Leczenie amnezji opiera się przede wszystkim na terapii przyczynowej oraz rehabilitacji poznawczej, gdyż nie istnieją specyficzne farmakologiczne środki bezpośrednio leczące większość typów amnezji. W przypadku amnezji spowodowanej udarem, urazem mózgu, zespołem Wernickego-Korsakoffa czy chorobą Alzheimera stosuje się odpowiednio leczenie choroby podstawowej, suplementację tiaminy, inhibitory cholinoesterazy oraz terapię poznawczą i zajęciową. Terapie poznawczo-behawioralne (CBT), terapia dialektyczno-behawioralna (DBT), hipnoza, EMDR oraz neurofeedback są wykorzystywane w leczeniu amnezji dysocjacyjnej i zaburzeń współistniejących. Warto podkreślić, że w amnezji anterogradowej nie ma zatwierdzonych leków, a terapia koncentruje się na wsparciu funkcji pamięciowych poprzez techniki organizacyjne i uczenie proceduralne. Wartościowe są także metody wspomagające, takie jak stosowanie pomocy pamięciowych, aplikacji mobilnych oraz technik redukcji stresu, w tym medytacji i mindfulness.

Terapia amnezji – podejście ogólne

Leczenie amnezji (amnezji) koncentruje się głównie na strategiach pomagających zrekompensować problem z pamięcią oraz na leczeniu chorób podstawowych będących przyczyną zaburzeń pamięci. Obecnie nie istnieją specyficzne leki, które mogłyby bezpośrednio leczyć większość typów amnezji.12 Podejście terapeutyczne musi być dostosowane do konkretnej przyczyny amnezji, ponieważ leczenie zależy od czynnika wywołującego zaburzenia pamięci.3

W wielu przypadkach amnezja ustępuje bez leczenia. Jednak gdy stan jest spowodowany chorobą podstawową, leczenie tej choroby jest najlepszym sposobem na odwrócenie amnezji.4 Złożoność procesów mózgowych sprawia, że mało prawdopodobne jest, aby pojedynczy lek mógł rozwiązać problemy z pamięcią.5 Kompleksowa diagnoza stanowi podstawę najlepszego i najbardziej skutecznego leczenia.6

Leczenie przyczyn podstawowych amnezji

Leczenie amnezji często rozpoczyna się od identyfikacji i leczenia chorób podstawowych, które mogą powodować utratę pamięci:7

Przy zespole Wernickego-Korsakoffa, leczenie może pomóc zapobiec dalszym uszkodzeniom, chociaż większość osób nie odzyska wszystkich utraconych wspomnień. Leczenie obejmuje uzupełnianie tiaminy w organizmie, zapewnienie odpowiedniego odżywiania i abstynencję od alkoholu.12

W przypadku amnezji spowodowanej chorobą Alzheimera, leczenie lekami nazywanymi inhibitorami cholinoesterazy może pomóc w łagodzeniu objawów.13 Chociaż obecne leki nie mogą wyleczyć choroby Alzheimera, FDA zatwierdziła leki, które dzielą się na dwie kategorie: leki zmieniające przebieg choroby u osób z wczesną chorobą Alzheimera oraz leki, które mogą tymczasowo łagodzić niektóre objawy otępienia Alzheimera.14

Podejście terapeutyczne w leczeniu amnezji

Terapia poznawcza i rehabilitacja poznawcza

Terapia poznawcza i rehabilitacja są podstawowymi metodami leczenia amnezji:15

  • Pacjenci mogą pracować z terapeutą zajęciowym, aby uczyć się nowych informacji i zastępować te, które zostały utracone16
  • Mogą wykorzystywać zachowane wspomnienia jako podstawę do przyswajania nowych informacji17
  • Trening pamięci może również obejmować strategie organizowania informacji, aby łatwiej je zapamiętać i lepiej rozumieć podczas rozmów z innymi18
  • Techniki poznawcze mogą pomóc w wydobyciu zapomnianych wspomnień19

Terapia poznawcza z wykorzystaniem logopedy może być pomocna dla pacjentów z łagodną do umiarkowanej utratą pamięci.20 Terapia poznawczo-behawioralna (CBT) może być również skuteczna w leczeniu szeregu stanów psychologicznych, w tym amnezji dysocjacyjnej.21 Terapia CBT koncentruje się na zmianie dysfunkcyjnych wzorców myślenia pacjenta i wynikających z nich zachowań i uczuć.22

