Zmiana techniki i sprzętu

Iniekcje toksyny botulinowej
Precyzyjne wstrzyknięcie toksyny botulinowej, takiej jak onabotulinumtoksyna A (Botox), incobotulinumtoksyna A (Xeomin), abobotulinumtoksyna A (Dysport) lub toksyna botulinowa typu B (Myobloc), do nadmiernie aktywnych mięśni może być stosowane w leczeniu dystonii ogniskowej810. - Przezwyciężyć ypsy poprzez uwolnienie się od niewyjaśnionej utraty zdolności wykonywania określonych umiejętności

Według specjalistów pracujących z baseballistami, technika Brainspotting wraz z EMDR jest jednym z najskuteczniejszych podejść terapeutycznych w leczeniu ypsów717.
EMDR
Terapia EMDR (Eye Movement Desensitization and Reprocessing – Desensytyzacja i przetwarzanie za pomocą ruchu gałek ocznych) to podejście pierwotnie zaprojektowane do leczenia zespołu stresu pourazowego, które jest również stosowane w leczeniu ypsów18.

W jednym z badań, wszyscy uczestnicy ukończyli leczenie bez powikłań, a dane z obserwacji sześciu golfistów po interwencji wykazały pozytywny wpływ terapii na objawy ypsów.
EFT (Emotional Freedom Techniques)
Technika Emocjonalnej Wolności (EFT) to unikalna forma akupunktury, która wykorzystuje opukiwanie zamiast igieł do stymulacji punktów akupresury na głowie i tułowiu5. EFT może pomóc w przezwyciężeniu ypsów poprzez2021:

