Objawy raka dróg żółciowych występują zazwyczaj dopiero wtedy, gdy nowotwór blokuje przewód żółciowy12. Główne objawy związane z blokadą przepływu żółci obejmują:
Żółtaczka i objawy związane z zaburzeniami przepływu żółci
Żółtaczka (zażółcenie skóry i białkówek oczu) jest najczęstszym objawem raka dróg żółciowych1314. Pojawia się, gdy guzy blokują przepływ żółci, co prowadzi do gromadzenia się bilirubiny we krwi i tkankach. Moment wystąpienia żółtaczki zależy od lokalizacji guza – wcześniej pojawia się przy nowotworach zlokalizowanych w dystalnej części dróg żółciowych, natomiast później przy nowotworach wewnątrzwątrobowych15.

Progresja choroby i rokowanie
Rak dróg żółciowych jest nowotworem agresywnym, charakteryzującym się szybkim wzrostem i wczesnym rozsiewem5051. Z uwagi na skąpe objawy wczesnego stadium, większość pacjentów w momencie rozpoznania ma już zaawansowaną chorobę52.
Progresja raka dróg żółciowych przebiega następująco:
- Początkowo nowotwór rozwija się miejscowo w ścianie przewodu żółciowego
- Następnie nacieka okoliczne tkanki, w tym wątrobę, trzustkę i naczynia krwionośne
- W dalszej kolejności szerzy się do regionalnych węzłów chłonnych
- W zaawansowanym stadium daje przerzuty odległe, najczęściej do płuc, otrzewnej i kości
Rokowanie w raku dróg żółciowych jest zazwyczaj niekorzystne54. Pięcioletnie przeżycie w zależności od stadium choroby wynosi:
- Dla choroby ograniczonej do dróg żółciowych: 18-24%
- Dla choroby regionalnej (z zajęciem okolicznych węzłów chłonnych): 9%
- Dla choroby przerzutowej: 2-3%
Warto jednak podkreślić, że rokowanie jest znacznie lepsze we wczesnym stadium choroby, szczególnie gdy możliwe jest całkowite chirurgiczne usunięcie nowotworu. Pacjenci z wczesnym rakiem dróg żółciowych mogą osiągnąć 5-letnie przeżycie na poziomie 80-100%57.
Znaczenie wczesnego rozpoznania
Wczesne rozpoznanie raka dróg żółciowych ma kluczowe znaczenie dla poprawy rokowania5859. W początkowym stadium, gdy nowotwór jest ograniczony do dróg żółciowych, istnieje możliwość całkowitego wyleczenia poprzez radykalne leczenie chirurgiczne, a w wybranych przypadkach również przeszczepienie wątroby60.
Niestety, ze względu na skąpoobjawowy przebieg wczesnego stadium, większość nowotworów dróg żółciowych rozpoznawana jest dopiero w zaawansowanym stadium, gdy leczenie radykalne nie jest już możliwe6162.
W grupach wysokiego ryzyka, szczególnie u pacjentów z pierwotnym stwardniającym zapaleniem dróg żółciowych, zaleca się regularne badania przesiewowe pod kątem raka dróg żółciowych63.
Podsumowanie obrazu klinicznego
Rak dróg żółciowych charakteryzuje się podstępnym początkiem, z objawami pojawiającymi się zazwyczaj dopiero w zaawansowanym stadium choroby6465. Najczęstszym objawem jest żółtaczka, związana z blokadą przepływu żółci66. Inne objawy, takie jak świąd skóry, odbarwione stolce, ciemny mocz, ból brzucha czy niezamierzona utrata masy ciała, również wynikają z zaburzeń przepływu żółci lub ogólnoustrojowego wpływu nowotworu67.
Ważne jest, aby pamiętać, że podobne objawy mogą występować w innych, mniej poważnych schorzeniach, takich jak kamica żółciowa czy zapalenie dróg żółciowych6869. Niemniej jednak, utrzymujące się objawy, szczególnie postępująca żółtaczka, powinny być zawsze dokładnie zdiagnozowane przez lekarza70.
Ze względu na agresywny charakter raka dróg żółciowych i trudności w jego wczesnym rozpoznaniu, choroba ta stanowi poważne wyzwanie diagnostyczne i terapeutyczne71.