Stymulacja poznawcza (CST) obejmuje aktywności i ćwiczenia, które zachęcają do myślenia, koncentracji, komunikacji i pamięci u osoby z demencją. CST jest zalecana dla osób z łagodną do umiarkowanej demencją, a nie w późniejszych stadiach.23

Terapia zajęciowa

Terapia zajęciowa jest zaprojektowana w celu oceny funkcji i potencjalnych powikłań związanych z ruchem kończyn górnych, umiejętnościami życia codziennego, poznaniem, wzrokiem i percepcją. Ta metoda pomaga zintegrować połączenie ciała/umysłu, co pozwala wspomnieniom ciała/komórek zintegrować się z mózgiem.24

W przypadku stałej utraty pamięci, terapia zajęciowa może pomóc w nauce nowych informacji i pamięci dla życia codziennego. Terapeuta może również nauczyć, jak używać pomocy pamięciowych i technik organizowania informacji, aby ułatwić ich przywoływanie.25

Metody psychoterapeutyczne w leczeniu amnezji

Psychoterapia lub terapia poznawczo-behawioralna (CBT) może pomóc niektórym osobom z amnezją.26 Różne podejścia terapeutyczne mogą być stosowane w zależności od przyczyny amnezji:

  • Hipnoza może być skutecznym sposobem przypominania zapomnianych wspomnień27
  • Desensytyzacja i przetwarzanie za pomocą ruchów gałek ocznych (EMDR) to technika opracowana do leczenia osób cierpiących na koszmary senne, retrospekcje i inne objawy zespołu stresu pourazowego28
  • Terapia psychoenergetyczna wykorzystuje punkty akupunkturowe i koncepcję Qi29
  • Neurofeedback pomaga w regulacji funkcji mózgu i odbudowie lub tworzeniu nowych połączeń neuronalnych30
  • Dźwięki bilateralne pomagają mózgowi odblokować się i uregulować, są skuteczne w reedukacji lewej i prawej półkuli mózgu31

W przypadku amnezji dysocjacyjnej, leczenie zwykle rozpoczyna się od stworzenia bezpiecznego i wspierającego środowiska. Samo to działanie często prowadzi do stopniowego odzyskiwania brakujących wspomnień.32 Gdy wspierające środowisko nie prowadzi do poprawy lub gdy potrzeba odzyskania wspomnień jest pilna, przepytywanie pacjentów pod hipnozą lub, rzadko, w wywołanym lekami (barbiturany lub benzodiazepiny) stanie półhipnotycznym może być skuteczne.33

Terapia dialektyczno-behawioralna (DBT) ma na celu nauczenie pacjentów umiejętności radzenia sobie z regulacją emocji, które mogą być związane ze stresującym lub traumatycznym wydarzeniem, które wywołało amnezję dysocjacyjną.34

Wsparcie technologiczne i pomoce pamięciowe

Wiele osób z amnezją uważa za pomocne korzystanie z nowoczesnych technologii:35

  • Smartfony lub tablety mogą być programowane, aby przypominać o ważnych wydarzeniach lub przyjmowaniu leków36
  • Nawet osoby z ciężką amnezją mogą, po odpowiednim treningu i praktyce, nauczyć się korzystać z elektronicznych organizerów, aby pomóc w codziennych zadaniach37
  • Prowadzenie notatek odręcznych, zdjęć, terminów i innych plików na urządzeniach elektronicznych38
  • Korzystanie z aplikacji przypominających oraz innych pomocy technicznych zaprojektowanych specjalnie dla pacjentów z amnezją39

Inne pomoce pamięciowe o niskim poziomie technologicznym, takie jak notesy, kalendarze ścienne, przypominacze o lekach i fotografie osób i miejsc, również mogą być pomocne.40

Specyficzne leczenie poszczególnych typów amnezji

Amnezja dysocjacyjna

Amnezja dysocjacyjna to typ zaburzenia dysocjacyjnego, charakteryzujący się niezdolnością do przypomnienia sobie ważnych informacji osobistych, które normalnie nie byłyby utracone przy zwykłym zapominaniu.41 Leczenie koncentruje się na:

  • Psychoterapii, czasami połączonej z hipnozą lub wywiadami ułatwionymi przez leki42
  • Stworzeniu bezpiecznego i wspierającego środowiska, co często prowadzi do stopniowego odzyskiwania wspomnień43
  • Terapii poznawczo-behawioralnej (CBT), która pomaga rozwiązać problemy dotyczące negatywnego myślenia i zachowania związanego z traumatycznym lub stresującym wydarzeniem44

