Epidemiologicznie dotyka około 2 miliardów osób na świecie, w tym 128 milionów w USA (38% populacji). Nie istnieją naukowo potwierdzone metody zapobiegania starczowzroczności, a ćwiczenia mięśni oka, suplementacja witaminami (A, C, E, luteina) czy zmiany stylu życia nie wpływają na elastyczność soczewki. Kluczowe jest jednak regularne monitorowanie wzroku po 40. roku życia oraz edukacja pacjentów o charakterze choroby i możliwościach korekcji.

Starczowzroczność (Presbyopia) – Definicja i charakterystyka

Starczowzroczność (presbyopia) to naturalne, związane z wiekiem schorzenie narządu wzroku, charakteryzujące się postępującym ograniczeniem zdolności oka do akomodacji i ostrego widzenia obiektów znajdujących się blisko. Proces ten rozpoczyna się zazwyczaj około 40. roku życia, kiedy to soczewka oka staje się mniej elastyczna, co ogranicza jej zdolność do zmiany kształtu podczas patrzenia na bliskie obiekty.12 Objawy zwykle nasilają się do około 65. roku życia, po czym zmiany w widzeniu stabilizują się.3 Według danych epidemiologicznych, starczowzroczność występuje u blisko 2 miliardów ludzi na świecie, z czego około 517 milionów doświadcza upośledzenia widzenia z powodu braku odpowiedniej korekcji wzroku do bliży.4 W Stanach Zjednoczonych dotyka ona około 128 milionów osób, co stanowi 38% populacji tego kraju.5

Możliwość zapobiegania starczowzroczności

Wbrew popularnym opiniom, starczowzroczność jest nieodłącznym elementem procesu starzenia się organizmu i nie istnieją naukowo potwierdzone metody jej zapobiegania. Jest to naturalny proces związany z wiekiem, któremu każdy człowiek prędzej czy później ulegnie.67 Żadne specjalne ćwiczenia oczu, suplementy diety czy określony styl życia nie mogą całkowicie zapobiec występowaniu starczowzroczności.8 Badania naukowe jednoznacznie wskazują, że nie ma skutecznej metody, która pozwoliłaby na całkowite uniknięcie tego stanu.9

Jest to szczególnie istotna informacja dla klinicystów, którzy powinni edukować pacjentów, że choć starczowzroczności nie można zapobiec, to można skutecznie zarządzać jej objawami przy pomocy dostępnych metod korekcyjnych, a także potencjalnie opóźnić jej wystąpienie lub złagodzić jej wpływ na codzienne funkcjonowanie poprzez wdrożenie odpowiednich praktyk zdrowotnych.10

Mity na temat zapobiegania starczowzroczności

Ćwiczenia oczu

W internecie i literaturze popularnonaukowej często można znaleźć informacje o specjalnych ćwiczeniach, które rzekomo pomagają zapobiegać starczowzroczności. Najczęściej te ćwiczenia koncentrują się na treningu mięśni oka. Należy jednak podkreślić, że nie istnieją naukowe dowody potwierdzające skuteczność takich metod.11 Ćwiczenie mięśni oka nie może zatrzymać procesu twardnienia i zmniejszenia elastyczności soczewki oka, który jest główną przyczyną starczowzroczności.12

Chociaż niektóre ośrodki medyczne sugerują, że ćwiczenia oczu mogą opóźnić progresję starczowzroczności, badania naukowe nie potwierdzają tych tez. Ćwiczenia mogą co najwyżej poprawić percepcję widzenia, ale nie odwrócą ani nie wyleczą starczowzroczności.13 Jedynie okulary, soczewki kontaktowe lub zabiegi chirurgiczne mogą skutecznie korygować ten stan.

