinienie. Zidentyfikowano ponad 20 takich hemoglobin.1 Pięć wariantów hemoglobiny M jest dziedziczonych w sposób autosomalny dominujący, a osoby z podstawieniem łańcucha alfa mogą prezentować cyjanozę od urodzenia.1

Methemoglobinemia, która może powodować sinienie skóry, jest uważana za rzadką, przy czym nabyta methemoglobinemia występuje częściej niż postać wrodzona.1 Niektórzy pacjenci mogą nie wykazywać objawów nawet przy stężeniu sulfhemoglobiny do 10 g/dl.1

Dystrybucja według wieku i płci

Cyjanoza może występować u osób w każdym wieku.1 Jednakże wrodzone sinicze wady serca występują częściej u noworodków.1 Cyjanoza jest równie powszechna zarówno u mężczyzn, jak i kobiet, co wynika z równego rozkładu wrodzonych wad serca między płciami.1

U niemowląt cyjanoza często występuje w postaci sinych ust i skóry, co może być pierwszym objawem wrodzonej wady serca. U noworodków z przewagą hemoglobiny płodowej (HbF) może wystąpić większe obniżenie natlenienia, zanim cyjanoza stanie się klinicznie widoczna.1 Niemowlęta, które mają głównie hemoglobinę dorosłych, będą wykazywać centralną cyjanozę (wysycenie tętnicze 75% do 85%) przy spadku PaO2 poniżej 50 mmHg. Natomiast u niemowląt z przewagą hemoglobiny płodowej, centralna cyjanoza może nie być zauważalna, dopóki PaO2 nie spadnie znacznie poniżej 40 mmHg.1

Diagnostyka i monitorowanie cyjanozy

Rozpoznanie cyjanozy opiera się głównie na obserwacji klinicznej, a nie na testach laboratoryjnych.1 Stwierdzenie sinych lub szarych ust i skóry wymaga dokładnej oceny lekarskiej w celu ustalenia przyczyny tego stanu.1

Metody diagnostyczne

Najczęstszym testem diagnostycznym stosowanym w przypadku cyjanozy jest analiza gazometrii krwi tętniczej oraz pomiar saturacji tlenem za pomocą pulsoksymetrii. Badania te przeprowadza się w celu oceny ilości tlenu we krwi tętniczej.1 Pulsoksymetria jest nieinwazyjnym testem, który wykorzystuje specjalne urządzenie z czujnikiem świetlnym umieszczanym na palcu dłoni lub stopy na kilka minut. Test ten określa, czy poziom tlenu jest prawidłowy czy niski.1

W przypadku podejrzenia methemoglobinemii, diagnoza może być rozważona, gdy saturacja tlenem pacjenta nie poprawia się po podaniu tlenu uzupełniającego. Znaczące stężenie methemoglobiny powoduje zmianę koloru krwi, co może zakłócać pomiary pulsoksymetru wykorzystującego tradycyjne techniki detekcji podwójnej długości fali, prowadząc do odczytu około 85% zamiast odzwierciedlenia rzeczywistego wysycenia tlenem.1 Diagnozę potwierdza się za pomocą gazometrii krwi żylnej lub tętniczej z kooksymetrią w celu bezpośredniego pomiaru poziomu methemoglobiny.1

Jednym z podejść do oceny etiologii cyjanozy jest pobranie heparynizowanej próbki krwi tętniczej.1 Złotym standardem w ocenie pacjenta z cyjanozą jest gazometria krwi tętniczej (ABG).1

Badania przesiewowe noworodków

W badaniach przesiewowych w kierunku krytycznych wrodzonych wad serca (CCHD) u noworodków, porównuje się saturację przedprzewodową (prawa kończyna górna) z saturacją poprzewodową (kończyna dolna). Wynik badania jest uznawany za nieprawidłowy, gdy wartość saturacji tlenu jest niższa niż 90%.1 Należy jednak zauważyć, że badanie przesiewowe CCHD jest mniej czułe w wykrywaniu wad serca, które mogą nie powodować wczesnej hipoksemii. Takie niemowlęta mogą przejść badanie przesiewowe CCHD i zostać wypisane po niepowikłanej i krótkiej hospitalizacji po urodzeniu.1

Trudności diagnostyczne

Rozpoznanie cyjanozy może być utrudnione u osób o ciemniejszej pigmentacji skóry.12 W takich przypadkach cyjanoza może być trudniejsza do wykrycia, ale nadal można ją zdiagnozować poprzez dokładne badanie typowych obszarów ciała, takich jak łożyska paznokci, język i błony śluzowe, gdzie skóra jest cieńsza i bardziej unaczyniona.2 U osób o ciemniejszej karnacji, cyjanoza może wyglądać bardziej jak szary lub biały odcień, szczególnie widoczny wokół ust, dziąseł i paznokci.12

Identyfikacja cyjanozy jest umiejętnością, która poprawia się wraz z doświadczeniem i może być utrudniona u obserwatorów z zaburzeniami rozpoznawania kolorów.1

