Przeciążenia i urazy jako przyczyna bólu na górze stopy

Jedną z najczęstszych przyczyn bólu na górze stopy są przeciążenia związane z aktywnością fizyczną, szczególnie bieganiem, skakaniem lub kopaniem. Intensywny lub powtarzalny wysiłek może prowadzić do różnych stanów zapalnych i mikrourazów struktury stopy.12

Tendinitis ekstensora (zapalenie ścięgien prostowników) – to jedna z najczęstszych przyczyn bólu na górze stopy. Ścięgna prostowników przebiegają wzdłuż górnej powierzchni stopy i są odpowiedzialne za unoszenie palców i stopy. Stan zapalny tych ścięgien może być spowodowany:123

  • Nadmiernym używaniem stopy, szczególnie podczas długotrwałego chodzenia lub biegania
  • Noszeniem zbyt ciasnego obuwia wywierającego nacisk na górną część stopy
  • Płaskostopiem lub wysokim podbiciem

Złamania przeciążeniowe (stresowe) – to drobne pęknięcia w kościach stopy, najczęściej w kościach śródstopia (metatarsalnych), które znajdują się na górnej części stopy. Złamania te są zwykle wynikiem:12

  • Powtarzającego się obciążenia podczas aktywności fizycznej, jak bieganie czy skakanie
  • Nagłego zwiększenia intensywności treningu
  • Osłabienia kości spowodowanego osteoporozą
  • Aktywności sportowej bez odpowiedniego przygotowania

Złamania stresowe charakteryzują się stopniowo narastającym bólem i obrzękiem na górze stopy, który nasila się podczas aktywności i zmniejsza podczas odpoczynku.1

Skręcenia i nadwyrężenia – mogą prowadzić do bólu na górze stopy w wyniku uszkodzenia więzadeł lub mięśni. Symptomy obejmują ból, obrzęk i zasinienie, które pojawiają się po intensywnym lub powtarzalnym wysiłku.12

Uraz Lisfranca – to uszkodzenie silnego więzadła w środkowej części stopy, które podtrzymuje łuk stopy. Uraz ten może być spowodowany gwałtownym skręceniem stopy lub upadkiem, gdy stopa jest skręcona pod nienaturalnym kątem. Może również wynikać z upuszczenia ciężkiego przedmiotu na stopę lub lądowania na stopach po skoku z wysokości.12

Schorzenia strukturalne jako przyczyna bólu na górze stopy

Problemy ze strukturą stopy mogą również prowadzić do bólu na jej górnej powierzchni. Te schorzenia obejmują:1

Płaskostopie – może powodować nadmierne obciążenie ścięgien na górze stopy, prowadząc do bólu. U osób z płaskostopiem struktury na górze stopy muszą pracować ciężej, aby podnieść stopę z podłoża podczas chodzenia.12

Wysokie podbicie – stopa z wysokim łukiem ma mniejszą zdolność do amortyzacji, co może prowadzić do zwiększonego zmęczenia i bólu.12

Zespół cieśni stępu (sinus tarsi syndrome) – charakteryzuje się zapaleniem kanału stępu (sinus tarsi), czyli przestrzeni między kością piętową a kością skokową. Powoduje ból na górze stopy i po zewnętrznej stronie kostki. Jest to często następstwo skręcenia stawu skokowego.12

Ostrogi kostne (wyrośla kostne) – to bolesne twory kostne, które mogą rozwijać się wzdłuż stawów na górze stopy, powodując ból, szczególnie u pacjentów z osteoartrytycznym zwyrodnieniem stawów.12

Torbiele ganglionowe – to wypełnione płynem guzki łączące się ze ścięgnami i stawami, często występujące na górze stopy. Mogą powodować ból, jeśli uciskają nerw, mięsień lub staw.12

