Mezotelioma
Leczenie
Terapie skojarzone, w tym trimodalne podejście łączące chemioterapię, operację i radioterapię, mogą wydłużyć medianę przeżycia do 20-29 miesięcy, a w przypadku mezoteliomy otrzewnej z cytoredukcją i HIPEC nawet do około 53 miesięcy. Nowości terapeutyczne to m.in. enzymatyczna terapia depletująca pegargiminazą (ADI-PEG20), która w badaniu ATOMIC-meso wydłużyła medianę przeżycia o 1,6 miesiąca oraz system NovoTTF-100L wykorzystujący pola elektryczne do hamowania wzrostu guza.

Mezotelioma – definicja i charakterystyka
Cele terapeutyczne w leczeniu mezoteliomy
- Kontrolę choroby i spowolnienie jej progresji4
- Łagodzenie objawów i poprawę jakości życia53
- Usunięcie widocznych zmian nowotworowych, jeśli to możliwe2
Do podstawowych metod należą:
Leczenie chirurgiczne
Chirurgia jest jedną z głównych opcji leczenia mezoteliomy, szczególnie w przypadku wczesnego stadium choroby. Zabiegi chirurgiczne mogą mieć charakter zarówno leczniczy, jak i paliatywny23.
Główne rodzaje zabiegów chirurgicznych w mezoteliomie opłucnej obejmują:
- Pleurektomia z dekortykacją (P/D) – usunięcie wyściółki płuca i klatki piersiowej wraz z widocznym guzem; jest to zabieg oszczędzający płuco27
- Pozaopłucnowa pneumonektomia (EPP) – radykalny zabieg polegający na usunięciu zajętego płuca wraz z wyściółką klatki piersiowej (opłucną), częściami osierdzia (otoczki serca) i przepony82
- Szeroka resekcja miejscowa – usunięcie nowotworu wraz z marginesem zdrowej tkanki8
Inne schematy chemioterapii stosowane w leczeniu mezoteliomy obejmują:
- Cisplatyna z gemcytabiną14
- Cisplatyna z raltitrexedem (Tomudex)14
- Winorelbina (Navelbine) z cisplatyną15
- HIPEC (dootrzewnowa chemioterapia w hipertermii) – stosowana głównie w mezoteliomie otrzewnej, polega na bezpośrednim podaniu podgrzanych leków chemioterapeutycznych do jamy brzusznej podczas operacji cytoredukcyjnej211
- HITHOC (wewnątrzopłucnowa chemioterapia w hipertermii) – podobna metoda stosowana w leczeniu mezoteliomy opłucnej1718
Radioterapia
Radioterapia wykorzystuje wysokoenergetyczne promieniowanie do niszczenia komórek nowotworowych.
- Leczenie neoadjuwantowe przed operacją w celu zmniejszenia guza14
Nowoczesne techniki radioterapii stosowane w leczeniu mezoteliomy to:
Immunoterapia
Immunoterapia stanowi przełom w leczeniu mezoteliomy, pomagając układowi odpornościowemu pacjenta rozpoznać i zwalczać komórki nowotworowe312.
Najważniejsze zatwierdzone schematy immunoterapii w leczeniu mezoteliomy to:
- W przypadku nawrotu choroby1
Terapie celowane
Terapie celowane wykorzystują leki lub inne substancje do identyfikacji i atakowania specyficznych komórek nowotworowych, minimalizując uszkodzenia zdrowych komórek84.
Główne terapie celowane stosowane w mezoteliomie to:
Terapia trimodalna
- Chemioterapię neoadjuwantową, następnie operację i radioterapię adjuwantową1
- Operację, a następnie chemioterapię i radioterapię adjuwantową10
- Radioterapię neoadjuwantową, a następnie operację i chemioterapię27
Badania jednoramienne fazy II wykazały przedłużone przeżycie (w porównaniu z historycznymi grupami kontrolnymi) u wybranych pacjentów, którzy otrzymali radioterapię adjuwantową po radykalnej operacji. Podobne wyniki uzyskano w badaniach nad chemioterapią neoadjuwantową (głównie z platyną i pemetreksedem lub gemcytabiną) przed radykalną operacją i radioterapią adjuwantową1.
Schematy chemoimmunologii
- Durwalumab w połączeniu ze standardową chemioterapią pierwszej linii – w dwóch jednoramiennych badaniach fazy II przekroczył predefiniowane kryteria bezpieczeństwa i aktywności w nieoperacyjnej złośliwej mezoteliomie opłucnej13
Nowe i eksperymentalne metody leczenia
Naukowcy stale badają nowe metody leczenia mezoteliomy, aby poprawić wyniki i jakość życia pacjentów2530.
Tumor Treating Fields (TTF)
Wykorzystuje on elektryczność do niszczenia komórek nowotworowych139.
TTF działa poprzez:
- Dostarczanie niskonapięciowych pól elektrycznych przez skórę do obszaru guza31
- Zakłócanie podziału komórek nowotworowych9
- Spowolnienie wzrostu guza32
FDA zatwierdziła tę terapię w maju 2019 roku w połączeniu z pemetreksedem i platyną22.
Terapia fotodynamiczna
Terapia fotodynamiczna (PDT) to ukierunkowane leczenie przeciwnowotworowe, które niszczy komórki nowotworowe bez trwałego uszkadzania otaczających tkanek1835.
Proces leczenia PDT obejmuje:
Badania kliniczne
- Wiroterapia – wykorzystanie wirusów do atakowania komórek nowotworowych25
- Terapia komórkowa CAR-T – modyfikacja komórek T pacjenta do rozpoznawania i niszczenia komórek nowotworowych3037
- Fizjoterapię w celu utrzymania sprawności i samodzielności41
Wczesne stadium (stadium I)
W przypadku zlokalizowanej mezoteliomy (stadium I) opcje leczenia obejmują1:
- Chirurgiczną resekcję en bloc wraz z przyległymi strukturami w celu zapewnienia szerokich marginesów wolnych od choroby1
- Pozaopłucnową pneumonektomię dla mezoteliomy wewnątrzjamowej1
- Pleurektomię i dekortykację z ewentualną pooperacyjną radioterapią1
- Radioterapię neoadjuwantową przed operacją – badania wykazały, że znacząco poprawia przeżycie14
- Chemioterapię po operacji14
- Leczenie multimodalne (w ramach badań klinicznych)1
Zaawansowane stadium (stadia II, III i IV)
W przypadku zaawansowanej mezoteliomy (stadia II, III i IV) opcje leczenia obejmują1:
- Systemową terapię pierwszej linii – immunoterapię (niwolumab z ipilimumabem) lub chemioterapię (pemetreksed z cisplatyną/karboplatyną)1414
- Multimodalne badania kliniczne1
W przypadku nawrotu mezoteliomy opcje leczenia obejmują1:

