W 2021 roku przedłużone zaburzenie żałoby zostało włączone do DSM-5-TR jako odrębna jednostka diagnostyczna. Leczenie żałoby skomplikowanej skupia się na pomocy osobom w procesie adaptacji do straty, rozwiązaniu komplikacji żałoby i ułatwieniu naturalnej, adaptacyjnej odpowiedzi na stratę6. Terapia jest najskuteczniejszą strategią w pomaganiu osobom przezwyciężyć żałobę skomplikowaną7.
Terapia Żałoby Skomplikowanej (CGT)
Terapia żałoby skomplikowanej (Complicated Grief Therapy, CGT) to wyspecjalizowane podejście terapeutyczne opracowane specjalnie do leczenia żałoby skomplikowanej. Jest to półustrukturyzowany, 16-sesyjny program terapeutyczny, ukierunkowany na akceptację rzeczywistości straty i przywrócenie zdolności do dobrostanu8. CGT ma korzenie zarówno w terapii interpersonalnej (IPT), jak i w terapii poznawczo-behawioralnej (CBT)9.
Podstawową zasadą leżącą u podstaw CGT jest przekonanie, że żałoba jest naturalnym, adaptacyjnym procesem. Oznacza to, że leczenie żałoby skomplikowanej obejmuje usuwanie przeszkód utrudniających pomyślne rozwiązanie procesu żałoby10.

Terapia CGT jest zwykle podzielona na trzy fazy i trwa około 16 sesji, z których każda trwa około 45-60 minut13. Jednym z kluczowych elementów CGT jest wprowadzenie „wyobrażeniowego odwiedzania” (imaginal revisiting), które pomaga pacjentom skonfrontować się i przepracować trudne wspomnienia związane ze stratą14.

Uczestnicy w grupie CGT wykazali znacznie większą poprawę w redukcji objawów, mierzoną tygodniową zmianą wyników w Inwentarzu Żałoby Skomplikowanej (ICG); 1,05 punktu/tydzień dla CGT vs 0,75 punktu/tydzień dla IPT; p≤0,001)18.
Badania wykazują, że osoby, które otrzymują CGT, mają lepsze wyniki terapeutyczne niż osoby, które otrzymują inne interwencje w żałobie skomplikowanej. Osoby, które otrzymują CGT, doświadczają znacznie większej poprawy w funkcjonowaniu w pracy i w domu oraz zredukowanych objawów żałoby skomplikowanej, takich jak smutek, poczucie winy i unikanie społeczne19.
Inne Podejścia Terapeutyczne w Leczeniu Żałoby Skomplikowanej
Oprócz CGT, istnieją również inne podejścia terapeutyczne, które mogą być skuteczne w leczeniu żałoby skomplikowanej:
Terapia Poznawczo-Behawioralna (CBT)
Terapia poznawczo-behawioralna może być skuteczna w leczeniu negatywnych wzorców myślowych i zachowań związanych z żałobą skomplikowaną20. Terapia poznawczo-behawioralna dostosowana specjalnie do żałoby (grief-focused CBT) została wykazana jako skuteczna w randomizowanych badaniach klinicznych. W jednym z takich badań, w którym porównano CBT skoncentrowaną na żałobie z terapią uważności, uczestnicy w grupie CBT wykazali trwalsze korzyści sześć miesięcy po zakończeniu terapii22. Uczestnicy w grupie CBT wykazali również większe zmniejszenie depresji i poznawczych aspektów żałoby w porównaniu z grupą terapii uważności23.
Terapia Interpersonalna (IPT)
Terapia interpersonalna skupia się bardziej na kwestiach związanych z relacjami i przejściem do nowego życia bez zmarłej osoby24. Chociaż IPT może być pomocna, badania wykazały, że jest ona mniej skuteczna niż CGT w leczeniu żałoby skomplikowanej25.
Terapia Traumatycznej Żałoby