Większość osób z amnezją dysocjacyjną odzyskuje swoje wspomnienia po ustąpieniu amnezji. Leczenie może przyspieszyć ten proces i pomóc w zarządzaniu skutkami odzyskiwania tych wspomnień.45

Amnezja potraumatyczna (PTA)

Amnezja potraumatyczna (PTA) to stan często obserwowany po urazowym uszkodzeniu mózgu, charakteryzujący się dezorientacją i utratą pamięci.46 Leczenie wymaga kompleksowego podejścia, obejmującego:

  • Leki do zarządzania wyzwaniami poznawczymi i emocjonalnymi związanymi z PTA47
  • Opiekę wspierającą, która koncentruje się na tworzeniu środowiska sprzyjającego powrotowi do zdrowia48
  • Techniki rehabilitacji, które pomagają pacjentom odzyskać funkcje poznawcze i radzić sobie z emocjonalnymi następstwami urazu49

PTA jest fazą powrotu do zdrowia, przez którą przechodzi osoba po urazie mózgu. Chociaż jest to bardzo stresujące dla rodziny i przyjaciół, ważne jest, aby pamiętać, że jest to faza, która minie.50

Przemijająca amnezja globalna (TGA)

Przemijająca amnezja globalna (TGA) to zespół kliniczny charakteryzujący się nagłym początkiem amnezji anterogradowej (niezdolność do tworzenia nowych wspomnień).51

Leczenie przemijającej amnezji globalnej polega głównie na opiece wspierającej i uspokajaniu, ponieważ nie jest wymagana ani dostępna żadna szczególna terapia.52 Może być konieczna obserwacja szpitalna do czasu ustąpienia deficytu pamięci. Chociaż rzadko, mogą wystąpić nawroty. Po ustąpieniu deficytów pamięci pacjenci nie wymagają ograniczeń aktywności, takich jak prowadzenie pojazdów.53

TGA można zarządzać poprzez unikanie aktywności wyzwalającej. Niektórzy lekarze zalecają stosowanie aspiryny w celu zwiększenia przepływu krwi, co opiera się na twierdzeniu, że zmniejszony przepływ krwi powoduje utratę pamięci.54

Amnezja anterogradowa

Amnezja anterogradowa to rzadka, ale poważna forma utraty pamięci, w której osoba nie może tworzyć nowych wspomnień po zdarzeniu wywołującym amnezję.55 Leczenie koncentruje się na:

  • Pomocy osobom w radzeniu sobie z utratą pamięci, w tym korzystaniu z dzienników, notatek, przypomnień i wsparcia rodziny56
  • Uczeniu się proceduralnym, które obejmuje naukę poprzez umiejętności motoryczne i poznawcze, takie jak jazda na rowerze lub prowadzenie samochodu57
  • Torowaniu, które polega na dawaniu wskazówek pomagających komuś się uczyć, co z czasem może pomóc im zapamiętać informacje przy mniejszej liczbie wskazówek58

Nie ma FDA-zatwierdzonego leczenia farmakologicznego amnezji anterogradowej. Suplementy witaminy B1 (tiaminy) mogą być stosowane w przypadkach, gdy występuje niedobór witaminy.59

Amnezja retrogradowa

Amnezja retrogradowa to stan, który uniemożliwia dostęp do wspomnień utworzonych przed początkiem amnezji. Jest spowodowana uszkodzeniem regionów mózgu związanych z przechowywaniem pamięci.60

Nie ma specyficznego leczenia amnezji retrogradowej. Ważne jest najpierw zrozumienie, co powoduje ten typ utraty pamięci, a następnie leczenie przyczyny podstawowej:61

  • Podjęcie kroków w celu złagodzenia stresu i leczenie niedoboru tiaminy może powstrzymać pogorszenie amnezji retrogradowej62
  • Psychoterapia może być pomocna dla osób doświadczających amnezji retrogradowej z powodu stresu lub traumy63
  • Intensywna rehabilitacja może być potrzebna do leczenia utraty pamięci związanej z urazowym uszkodzeniem mózgu64

Amnezja retrogradowa jest zwykle stopniowana czasowo, co oznacza, że najpierw dotknięte są najnowsze wspomnienia, a najstarsze wspomnienia są zwykle oszczędzone.65