If you’ve been diagnosed with mesothelioma, your cancer care team will discuss your treatment options with you. It’s important to weigh the benefits of each treatment option against the possible risks and side effects. […] The main factors in selecting treatment for mesotheliomas are the location and extent of the tumor, whether it has spread to lymph nodes or other organs, and your health and personal preferences. Based on these factors, your treatment options may include: Surgery for Mesothelioma, Palliative Procedures for Mesothelioma, Radiation Therapy for Mesothelioma, Chemotherapy for Mesothelioma, Immunotherapy for Mesothelioma, Targeted Therapy for Mesothelioma, Tumor Treating Fields (TTF) Therapy for Mesothelioma. […] Mesothelioma can be hard to treat because it typically does not grow as a single tumor mass. It tends to spread along nearby surfaces, nerves, and blood vessels. This often makes it very hard to get rid of the cancer completely with surgery and/or radiation. For some people, palliative procedures might be used to help treat some symptoms of mesothelioma.

General Information About Malignant Mesothelioma Treatment […] Standard treatment for all but localized mesothelioma is generally not curative. Some patients will experience long-term survival with aggressive treatment approaches, but it remains unclear if overall survival (OS) is significantly altered by different treatment modalities or combinations of modalities. […] Extrapleural pneumonectomy may improve the recurrence-free survival of selected patients with early-stage disease, but the impact on OS is unknown. […] Pleurectomy and decortication can provide palliative relief from symptomatic effusions, discomfort caused by tumor burden, and pain caused by invasive tumor. […] Trimodality therapy refers to a combination of chemotherapy, definitive surgery, and radiation therapy. Because of the rarity of mesothelioma and the complexities of patient selection, surgical technique, and optimal sequencing of therapy, delivery of such therapy in centers with established experience and expertise in the management of mesothelioma has shown better results. […] Several single-arm phase II studies have demonstrated prolonged survival times (compared with historical controls) for selected patients who received adjuvant radiation therapy after definitive surgery. […] Most patients with pleural mesothelioma who receive radiation therapy experience pain relief, but symptom control is short-lived. […] Other single-arm phase II studies investigated neoadjuvant chemotherapy (mainly with platinum and pemetrexed or gemcitabine) followed by definitive surgery and adjuvant radiation therapy. […] These studies have also shown prolonged survival compared with historical controls; however, this advantage has yet to be confirmed in a randomized study.

