Infekcja tasiemcem
Objawy

Objawy infekcji tasiemcem

Infekcja tasiemcem (pinworm infection) to jedna z najczęstszych infekcji pasożytniczych na świecie, szczególnie rozpowszechniona wśród dzieci w wieku szkolnym. Dorosłe osobniki Enterobius vermicularis (owsik ludzki) osiągają zazwyczaj długość od 6 do 13 mm i przypominają małe białe nitki. Pasożyt ten zamieszkuje jelito grube i odbytnicę człowieka, gdzie dojrzewa i rozmnaża się12.

Symptomy podstawowe

Do typowych objawów infekcji owsikami należą:

  • Intensywny świąd okolicy odbytu, nasilający się w godzinach nocnych1
  • Zaburzenia snu i bezsenność spowodowane świądem2
  • Rozdrażnienie i niepokój3

U kobiet i dziewczynek owsiki mogą migrować do okolicy narządów płciowych, powodując dodatkowe dolegliwości12:

Objawy dodatkowe i mniej częste

Przy znacznym nasileniu infekcji mogą wystąpić dodatkowe objawy12:

  • Okresowy ból brzucha1
  • Nudności i wymioty2
  • Utrata apetytu i spadek masy ciała3
  • Moczenie nocne u dzieci (spowodowane podrażnieniem cewki moczowej)4
  • Zaczerwienienie i bolesność skóry wokół odbytu (często na skutek drapania)5

Przebieg infekcji tasiemcem

Cykl rozwojowy pasożyta

Cykl życiowy owsika obejmuje kilka etapów12:

  2. Jaja wylęgają się w jelicie cienkim, gdzie larwy rozwijają się do formy dojrzałej2
  3. Dorosłe owsiki migrują do jelita grubego, gdzie dojrzewają płciowo i rozmnażają się3

  • Zapalenie sromu i pochwy (vulvovaginitis)1
  • Zakażenia dróg moczowych2
  • Zmiany zapalne w obrębie miednicy3
  • W rzadkich przypadkach – zmiany zapalne otrzewnej4
  • Zapalenie błony śluzowej macicy (endometriosis)5
  • Zapalenie jajowodów (salpingitis)6

Rzadkie powikłania

W literaturze medycznej opisywano również rzadkie powikłania infekcji owsikami12:

  • Zapalenie wyrostka robaczkowego – związek przyczynowy pozostaje kontrowersyjny1
  • Znaczna utrata masy ciała przy rozległych zakażeniach2
  • Zaburzenia psychospołeczne spowodowane przewlekłym świądem i zaburzeniami snu3

Czynniki ryzyka reinfekcji

Infekcja owsikami charakteryzuje się wysokim ryzykiem ponownego zakażenia12. Czynniki zwiększające prawdopodobieństwo reinfekcji to:

  • Bliski kontakt z osobami zakażonymi, szczególnie w gospodarstwach domowych1
  • Zatłoczone warunki mieszkaniowe2
  • Nieprzestrzeganie zasad higieny3
  • Obecność jaj owsików w otoczeniu (pościel, ręczniki, zabawki)4
  • Brak jednoczasowego leczenia wszystkich domowników5

Leczenie i ustępowanie objawów

Skuteczne leczenie infekcji owsikami obejmuje zarówno farmakoterapię, jak i przestrzeganie zasad higieny12.

Skuteczność leczenia

Przy właściwym leczeniu skuteczność eliminacji owsików wynosi 90-100%1. Typowy schemat leczenia obejmuje23:

Leczenie objawowe

W celu złagodzenia objawów, szczególnie świądu, można zastosować12:

  • Przemywanie okolicy odbytu ciepłą wodą z mydłem1
  • Miejscowe stosowanie 1% kremu hydrokortyzonowego na podrażnioną skórę2
  • Stosowanie maści cynkowej w celu zmniejszenia świądu3

Wskazania do konsultacji lekarskiej

Należy skonsultować się z lekarzem w przypadku123:

  • Nasilonego świądu odbytu, szczególnie w nocy1
  • Zauważenia białych, nitkowatych robaków w kale lub okolicy odbytu2
  • Utrzymywania się objawów świądu ponad tydzień po leczeniu3
  • Pojawienia się zaczerwienienia i bolesności skóry wokół odbytu4

Pilnej konsultacji lekarskiej wymagają12:

  • Gorączka i silny ból brzucha (mogące wskazywać na powikłania)1
  • Zaczerwienienie, obrzęk i bolesność okolicy narządów płciowych2