Wsparcie i edukacja w leczeniu amnezji

Wsparcie rodzinne i społeczne

Wsparcie rodzinne jest kluczowe w leczeniu amnezji:66

  • Spędzanie czasu razem i powracanie do starych wspomnień może pomóc przywrócić wspomnienia z przeszłości67
  • Terapia rodzinna pomaga nauczyć rodzinę pacjenta o zaburzeniu i jego przyczynach68
  • Osoby z amnezją retrogradową mogą odkryć, że wsparcie rodziny lub uczestnictwo w grupach wsparcia może pomóc w radzeniu sobie z frustracją związaną z utratą pamięci69

Wsparcie od rodziny i przyjaciół jest kluczowe w procesie powrotu do zdrowia. Silny system wsparcia może zapewnić stabilność emocjonalną i zapewnienie, gdy osoba pracuje nad swoimi wspomnieniami i emocjami.70

Terapie uzupełniające i styl życia

Oprócz konwencjonalnej terapii, niektóre osoby mogą również skorzystać z terapii uzupełniających:71

  • Medytacja i mindfulness mogą pomóc osobie zrelaksować umysł, co może pomóc w przywoływaniu zapomnianych wspomnień72
  • Techniki redukcji stresu, takie jak medytacja mindfulness, joga i ćwiczenia głębokiego oddychania, mogą pomóc w zarządzaniu lękiem i zapobieganiu dalszym epizodom amnezji73
  • Terapie kreatywne, takie jak arteterapia lub muzykoterapia, mogą pomóc w leczeniu amnezji dysocjacyjnej74
  • Odpowiednie odżywianie, szczególnie spożywanie pokarmów bogatych w kwasy tłuszczowe omega-3 oraz stosowanie diety przeciwzapalnej75

W przypadku amnezji wywołanej przez stres traumatyczny, sedacja, dużo miłości i opieki oraz (być może) leczenie psychiatryczne mogą przynieść korzyści.76 Wstrząśnienie mózgu wymaga odpoczynku, po wykluczeniu powikłań.77

Programy rehabilitacyjne i wellness

Programy poznawczego zdrowia mózgu oraz ćwiczenia fizyczne i umysłowe, wraz z zaleceniami żywieniowymi i dotyczącymi stylu życia, okazały się pomocne w poprawie pamięci i zwiększeniu odporności mózgu.78

Program Wellness dla Mózgu integruje badania naukowe i sprawdzone techniki, aby oferować jasny plan osiągnięcia optymalnej funkcji mózgu. Programy prowadzone przez ekspertów dotyczą pięciu kluczowych czynników stylu życia dla zdrowego mózgu:79

  • Trening pamięci
  • Coaching żywieniowy
  • Zarządzanie stresem
  • Optymalizacja sprawności fizycznej
  • Regulacja snu

Nowe podejścia terapeutyczne i badania

Eksperymentalne metody leczenia

Badacze eksperymentują obecnie z lekami wywołującymi amnezję w celu lepszego zrozumienia ludzkiej pamięci i opracowania lepszych leków do leczenia zaburzeń psychicznych i zaburzeń pamięci.80 Badacze uważają, że leki te pomogą pacjentom z zespołem stresu pourazowego lepiej przetwarzać traumę bez emocjonalnego przeżywania traumy.81

Niektóre nowe i wschodzące leki są zaprojektowane, aby pomóc odbudować części mózgu związane z amnezją i utratą pamięci.82 Pokazano również, że różne suplementy pomagają pacjentom z amnezją odzyskać utracone wspomnienia.83

Inna eksperymentalna metoda opisana w przeglądzie z 2021 roku nazywana „visual lifelogging” wykorzystuje kamerę noszoną na ciele do rejestrowania codziennych aktywności życiowych. Badacze odkryli, że ta metoda może być skuteczna w pomaganiu w odzyskiwaniu pamięci u pacjentów z amnezją.84

Terapia komórkami macierzystymi

Innowacyjne podejście do leczenia amnezji obejmuje wykorzystanie terapii komórkami macierzystymi. W jednym przypadku pacjent, który cierpiał na ciężką epilepsję i miał problemy z pamięcią w wyniku epilepsji i urazu mózgu, przeszedł leczenie z zastosowaniem wstrzyknięć komórek macierzystych pochodzących z tkanki tłuszczowej oraz osocza bogatopłytkowego, co przyczyniło się do jego powrotu do zdrowia.85