Badania wykazały, że chociaż terapia uważności może przynieść pewne korzyści w leczeniu żałoby skomplikowanej, może nie zapewniać tak trwałych efektów jak terapia poznawczo-behawioralna31.
Farmakoterapia w Żałobie Skomplikowanej
Istnieje niewiele solidnych badań na temat stosowania leków psychiatrycznych w leczeniu żałoby skomplikowanej32. Jednak leki przeciwdepresyjne mogą być pomocne u osób, które cierpią na depresję kliniczną oprócz żałoby skomplikowanej33.
Postrzeganie żałoby skomplikowanej jako stanu reakcji na stres, z nakładającymi się objawami PTSD i depresji, sugeruje, że leki przeciwdepresyjne, ale nie benzodiazepiny, mogą być korzystne w leczeniu żałoby skomplikowanej34. W niektórych przypadkach, maksymalizacja dawki leku przeciwdepresyjnego dostosowanej do odpowiedzi i tolerancji może przynieść korzyści35.
Leki, szczególnie przeciwdepresyjne, są przepisywane w celu zarządzania współistniejącymi objawami, takimi jak depresja lub lęk, u osób doświadczających żałoby skomplikowanej36. Jednak badania sugerują, że leczenie depresji jest mniej pomocne niż terapia przedłużonej żałoby dla osób z tym zaburzeniem37.
Cele Leczenia Żałoby Skomplikowanej
Celem leczenia żałoby skomplikowanej nie jest sprawienie, aby pacjenci zapomnieli o wspomnieniach, które dzielą ze zmarłymi, ani aby ich poczucie straty zniknęło. Zamiast tego, leczenie jest podawane w celu znalezienia rozwiązania i akceptacji38.
Model żałoby opracowany przez profesora psychologii z Harvardu, J. Williama Wordena, wymienia cztery zadania żałoby, które obejmują te wspólne długoterminowe cele39:
- Zaakceptować rzeczywistość straty
- Przepracować emocjonalny ból żałoby
- Dostosować się do życia bez osoby, która zmarła
- Zachować połączenie z tą osobą, budując jednocześnie nowe życie
Ostatecznie, celem leczenia żałoby nie jest przezwyciężenie żałoby ani zaprzestanie odczuwania bólu straty, ale zintegrowanie tego bólu w nową wizję życia40.
Struktura Leczenia Żałoby Skomplikowanej
Leczenie żałoby skomplikowanej obejmuje zwykle następujące komponenty:
Psychoedukacja
Dostarczanie psychoedukacji na temat procesu żałoby może pomóc osobom zrozumieć swoje doświadczenia i znormalizować ich reakcje41. Psychoedukacja obejmuje również edukacyjny aspekt, w którym ludzie uczą się o znakach i symptomach żałoby skomplikowanej. Pomaga im to zrozumieć, czego doświadczają42.
Terapia Indywidualna
Regularna, intensywna psychoterapia będzie podstawą każdego programu leczenia żałoby skomplikowanej43. Psychiatrzy i psychologowie leczący osoby z uporczywą żałobą będą stosować podobne techniki do tych zalecanych dla pacjentów cierpiących na depresję lub PTSD (dwa zaburzenia, które najlepiej pasują do objawów żałoby skomplikowanej). Jednak wszystkie oferowane terapie będą specjalnie dostosowane do radzenia sobie z intensywną żałobą jako unikalnym i upośledzającym zjawiskiem o własnych, odrębnych cechach44.
Terapia Grupowa i Grupy Wsparcia
Pacjenci z żałobą skomplikowaną mogą korzystać z terapii grupowej, jak również z terapii indywidualnej45. Grupy wsparcia zapewniają środowisko wspólnotowe, w którym osoby dzielą się swoimi doświadczeniami i uczuciami związanymi ze stratą. Grupy te często sprzyjają poczuciu przynależności i zrozumienia wśród uczestników, którzy doświadczyli podobnych strat46.
Łączenie się z innymi doświadczającymi przedłużonej żałoby poprzez grupy wsparcia może również przyspieszyć proces leczenia poprzez normalizację objawów, zmniejszenie izolacji i samotności, naukę wskazówek dotyczących radzenia sobie od osób, które są dalej na swojej drodze żałoby, oraz zyskanie nadziei poprzez dzielenie się historiami przezwyciężania druzgocących strat47.
Terapia Rodzinna
Terapia rodzinna może być pomocna w leczeniu żałoby skomplikowanej, pomagając rodzinie jako całości w procesie żałoby i adaptacji do straty48.
Efektywność Leczenia i Rokowania
Leczenie żałoby skomplikowanej może być wysoce skuteczne. W badaniach klinicznych, Terapia Żałoby Skomplikowanej (CGT) wykazała skuteczność u 2 na 3 osoby i jest bardziej skuteczna niż inne metody leczenia żałoby skomplikowanej, w tym terapia interpersonalna i leki przeciwdepresyjne, dlatego jest leczeniem z wyboru w przypadku żałoby skomplikowanej49.
Ponieważ żałoba skomplikowana jest stosunkowo nowym obszarem badań, badania nad skutecznością leczenia nadal trwają. W zależności od jakości programu leczenia, długości uczestnictwa i zaangażowania pacjenta, wskaźniki sukcesu wahają się od 67% do 88% osób, które poddają się leczeniu50.

Wczesna interwencja może być kluczowa w zapobieganiu rozwojowi żałoby skomplikowanej. Nie jest jasne, jak zapobiec żałobie skomplikowanej, ale uzyskanie poradnictwa wkrótce po stracie może pomóc, szczególnie osobom o zwiększonym ryzyku rozwinięcia żałoby skomplikowanej53.
Poprzez wczesne poradnictwo po stracie, można eksplorować emocje związane ze stratą i nauczyć się zdrowych umiejętności radzenia sobie. Może to pomóc zapobiec uzyskaniu przez negatywne myśli i przekonania tak silnego wpływu, że trudno je przezwyciężyć54.
Podejście Multidyscyplinarne
Leczenie żałoby skomplikowanej często wymaga multidyscyplinarnego podejścia obejmującego psychoterapię, leki i wsparcie społeczności w celu ułatwienia zdrowienia i reintegracji z codziennym życiem55.