- #7 Mesothelioma – Wikipediahttps://en.wikipedia.org/wiki/Mesothelioma
Mesothelioma is generally resistant to radiation and chemotherapy treatment. Long-term survival and cures are exceedingly rare. Treatment of malignant mesothelioma at earlier stages has a better prognosis. Clinical behavior of the malignancy is affected by several factors including the continuous mesothelial surface of the pleural cavity which favors local metastasis via exfoliated cells, invasion to underlying tissue and other organs within the pleural cavity, and the extremely long latency period between asbestos exposure and development of the disease. The histological subtype and the patient’s age and health status also help predict prognosis. The epithelioid histology responds better to treatment and has a survival advantage over sarcomatoid histology. […] Surgery, by itself, has proved disappointing. In one large series, the median survival with surgery (including extrapleural pneumonectomy) was only 11.7 months. However, research indicates varied success when used in combination with radiation, chemotherapy, or both. A pleurectomy/decortication is the most common surgery, in which the lining of the chest is removed. Less common is an extrapleural pneumonectomy (EPP), in which the lung, lining of the inside of the chest, the hemi-diaphragm, and the pericardium are removed. In localized pericardial mesothelioma, pericardectomy can be curative; when the tumor has metastasized, pericardectomy is a palliative care option. It is often not possible to remove the entire tumor.

For patients with localized disease, and who can tolerate a radical surgery, radiation can be given post-operatively as a consolidative treatment. The entire hemithorax is treated with radiation therapy, often given simultaneously with chemotherapy. Delivering radiation and chemotherapy after a radical surgery has led to extended life expectancy in selected patient populations. It can also induce severe side-effects, including fatal pneumonitis. […] Chemotherapy is the only treatment for mesothelioma that has been proven to improve survival in randomised and controlled trials. The landmark study published in 2003 by Vogelzang and colleagues compared cisplatin chemotherapy alone with a combination of cisplatin and pemetrexed (brand name Alimta) chemotherapy in patients who had not received chemotherapy for malignant pleural mesothelioma previously and were not candidates for more aggressive „curative” surgery. This trial was the first to report a survival advantage from chemotherapy in malignant pleural mesothelioma, showing a statistically significant improvement in median survival from 10 months in the patients treated with cisplatin alone to 13.3 months in the group of patients treated with cisplatin in the combination with pemetrexed and who also received supplementation with folate and vitamin B12.

Patients with early-stage peritoneal mesotheliomas might also benefit from surgery to take out as much of the cancer as possible. This may be combined with heated intraperitoneal chemotherapy (HIPEC). Some patients have long remissions after this treatment. (This means the cancer is under control and not growing or spreading.) […] Doctors are still studying whether giving chemotherapy (chemo) before surgery (called neoadjuvant therapy) or giving chemo and/or radiation therapy after surgery (adjuvant therapy) is helpful. Not all doctors agree on the best ways to use these treatments together. Some doctors prefer to give chemo, either before or after surgery. Radiation therapy might be used after surgery, either alone or along with chemo. […] Chemo is typically the main treatment for these cancers. It may ease symptoms and shrink or slow the growth of the cancer for a time. Sometimes chemo might be used along with an immunotherapy or targeted therapy drug, or with a device that creates electric fields in the tumor. The stage (extent) of mesothelioma is an important factor in determining a person’s treatment options. But other factors, such as whether the doctor feels the cancer is resectable (all visible cancer can be removed by surgery), as well as a persons overall health and preferences, also play a role. […] Mesothelioma tends to be hard to treat, whether the cancer is resectable or not. Its best to be treated by a team of doctors who have a lot of experience with mesothelioma. It’s also very important that you understand the goal of treatment before it starts whether it’s to try to cure the cancer or to help relieve symptoms as well as the possible benefits and risks. This can help you make an informed decision when looking at your treatment options. […] Many people with resectable pleural mesothelioma have it removed by either pleurectomy/decortication (P/D) or extrapleural pneumonectomy (EPP). Surgery is more likely to have long-term benefits in early-stage cancers, where there’s a better chance that most or all of the cancer can be removed. EPP might offer the best chance to remove the cancer, but it’s a complex and extensive operation that’s more likely to cause complications, and not all patients can tolerate it.
