Zapobieganie i profilaktyka

Zespół Sjögrena jest autoimmunologiczną chorobą o podłożu genetycznym, dla której nie istnieje skuteczna metoda pierwotnej prewencji. Wczesna diagnostyka oraz wielospecjalistyczna opieka są kluczowe dla zapobiegania powikłaniom, takim jak chłoniak, oraz łagodzenia objawów, w tym suchości oczu i jamy ustnej. Leczenie jest głównie objawowe i paliatywne, obejmujące stosowanie kropli nawilżających bez konserwantów, maści na noc, ciepłych kompresów, a także profilaktykę stomatologiczną z użyciem fluoru, chlorheksydyny, środków remineralizujących (np. fosforan wapnia amorficznego, kazeino-fosfopeptydy-ACP) oraz stymulację wydzielania śliny poprzez ksylitol i sztuczną ślinę. Wskazane jest także unikanie leków o działaniu przeciwcholinergicznym, które mogą nasilać suchość, oraz regularne monitorowanie pacjentów pod kątem powikłań.

Prewencja zespołu Sjögrena

Zespół Sjögrena jest chorobą autoimmunologiczną o podłożu genetycznym, co sprawia, że obecnie nie istnieje skuteczna metoda zapobiegania jej rozwojowi.123 Z uwagi na nieznane dokładne przyczyny choroby, nie można przewidzieć kiedy i u kogo wystąpi.1 Mimo braku możliwości pierwotnej prewencji, kluczową rolę odgrywa wczesna diagnostyka oraz działania mające na celu zapobieganie powikłaniom i łagodzenie objawów.45 Warto podkreślić, że trwają intensywne badania nad lepszym zrozumieniem patogenezy choroby, co w przyszłości może prowadzić do nowych metod prewencji.6

Prewencja powikłań zespołu Sjögrena

Chociaż nie można zapobiec samej chorobie, istnieją liczne strategie mające na celu zapobieganie powikłaniom i progresji objawów. Leczenie zespołu Sjögrena jest głównie objawowe i paliatywne, skupiające się na łagodzeniu symptomów i zapobieganiu powikłaniom.7 Kluczową rolę odgrywa wczesna diagnoza i odpowiednia opieka wielospecjalistyczna.84

Ważne jest regularne monitorowanie pacjentów pod kątem powikłań, szczególnie wczesnego wykrywania chłoniaka, który jest jednym z poważniejszych powikłań zespołu Sjögrena.910 Pacjentki planujące ciążę powinny być poinformowane, że wczesne wdrożenie hydroksychlorochiny może znacząco zmniejszyć ryzyko powikłań ciążowych.9

Zapobieganie suchości oczu

Suchość oczu jest jednym z głównych objawów zespołu Sjögrena, który można łagodzić poprzez odpowiednie postępowanie:511

  • Stosowanie kropli nawilżających bez konserwantów regularnie w ciągu dnia, nawet przy braku objawów1213
  • Używanie maści lub żeli do oczu na noc – są one gęstsze i dłużej utrzymują nawilżenie13

Pacjenci z zespołem Sjögrena są narażeni na zwiększone ryzyko próchnicy i innych problemów stomatologicznych z powodu zmniejszonego wydzielania śliny.1511 Zalecane działania profilaktyczne obejmują:

  • Rygorystyczne przestrzeganie higieny jamy ustnej – szczotkowanie zębów po każdym posiłku i przed snem oraz codzienne używanie nici dentystycznej1115