Dieta i suplementacja

Choć odpowiednia dieta ma istotne znaczenie dla ogólnego zdrowia oczu, nie istnieją dowody naukowe potwierdzające, że jakiekolwiek specyficzne produkty spożywcze czy suplementy mogą zapobiec starczowzroczności.14 Prowadzenie zdrowego stylu życia nie zapobiega powstawaniu starczowzroczności, choć może przyczyniać się do ogólnej kondycji narządu wzroku.15

Niektóre źródła sugerują, że przyjmowanie witamin, takich jak A, C, E czy luteina, może zapobiegać starczowzroczności, jednak nie ma na to naukowych dowodów. Chociaż te składniki odżywcze są ważne dla zdrowia oczu i mogą zmniejszać ryzyko innych schorzeń ocznych, nie wpływają bezpośrednio na elastyczność soczewki związaną z wiekiem.16

Potencjalne sposoby opóźnienia lub złagodzenia starczowzroczności

Chociaż całkowite zapobieganie starczowzroczności nie jest możliwe, istnieją pewne strategie, które mogą potencjalnie opóźnić jej wystąpienie lub złagodzić jej objawy. Są to działania ukierunkowane na ogólne zachowanie zdrowia oczu i minimalizowanie czynników, które mogą przyspieszyć rozwój problemów z widzeniem.17

Regularne badania okulistyczne

Regularne badania wzroku są kluczowym elementem profilaktyki zdrowia oczu. Pozwalają na wczesne wykrycie zmian w widzeniu i wdrożenie odpowiedniego leczenia.18 Zaleca się, aby osoby po 40. roku życia poddawały się regularnym badaniom okulistycznym, nawet jeśli wcześniej nie miały problemów ze wzrokiem.19 Wczesne zdiagnozowanie starczowzroczności umożliwia dobór odpowiednich metod korekcji, co może złagodzić dyskomfort związany z tym stanem.20

Zdrowy styl życia i dieta

Zdrowy styl życia, choć nie zapobiega starczowzroczności, może korzystnie wpływać na ogólny stan zdrowia oczu. Zalecenia obejmują:

  • Zbilansowaną dietę bogatą w witaminy A, C, E oraz luteinę, które wspierają zdrowie oczu21
  • Regularne ćwiczenia fizyczne22
  • Odpowiednie nawodnienie organizmu23
  • Ograniczenie spożycia alkoholu24
  • Niepalenie tytoniu (lub rzucenie palenia)25

Ochrona oczu przed promieniowaniem UV

Noszenie okularów przeciwsłonecznych z filtrem UV pomaga chronić oczy przed szkodliwym promieniowaniem ultrafioletowym.26 Długotrwałe narażenie oczu na promieniowanie UV może przyspieszać starzenie się struktur oka, w tym soczewki, co potencjalnie może wpływać na rozwój starczowzroczności.27 Wybierając okulary przeciwsłoneczne, należy upewnić się, że zapewniają one 100% ochrony przed promieniowaniem UV.28

Ograniczenie zmęczenia oczu

Osoby wykonujące prace wymagające intensywnego widzenia z bliska, takie jak praca przy komputerze czy czytanie, mogą być bardziej narażone na wcześniejsze wystąpienie objawów starczowzroczności.29 Aby zmniejszyć zmęczenie oczu, zaleca się:

  • Robienie regularnych przerw podczas pracy z bliska (np. 10-minutowa przerwa co 1-2 godziny)30
  • Stosowanie odpowiedniego oświetlenia podczas czytania i pracy z bliska31
  • Patrzenie na obiekty znajdujące się w średniej lub dalekiej odległości, aby odciążyć mechanizm akomodacji oka32
  • Ograniczenie czasu spędzanego przed ekranami urządzeń elektronicznych33
  • Stosowanie okularów z filtrem światła niebieskiego podczas pracy z urządzeniami elektronicznymi34

Kontrola chorób współistniejących

Odpowiednie leczenie i kontrola chorób współistniejących, takich jak cukrzyca czy nadciśnienie tętnicze, może pomóc w utrzymaniu dobrego zdrowia oczu.35 Te choroby mogą prowadzić do problemów z widzeniem, które w połączeniu ze starczowzrocznością mogą znacząco obniżać jakość życia.36