Rutyna przed wykonaniem uderzenia/rzutu
Opracowanie i konsekwentne stosowanie rutyny przed wykonaniem ruchu może pomóc sportowcom uprościć myślenie i przygotować się do działania13. Rutyna ta może obejmować:
- Techniki oddechowe
- Wizualizację udanego wykonania
- Słowa-klucze (ang. cue words)
- Koncentrację na celu zamiast na mechanice ruchu31
Ukierunkowana wyobraźnia zorientowana na rozwiązanie (SFGI)
Solution-Focused Guided Imagery (SFGI) wydaje się być skuteczną alternatywną metodą leczenia w redukcji objawów u golfistów doświadczających ypsów32. W jednym z badań uczestnik doświadczający średnio 9,2 przypadków ypsów na rundę podczas fazy wyjściowej badania, zdołał zmniejszyć liczbę ypsów praktycznie do zera, ze średnią 0,2 przypadku na rundę podczas leczenia33.
Kluczowym elementem tego podejścia jest skupienie się na pozytywnym rozwiązaniu, a nie na negatywnym doświadczeniu – koncentracja na procesie poprawy i wizualizacja siebie wykonującego dane zadanie prawidłowo34.
Podejścia integracyjne i multidyscyplinarne
Ze względu na złożoną naturę ypsów, skuteczne leczenie często wymaga integracji różnych podejść. Jak zauważył jeden z badaczy, nie warto tracić czasu na określanie przyczyny dolegliwości – zamiast tego należy skoncentrować się na przezwyciężeniu objawów35.
W przypadku baseballistów zaleca się potrójne podejście7:
- Nawiązanie relacji terapeutycznej między klientem a klinicystą
- Analiza zdolności sportowca do psychosomatycznej samokontroli
- Wykorzystanie EMDR i/lub Brainspotting do przetwarzania blokad mentalnych
Dodatkowo, sportowcy mogą korzystać z modyfikacji treningów, takich jak korzystanie z „zastępczych pałkarzy” podczas ćwiczeń, aby odczulić się na doświadczenie rywalizacji z prawdziwymi pałkarzami36.
Istotne jest również podejście, które uwzględnia zarówno ciało, jak i umysł. Interwencje ukierunkowane na poznanie i mięśnie lub podejścia łączone mogą stanowić najbardziej efektywny pakiet leczniczy dla dystonii ogniskowej37.
Skuteczność metod leczenia ypsów – zalecenia oparte na dowodach
Mimo różnorodności proponowanych metod leczenia ypsów, dowody naukowe na ich skuteczność są ograniczone. Systematyczny przegląd badań wykazał3839:
- Ograniczone dowody wspierające pozytywne efekty wyobraźni motorycznej dla golfistów z ypsami typu I
- Praktycznie brak formalnych dowodów wspierających lub negujących skuteczność leków, iniekcji toksyny botulinowej, rutyn przed-wykonawczych i innych interwencji
Biorąc pod uwagę niski poziom dowodów, niską jakość metodologiczną i brak analizy statystycznej w badaniach, nie można wyciągnąć jednoznacznych wniosków dotyczących skuteczności jakiegokolwiek leczenia ypsów40.
Niemniej jednak, klinicyści powinni łączyć dostępne dowody naukowe ze swoją wiedzą kliniczną, wartościami sportowców i okolicznościami, aby umożliwić jak najlepsze podejmowanie decyzji. Mogą rozważyć wyobraźnię motoryczną dla sportowców z ypsami typu I jako potencjalną interwencję. Zastosowanie innych interwencji, takich jak rutyny przed-wykonawcze, leki, iniekcje toksyny botulinowej, powinno być uzasadnione indywidualnymi potrzebami sportowca i solidnym rozumowaniem klinicznym40.
Innowacyjne i eksperymentalne podejścia do leczenia ypsów
Obok tradycyjnych metod, badane są również nowsze, często eksperymentalne podejścia do leczenia ypsów:
Zabiegi neurochirurgiczne
W wyjątkowych przypadkach, gdy inne metody leczenia zawodzą, rozważane mogą być interwencje chirurgiczne:
- Talamotomia brzuszno-ustna – opisano przypadek baseballisty z dystonią związaną z rzucaniem piłką, który został skutecznie leczony talamotomią, co doprowadziło do całkowitego powrotu do zdrowia2
- Głęboka stymulacja mózgu (DBS) – bardziej radykalna metoda leczenia dystonii ogniskowej polegająca na stymulacji określonych obszarów mózgu37
Terapia psilocybinowa
Niedawne badania sugerują, że terapia psilocybinowa może być również skuteczna w przezwyciężaniu psychologicznych blokad związanych z wydajnością u sportowców. Wielu sportowców, którzy przeszli terapię psilocybinową, zgłasza znaczną poprawę swojej wydajności i samopoczucia psychicznego, opisując odnowione poczucie koncentracji, zmniejszony niepokój i zwiększoną zdolność do działania pod presją41.
Zalecenia praktyczne dla sportowców z ypsami
Dla sportowców zmagających się z ypsami, poniższe zalecenia mogą być pomocne w radzeniu sobie z tym trudnym stanem4243:
- Akceptacja trudności – przyznanie, że wydajność może nie być na zwykłym poziomie przez pewien czas, zmniejsza presję
- Zmiana skupienia – koncentracja na innych aktywnościach może pomóc przywrócić pewność siebie i zmniejszyć presję
- Zmiana podejścia – sportowcy mogą spróbować podejść do problemu lub zadania w nowy sposób
- Rozważenie innych czynników – ypsy mogą wynikać ze stresu lub niepokoju w innym obszarze życia
- Resetowanie celów – rozbicie istniejących celów na mniejsze części i skupienie się na rezultatach
Ponadto, ważne jest zrozumienie, że ypsy to nie tylko problem techniczny, ale również psychologiczny. Dr Richard Crowley, autor książki o przezwyciężaniu ypsów, prowadzi sportowców przez zestaw ćwiczeń mentalnych, aż ich niepokój i obawy ustąpią. Ostrzega jednak, że nie ma ścisłego zestawu instrukcji do pokonania tego stanu35.
Dr Charlie Maher, psycholog sportowy Cleveland Indians, unika używania terminu „ypsy”, a zamiast tego odnosi się do tego stanu jako „niewłaściwego skupienia”. Usuwa to pojęcie, że sportowiec cierpi na jakiś rodzaj poważnej dolegliwości. Maher oferuje sportowcom trzy zalecenia, gdy mierzą się z przeszkodą mentalną: nie pozwól swojemu umysłowi dryfować, zrelaksuj się i zaakceptuj to, co jest pod ręką, oraz zajmij się grą w miarę jej przebiegu. Maher radzi sportowcom, aby podejmowali małe kroki, aby stopniowo udowodnić sobie, że mogą wrócić do formy44.
Podsumowanie skuteczności terapii ypsów
Pomimo braku jednoznacznych dowodów naukowych na skuteczność poszczególnych metod leczenia ypsów, przegląd literatury wskazuje, że 12 badań wykazało ograniczone dowody na to, że trening wyobrażeniowy, słuchanie muzyki, rekonstrukcja poznawcza, trening relaksacyjny, edukowanie kolegów z drużyny, rutyny przed wykonaniem zadania, pozytywny instruktażowy self-talk podczas uderzeń oraz ćwiczenia oddechowe były skutecznymi metodami leczenia ypsów45.
Wyniki badania Mine i wsp. (2018) wskazują, że wyobraźnia motoryczna lub ukierunkowana wyobraźnia zorientowana na rozwiązanie (SFGI) wykazały umiarkowane do dużych wielkości efektu w przypadku ypsów typu I45.
Badania sugerują również, że sportowcy doświadczający ypsów rzutowych mają tendencję do myślenia „Kiedy rzut idzie dobrze, cieszę się, że nie zawiodłem”, co sugeruje, że koncentrują się na strachu przed popełnieniem błędu, co może prowadzić do utrzymywania się ypsów45.
Co ważne, wyniki wskazują na potrzebę zachęcania sportowców do przemyślenia celu uprawiania sportu, pragnienia dobrej gry i radości z gry, zamiast koncentrowania się na unikaniu błędów46.
Wyższe zlewanie poznawcze (cognitive fusion) wiąże się pozytywnie z poczuciem wysokiego napięcia wywołanego obrazami dzikiego rzutu, poczuciem niższości z powodu niemożności rzucania, nadmierną świadomością krytycznych sytuacji i częstotliwością błędów rzutowych. Dlatego dystansowanie się sportowców od negatywnych myśli może być skuteczną interwencją wspomagającą osoby z ypsami46.
Ponadto, interwencje przeciwne do zlewania poznawczego, takie jak uważność i zwracanie uwagi na całe ciało, mogą być niezbędne do poprawy objawów ypsów46.
Mimo że większość klasycznych rad dotyczących ypsów obejmuje poradnictwo, zarządzanie stresem i rehabilitację fizyczną, z sukcesami dla większości sportowców, nadal brakuje silnych dowodów naukowych potwierdzających skuteczność poszczególnych metod leczenia. Konieczne są dalsze badania o lepszej jakości metodologicznej, aby informować sportowców, trenerów i klinicystów o najbardziej efektywnych strategiach leczenia ypsów.
