Leczenie przedwczesnych wytrysków wymaga najczęściej podejścia multimodalnego, łączącego terapię behawioralną, psychologiczną i farmakologiczną34. Obecnie nie istnieje definitywny sposób wyleczenia, a dostępne metody skupiają się głównie na kontrolowaniu objawów oraz wydłużeniu czasu przed wytryskiem5.

Farmakoterapia stanowi jeden z głównych filarów leczenia przedwczesnych wytrysków, szczególnie w przypadkach o podłożu neurobiologicznym6. Poniżej przedstawiono najczęściej stosowane grupy leków w terapii tego zaburzenia.

Selektywne inhibitory wychwytu zwrotnego serotoniny (SSRI)

SSRI są uznawane za złoty standard w farmakologicznym leczeniu przedwczesnych wytrysków78. Leki te zwiększają poziom serotoniny w synapsach, prowadząc do zwiększonej transmisji nerwowej postsynaptycznej, co skutkuje opóźnieniem wytrysku9. Do najczęściej stosowanych SSRI w leczeniu przedwczesnych wytrysków należą:

  • Paroksetyna – uważana za najbardziej skuteczny SSRI w leczeniu przedwczesnych wytrysków1011

Doraźne stosowanie SSRI (z wyjątkiem dapoksetyny) jest mniej skuteczne niż codzienne leczenie19. Dapoksetyna wyróżnia się szybkim działaniem i krótkim okresem półtrwania, co czyni ją odpowiednią do stosowania doraźnego przed aktywnością seksualną2021.

Miejscowe środki znieczulające

Stosowanie miejscowych środków znieczulających w celu zmniejszenia wrażliwości prącia jest prawdopodobnie najstarszą znaną metodą leczenia przedwczesnych wytrysków2223. Środki te obejmują:

  • Kremy, żele i spraye zawierające lidokainę lub prilokainę24
  • Mieszanki eutetyczne lidokainy i prilokainy (np. Fortacin)25
  • Inne preparaty zawierające miejscowe środki znieczulające26