Nowe badania i perspektywy w profilaktyce starczowzroczności

Badania nad kroplami do oczu

Obecnie prowadzone są intensywne badania nad nowymi metodami leczenia starczowzroczności, w tym nad kroplami do oczu, które mogłyby pomóc w jej korekcji.37 W 2021 roku FDA zatwierdziło pierwsze krople do oczu (pilocarpine hydrochloride 1,25%, Vuity) przeznaczone do leczenia starczowzroczności.38 Krople te działają przez zmniejszenie średnicy źrenicy, co zwiększa głębię ostrości i poprawia widzenie z bliska. Są skuteczne przez okres do sześciu godzin i muszą być stosowane codziennie.39

Oprócz już zatwierdzonych kropli, trwają badania nad innymi preparatami, które mogłyby potencjalnie przywrócić naturalną elastyczność soczewki. Jednym z takich preparatów jest UNR844, który znajduje się obecnie w fazie badań klinicznych. Według producenta, Novartis, substancja ta ma na celu przywrócenie naturalnej elastyczności i zdolności akomodacyjnej soczewki.40

Substancje zmiękczające soczewkę

Innym innowacyjnym podejściem do leczenia starczowzroczności jest stosowanie substancji zmiękczających soczewkę. Proponowane podejście wykorzystuje kwas liponowy i ester chlorku choliny do rozluźnienia wiązań dwusiarczkowych, które są odpowiedzialne za postępujące twardnienie soczewki.41 Wysiłki mające na celu poprawę plastyczności soczewki oferują obiecującą alternatywę dla ustalonych metod korekcji starczowzroczności poprzez przywrócenie naturalnej zdolności oka do akomodacji w starzejącym się oku.42

Techniki perceptualnego uczenia się

Stosunkowo nowym kierunkiem badań są techniki perceptualnego uczenia się, które mają na celu „przeprogramowanie” części mózgu odpowiedzialnej za widzenie. Regularne wykonywanie ćwiczeń, które obejmują patrzenie na specjalne wzorce Gabora rozmieszczone strategicznie w celu poprawy czułości kontrastu i szybkości przetwarzania, wykazało potencjał w łagodzeniu objawów starczowzroczności i poprawie ostrości widzenia.43

Ćwiczenia te mogą potencjalnie przeszkolić część mózgu zaangażowaną w widzenie, co może poprawić objawy starczowzroczności i częściowo przywrócić funkcje wzrokowe utracone wraz z wiekiem.44 Należy jednak zaznaczyć, że metody te są nadal w fazie badań i wymagają dalszej weryfikacji naukowej.

Aktualne metody korekcji starczowzroczności

Chociaż starczowzroczności nie można zapobiec, istnieje wiele skutecznych metod jej korekcji. Wybór odpowiedniej metody powinien być dostosowany do indywidualnych potrzeb pacjenta, jego stylu życia oraz współistniejących zaburzeń refrakcji.45

Korekcja okularowa

Najbardziej powszechną metodą korekcji starczowzroczności są okulary. Dostępne opcje obejmują:

  • Okulary do czytania – przeznaczone wyłącznie do pracy z bliska46
  • Okulary dwuogniskowe (bifokalne) – posiadające dwie różne moce optyczne w jednej soczewce47
  • Okulary wieloogniskowe (progresywne) – zapewniające płynne przejście między różnymi mocami optycznymi48

Soczewki kontaktowe

Soczewki kontaktowe mogą być dobrym rozwiązaniem dla osób, które nie chcą nosić okularów. Dostępne opcje to:

  • Soczewki kontaktowe wieloogniskowe – posiadające kilka stref o różnej mocy optycznej49
  • Soczewki kontaktowe do monowizji – gdzie jedno oko jest korygowane do widzenia na dal, a drugie do widzenia z bliska50

Metody chirurgiczne

Dla osób poszukujących trwałego rozwiązania problemu starczowzroczności dostępne są również opcje chirurgiczne:

  • Chirurgia refrakcyjna (LASIK, SMILE, PRK) – techniki laserowej korekcji wzroku, które mogą być zastosowane do utworzenia monowizji51
  • Refrakcyjna wymiana soczewki (RLE) – zabieg polegający na zastąpieniu naturalnej soczewki oka sztuczną soczewką wewnątrzgałkową (IOL)52
  • Wszczepy rogówkowe – małe urządzenia implantowane w rogówkę w celu poprawy widzenia bliskiego53