Leczenie amnezji przy schorzeniach współistniejących

Amnezja i uzależnienia

Amnezja dysocjacyjna i uzależnienie często współwystępują, wymagając kompleksowego podejścia terapeutycznego, takiego jak leczenie podwójnej diagnozy. Ta konkretna metoda leczenia uzależnień koncentruje się na pomocy ludziom w wyleczeniu z uzależnienia i współwystępujących zaburzeń, takich jak amnezja dysocjacyjna.86

Plan leczenia będzie dostosowany do indywidualnych potrzeb i może obejmować:87

  • Terapia poznawczo-behawioralna (CBT)
  • Terapia dialektyczno-behawioralna (DBT)
  • Terapia desensytyzacji i przetwarzania za pomocą ruchów gałek ocznych (EMDR)
  • Leki
  • Grupy wsparcia

Skuteczne leczenie amnezji dysocjacyjnej i uzależnienia razem wymaga, aby terapie koncentrowały się na przetwarzaniu traumy, dzięki czemu osoba może sobie z nią poradzić i iść naprzód, zamiast tłumić ją lub pozwalać, aby ich umysł nadal utrzymywał te wspomnienia niedostępne.88

Amnezja i współistniejące zaburzenia psychiczne

Niektóre osoby z amnezją dysocjacyjną mogą również mieć objawy depresji lub lęku.89 Chociaż nie ma leków zatwierdzonych do leczenia samej amnezji dysocjacyjnej, leki mogą być przepisywane do leczenia objawów towarzyszących lub potencjalnie współistniejących stanów zdrowia psychicznego:90

  • Antydepresanty mogą być zalecane, gdy depresja lub lęk pogarsza utratę pamięci91
  • Leki stabilizujące nastrój mogą być brane pod uwagę, jeśli zaburzenia nastroju przyczyniają się do zaburzeń pamięci92
  • Selektywne inhibitory wychwytu zwrotnego serotoniny (SSRI) mogą być przepisywane do zarządzania podstawową depresją lub lękiem, które przyczyniają się do utraty pamięci93

Leczenie zwykle obejmuje terapie mające na celu pomoc pacjentowi w wyznaczaniu i osiąganiu celów oraz w nauce korzystania ze zdrowych technik radzenia sobie i relaksacji.94

Prognozy i efekty leczenia amnezji

Czynniki wpływające na prognozy

Prognoza dla amnezji dysocjacyjnej jest ogólnie pozytywna przy odpowiednim leczeniu. Większość pacjentów dobrze reaguje na leczenie i ostatecznie odzyska wspomnienia.95 To, jak szybko lub całkowicie one powrócą, zależy od wielu czynników, w tym od obecnego środowiska osoby i poziomu wsparcia, ogólnego stanu zdrowia oraz ciężkości traumy.96

Większość osób wraca do zdrowia, ale niektórzy nigdy nie odzyskają wszystkich swoich wspomnień. Ważne jest, aby jak najszybciej rozpocząć leczenie amnezji, co poprawia wyniki leczenia.97

Ważne jest również kontynuowanie pracy z wyszkolonym terapeutą nawet po powrocie wspomnień. Może to pomóc uniknąć dodatkowego stresu w przyszłości.98

Ograniczenia obecnych metod leczenia

Chociaż nastąpiła poprawa, gdy pacjenci otrzymują określone leczenie, nadal nie ma rzeczywistego lekarstwa na amnezję. To, w jakim stopniu pacjent wraca do zdrowia i jak długo będzie trwać amnezja, zależy od typu i ciężkości zmiany chorobowej.99

Złożoność procesów mózgowych sprawia, że mało prawdopodobne jest, aby pojedynczy lek mógł rozwiązać problemy z pamięcią.100 Badania mogą pewnego dnia doprowadzić do nowych metod leczenia zaburzeń pamięci.101

Znaczenie wczesnej interwencji

Rozpoczęcie leczenia przed lub tak szybko, jak tylko pojawią się objawy, może zwiększyć skuteczność leczenia.102 Otrzymanie leczenia zanim amnezja dysocjacyjna stanie się bardziej nasilona jest ważne i poprawia wyniki leczenia.103

Leczenie jest często znacznie bardziej skuteczne, gdy stan jest diagnozowany we wczesnych stadiach. Jeśli otępienie pozostaje nieleczone, objawy mogą mieć znaczący wpływ na jakość życia i prowadzić do różnych powikłań zdrowotnych i medycznych.104

Jeśli zauważysz objawy amnezji anterogradowej u siebie lub u kogoś, z kim jesteś blisko, może być konieczne uzyskanie diagnozy medycznej i leczenia od pracownika służby zdrowia tak szybko, jak to możliwe.105