Schorzenia medyczne jako przyczyna bólu na górze stopy

Różne schorzenia medyczne mogą manifestować się jako ból na górze stopy:1

Dna moczanowa – jest rodzajem zapalenia stawów spowodowanym nagromadzeniem kryształów kwasu moczowego w stawach. Dna moczanowa często objawia się jako nagły, intensywny ból w stawie u podstawy dużego palca, ale może również powodować ból na górze stopy. Charakteryzuje się zaczerwienieniem, obrzękiem i ciepłem skóry oraz silnym bólem przy dotykaniu stopy.123

Zapalenie stawów – zarówno osteoartroza (choroba zwyrodnieniowa stawów), jak i reumatoidalne zapalenie stawów mogą powodować ból, sztywność i obrzęk stawów na górze stopy. Osteoartroza jest wynikiem zużycia chrząstki stawowej w czasie, podczas gdy reumatoidalne zapalenie stawów to choroba autoimmunologiczna, która atakuje tkankę wyściełającą stawy.123

Neuropatia obwodowa – to uszkodzenie nerwów obwodowych, które może powodować ból, mrowienie, pieczenie lub drętwienie na górze stopy. Neuropatia może być spowodowana:123

  • Cukrzycą
  • Chorobami autoimmunologicznymi
  • Urazem bezpośrednim nerwu strzałkowego wspólnego (gałąź nerwu kulszowego)
  • Zaburzeniami hormonalnymi
  • Chorobami nerek lub wątroby
  • Złym przepływem krwi do nerwu
  • Działaniami niepożądanymi leków (np. chemioterapii)

Zapalenie stawu śródstopno-paliczkowego (MTP) – staw ten znajduje się u podstawy każdego palca i może powodować ból na górze stopy, jeśli jest dotknięty zapaleniem stawów.1

Paluch sztywny (hallux rigidus) – to zapalenie stawu śródstopno-paliczkowego palucha, które powoduje ból na górze stopy nad dużym palcem. Stan ten różni się od palucha koślawego (hallux valgus) i dny moczanowej, które mogą dotyczyć tego samego obszaru.1

Czynniki wpływające na rozwój bólu na górze stopy

Istnieje kilka czynników, które mogą zwiększać ryzyko rozwoju bólu na górze stopy:12

Mechanizmy rozwoju bólu na górze stopy

Mechanizmy prowadzące do bólu na górze stopy są różnorodne i zależą od podstawowej przyczyny:12

Zapalenie – jest częstym mechanizmem bólu na górze stopy. Kiedy tkanki stopy są uszkodzone lub podrażnione, organizm reaguje stanem zapalnym, który obejmuje rozszerzenie naczyń krwionośnych i zwiększony przepływ krwi, co pozwala białym krwinkom napłynąć do uszkodzonego obszaru. W niektórych przypadkach przednia część stopy może stać się zaczerwieniona lub opuchnięta. Ten proces zapalny jest przyczyną bólu.1

Nieprawidłowy rozkład obciążenia – odziedziczone wady anatomiczne mogą predysponować osoby do bólu na górze stopy. Może to obejmować wysoki łuk, deformację palców młotkowatych lub różnice w długości kości śródstopia. Każdy z tych stanów może wpływać na rozkład obciążenia na stopie.1

Uciśnięcie nerwu – w przypadku neuropatii obwodowej lub dysfunkcji nerwu strzałkowego wspólnego, ból może wynikać z uciśnięcia lub uszkodzenia nerwu. Może to prowadzić do uczucia pieczenia, mrowienia lub drętwienia na górze stopy.12

Zmniejszony przepływ krwi – w przypadku chorób naczyniowych, takich jak choroba tętnic obwodowych (PAD), ból na górze stopy może wynikać z niedostatecznego dopływu krwi do nerwów, co prowadzi do ich uszkodzenia.1

Odkładanie się kryształów – w przypadku dny moczanowej, ból jest spowodowany odkładaniem się kryształów kwasu moczowego w stawach stopy. Prowadzi to do ostrego stanu zapalnego, zaczerwienienia, obrzęku i bólu.1