Warto podkreślić, że refrakcyjna wymiana soczewki nie tylko koryguje starczowzroczność, ale także zapobiega rozwojowi zaćmy w przyszłości, ponieważ naturalna soczewka, która z czasem mogłaby ulec zmętnieniu, zostaje zastąpiona sztucznym implantem.54

Krople do oczu

Jako najnowsze podejście do korekcji starczowzroczności pojawiły się krople do oczu, takie jak wspomniany wcześniej Vuity. Krople te są miotycznymi preparatami, co oznacza, że powodują zwężenie źrenicy, tworząc efekt „dziurki od klucza”, który naśladuje naturalną zdolność oka do ostrego widzenia. Pozwala to na poprawę widzenia z bliska u pacjentów ze starczowzrocznością.55

Krople te są najskuteczniejsze u pacjentów, którzy dopiero zaczynają doświadczać objawów starczowzroczności i stanowią nieinwazyjną alternatywę dla innych metod korekcji.56

Znaczenie edukacji pacjentów przez klinicystów

Klinicyści odgrywają kluczową rolę w edukacji pacjentów na temat starczowzroczności. Powinni informować pacjentów, że jest to naturalny proces starzenia się, którego nie można zapobiec, ale można skutecznie korygować.57 Wczesna edukacja pacjentów, nawet tych, którzy jeszcze nie doświadczają objawów starczowzroczności, może pomóc im lepiej przygotować się na zmiany w widzeniu, które nastąpią w przyszłości.58

Edukacja pacjentów powinna obejmować informacje o dostępnych opcjach korekcji oraz o tym, jak mogą one wpłynąć na ich codzienne funkcjonowanie i jakość życia. Klinicyści powinni również informować pacjentów o znaczeniu regularnych badań okulistycznych w celu monitorowania zmian w widzeniu i wczesnego wykrywania innych problemów ocznych.59

Przyszłość leczenia i profilaktyki starczowzroczności

Badania nad nowymi metodami leczenia i profilaktyki starczowzroczności są nadal w toku. Oprócz wspomnianych już kropli do oczu i technik zmiękczania soczewki, prowadzone są również badania nad innymi innowacyjnymi podejściami, takimi jak laserowa mikroporacja twardówki (LSM).60

Technika LSM polega na zmiękczeniu ściany twardówki w miejscach, gdzie przyczepiają się mięśnie, co czyni ją bardziej elastyczną. Ma to potencjał do rozwiązania podstawowej przyczyny starczowzroczności bez tradycyjnych kompromisów związanych z innymi metodami korekcji.61

Postęp technologiczny w dziedzinie soczewek wewnątrzgałkowych również daje nadzieję na lepsze rozwiązania dla pacjentów ze starczowzrocznością. Nowe soczewki o pełnym zakresie widzenia zostały zaktualizowane, aby zmniejszyć niepożądane efekty wzrokowe i mniejszy wpływ na czułość kontrastu.62

Wnioski

Starczowzroczność jest naturalnym procesem starzenia się, któremu nie można zapobiec, ale można skutecznie nim zarządzać. Chociaż nie istnieją naukowo potwierdzone metody zapobiegania starczowzroczności, utrzymanie ogólnego zdrowia oczu poprzez regularne badania, ochronę przed promieniowaniem UV, zdrowy styl życia i odpowiednie praktyki podczas pracy z bliska może potencjalnie opóźnić jej wystąpienie lub złagodzić jej objawy.63

Dostępne obecnie metody korekcji starczowzroczności, takie jak okulary, soczewki kontaktowe, zabiegi chirurgiczne i krople do oczu, oferują szeroki wachlarz możliwości dostosowanych do indywidualnych potrzeb pacjentów. Trwające badania nad nowymi metodami leczenia dają nadzieję na jeszcze lepsze rozwiązania w przyszłości.64

Kluczowa rola w zarządzaniu starczowzrocznością przypada klinicystom, którzy powinni edukować pacjentów na temat tego stanu i dostępnych opcji korekcji. Wczesne wykrycie i odpowiednie leczenie starczowzroczności może znacząco poprawić jakość życia pacjentów i ich funkcjonowanie w codziennych aktywnościach.65
