Macica podwójna (uterus didelphys) to wrodzona anomalia występująca u około 0,3% kobiet, charakteryzująca się obecnością dwóch oddzielnych jam macicy, często z dwoma szyjkami i czasem podwójną pochwą. Diagnostyka opiera się na badaniu ginekologicznym, ultrasonografii, histerosalpingografii, rezonansie magnetycznym (MRI) oraz histeroskopii i laparoskopii. Objawy kliniczne mogą obejmować dysmenorrheę, obfite krwawienia miesiączkowe, dyspareunię, przewlekły ból miednicy oraz nawracające poronienia. Szczególną uwagę zwraca ciągłe krwawienie mimo stosowania tamponów u pacjentek z podwójną pochwą. W przypadkach niedrożności jednej z pochw może dochodzić do hematocolpos lub pyocolpos, co wymaga pilnej interwencji. Macica podwójna (uterus didelphys) to rzadka wada wrodzona układu rozrodczego, występująca u około 0,3% kobiet. Objawy macicy podwójnej

U większości kobiet macica podwójna nie powoduje żadnych objawów i często zostaje wykryta przypadkowo podczas rutynowego badania ginekologicznego, badań obrazowych wykonywanych z innych powodów lub podczas diagnostyki nawracających poronień.678

Ból związany z macicą podwójną

Ból jest często zgłaszanym objawem u kobiet z macicą podwójną. Może on przybierać różne formy:1149

  • Intensywny ból przed i w trakcie miesiączki
  • Ból podczas stosunku seksualnego, szczególnie gdy obecna jest przegroda pochwowa
  • Chroniczny ból miednicy

W szczególnych przypadkach, gdy jedna z pochw jest niedrożna, może dochodzić do gromadzenia się krwi menstruacyjnej (hematocolpos) lub ropy (pyocolpos), co powoduje silny ból i może prowadzić do infekcji. Ten stan jest częścią zespołu znanego jako zespół OHVIRA (obstructed hemivagina and ipsilateral renal anomaly).1514

Jeden z opisanych przypadków pacjentki o imieniu Wendy ilustruje, jak poważny może być ból związany z macicą podwójną. Gdy Wendy zaczęła miesiączkować w wieku nastoletnim, ból był tak silny, że musiała pozostawać w łóżku przez kilka dni. Dane na temat wpływu macicy podwójnej na płodność są niejednoznaczne. Niektóre badania sugerują, że płodność kobiet z macicą podwójną jest lepsza niż u kobiet z innymi anomaliami przewodów Müllera, ale nadal niższa niż u kobiet z normalną anatomią macicy.19

  • Ograniczenie wzrostu płodu – ze względu na ograniczoną przestrzeń w mniejszej macicy.
  • Rozdarcie przegrody pochwowej – w przypadku porodu pochwowego, jeśli obecna jest przegroda pochwowa.

Badanie przeprowadzone przez Heinonena wykazało, że u kobiet z macicą podwójną wskaźnik przeżywalności płodów wynosił 67,5%. Przedwczesny poród wystąpił w 21% ciąż, położenie miednicowe płodu w 43% przypadków, a cesarskie cięcie wykonano u 82% pacjentek.21

Monitorowanie ciąży

Ze względu na zwiększone ryzyko powikłań, kobiety z macicą podwójną wymagają szczególnej opieki podczas ciąży:182223

Leczenie macicy podwójnej

Leczenie macicy podwójnej zazwyczaj nie jest konieczne, jeśli nie występują żadne objawy lub inne problemy. Chirurgiczne połączenie podwójnej macicy zwykle nie jest wykonywane.132527

Postępowanie podczas ciąży