Nadzór i monitoring cyjanozy

Cyjanoza może być objawem poważnego problemu zdrowotnego, dlatego wymaga natychmiastowej oceny medycznej, szczególnie gdy pojawia się nagle lub towarzyszy jej duszność.11 W szczególności centralna cyjanoza, obejmująca sinienie języka, warg, głowy lub tułowia, powinna być natychmiast oceniona przez lekarza.1

Monitorowanie cyjanozy u dzieci i niemowląt

U dzieci i niemowląt, ostry i utrzymujący się niebieski lub fioletowy kolor twarzy, języka lub tułowia może wskazywać na poważniejszy problem i wymaga natychmiastowej pomocy medycznej.1 Rodzice powinni natychmiast skontaktować się z lekarzem, jeśli skóra dziecka ma nietypowy niebieskawy kolor.1

Obecność cyjanozy była silnym predyktorem krztuśca, a także wyraźnej leukocytozy i limfocytozy.1 Ponadto, obecność cyjanozy powinna inicjować natychmiastową pełną ocenę kardiologiczną w kierunku wrodzonych wad serca.1

Kompleksowe dane statystyczne dotyczące częstości występowania cyjanozy jako objawu są ograniczone, jednak można wyciągnąć pewne wnioski na podstawie częstości występowania schorzeń, które często powodują sinienie skóry i błon śluzowych.

Statystyki występowania

Różnice demograficzne

Cyjanoza wrodzona dotyka w równym stopniu chłopców i dziewczynki, bez predyspozycji rasowych.1 Jednakże rozpoznanie cyjanozy może być trudniejsze u osób o ciemniejszej pigmentacji skóry.12

U osób o ciemniejszej karnacji, cyjanoza może wyglądać bardziej jak szary lub biały odcień, szczególnie widoczny wokół ust, dziąseł i paznokci.12 U osób o normalnym poziomie hemoglobiny (13,5-18 g/dl u mężczyzn, 11,5-16 g/dl u kobiet), centralna cyjanoza jest widoczna, gdy saturacja tlenem spada między 80 a 87%.1

Cyjanoza może być również trudniejsza do wykrycia u osób z ciężką anemią (np. Hb <5 g/dl [3,1 mmol/l]).1

    Cyanosis is a blue discoloration of the skin and/or mucous membranes. It is due to the presence of greater than 3 g/dL of reduced or deoxygenated Hb (Hb) in the blood. It is important to note that cyanosis is dependent on the absolute concentration of reduced Hb. Cyanosis can be clinically appreciated when the O2 saturation is 85%. […] At a normal Hb level of 15 g/dL, the presence of 3 g/dL of reduced Hb results in 20% desaturation. Therefore, cyanosis is visible when O2 saturation is approximately ~80%. The lower the Hb level, the lower the O2 saturation needed before cyanosis can be appreciated. […] Infants with a higher proportion of HbF may have a greater reduction in oxygenation before cyanosis is clinically apparent. For example, in infants who have mostly adult Hb, central cyanosis (arterial saturation 75% to 85%) will be observed when the PaO2 falls below 50 mmHg.     Cyan means blue, and the abnormal bluish skin and mucous membrane discoloration is called cyanosis. It is a pathologic sign and not a disease by itself. Underlying diseases that increase the deoxygenated hemoglobin to 5.0 g/dl or more leads to cyanosis. Cyanosis can be best appreciated in areas with rich superficial vasculature and thin overlying dermis. These include lips, nose, earlobes, oral cavity, extremities, and tips of fingers and toes. Cyanosis usually becomes evident in hypoxemia, that is, abnormally low oxygenation concentration (80-85%) in arterial blood. However, it is not sensitive or a specific indicator of hypoxemia. Cyanosis is further classified as central, peripheral, and differential. Central cyanosis is a generalized bluish discoloration of the body and visible mucous membranes. It occurs due to inadequate oxygenation secondary to conditions that increase deoxygenated hemoglobin or abnormal hemoglobin. Peripheral cyanosis is the bluish discoloration of the distal extremities (hands, fingertips, toes). It can sometimes involve circumoral and periorbital areas, but mucous membranes are generally not involved. Peripheral cyanosis is rarely a life-threatening medical emergency. However, it is essential to determine the underlying cause and manage it promptly to prevent potential complications. Differential cyanosis is the asymmetrical bluish discoloration between the upper and lower extremities. It usually indicates serious underlying cardiopulmonary conditions. All causes of central cyanosis can also cause peripheral cyanosis. Cardiopulmonary causes and hemoglobin abnormalities are the common causes of central cyanosis. Differential cyanosis can be seen in patent ductus arteriosus with pulmonary hypertension. Peripheral cyanosis occurs due to the inability of the body to deliver oxygen-rich blood to the peripheral tissues. Congestive peripheral cyanosis can be caused due to the slowing of blood flow. Ischemic peripheral cyanosis occurs when vasoconstriction leads to diminished peripheral blood flow. In peripheral cyanosis, there is normal arterial oxygen saturation but increased oxygen extraction by the peripheral tissue in the capillary bed in the setting of peripheral vasoconstriction and decreased peripheral blood flow. This results in a significant difference in the saturation between the arterial and venous blood, with increased deoxygenated blood on the venous side of the capillary beds. Reduced cardiac output in heart failure and shock can, if severe, lead to peripheral cyanosis.
