Należy podkreślić, że zaburzenia osobowości zazwyczaj nie są diagnozowane w dzieciństwie lub adolescencji, właśnie ze względu na trwający proces kształtowania się osobowości. 11 Mimo to, rozpoznanie wczesnych objawów i wdrożenie odpowiednich działań terapeutycznych może mieć kluczowe znaczenie dla przyszłego funkcjonowania osób zagrożonych rozwojem tego zaburzenia. 12

Identyfikacja osób z grupy ryzyka

Wytyczne NICE (National Institute for Health and Care Excellence) podkreślają znaczenie identyfikacji osób zagrożonych rozwojem zaburzeń osobowości. 13 Chociaż zalecenia te dotyczą głównie innych zaburzeń osobowości (np. antyspołecznego zaburzenia osobowości), podobne podejście można zastosować w identyfikacji ryzyka rozwoju schizoidalnego zaburzenia osobowości:

  • Ustanowienie solidnych metod identyfikacji dzieci zagrożonych rozwojem problemów z zachowaniem
  • Identyfikacja osób podatnych już na etapie przedporodowym
  • Wdrażanie różnorodnych interwencji, od zarządzania gniewem po zajęcia dla rodziców 14

Zaburzenie to ma prawdopodobnie swoje korzenie w dzieciństwie, a charakterystyczne cechy są zazwyczaj widoczne w późnych latach nastoletnich lub wczesnej dorosłości. 15 Dlatego szczególną uwagę należy zwrócić na dzieci wykazujące tendencje do wycofania społecznego, trudności w nawiązywaniu relacji i ograniczoną ekspresję emocjonalną.

Podejście terapeutyczne jako element zarządzania zaburzeniem

Chociaż nie można zapobiec rozwojowi schizoidalnego zaburzenia osobowości, odpowiednie leczenie może pomóc osobom z predyspozycjami do tego stanu nauczyć się bardziej produktywnych sposobów modyfikowania niepomocnych zachowań i myśli. 16 Wczesne poszukiwanie pomocy po pojawieniu się objawów może pomóc zmniejszyć zakłócenia w życiu osoby, jej rodziny i przyjaźni. 17

    • Regularne uczestnictwo może zapewnić skuteczną drogę do poprawy funkcjonowania 25

Rola sieci wsparcia

Oprócz formalnych interwencji terapeutycznych, istotne jest również:

  • Promowanie korzystania przez pacjenta z istniejących systemów wsparcia społecznego
  • Angażowanie rodziny pacjenta w proces leczenia, co może zapewnić cenny nadzór zapobiegający negatywnym zmianom w stanie pacjenta
  • Edukowanie zarówno pacjenta, jak i członków jego rodziny na temat sposobów stabilizacji warunków życia pacjenta 26

Stosowanie standaryzowanych ocen jakości życia pacjenta może ujawnić możliwości poprawy jego codziennego funkcjonowania, co prawdopodobnie przyczyni się do poprawy jego codziennego życia. 27

Podsumowanie możliwości prewencyjnych

Chociaż schizoidalnego zaburzenia osobowości generalnie nie można zapobiec, istnieją działania, które mogą złagodzić jego przebieg i poprawić funkcjonowanie osób dotkniętych tym zaburzeniem:

  • Wczesna identyfikacja objawów i interwencja terapeutyczna, szczególnie u młodzieży
  • Rozwijanie umiejętności społecznych i strategii komunikacyjnych
  • Terapia poznawczo-behawioralna (CBT) w przypadku współwystępowania innych zaburzeń psychicznych
  • Terapia grupowa jako narzędzie rozwoju umiejętności społecznych
  • Włączenie systemów wsparcia społecznego i rodziny w proces terapeutyczny 282930

Należy pamiętać, że rozwój tego zaburzenia jest prawdopodobnie wynikiem kombinacji czynników genetycznych, środowiskowych i neurologicznych, co komplikuje opracowanie skutecznych strategii prewencyjnych. 31 Niemniej jednak, korzyści z terapii są liczne, nawet jeśli pacjent nie odczuwa znacznego dyskomfortu, ponieważ nauka umiejętności społecznych i komunikacyjnych jest bardzo pomocna w funkcjonowaniu w środowisku szkolnym lub zawodowym. 32
