Rokowania, prognozy i postęp choroby

Rozstępy (striae distensae) to powszechna dermatoza, dotykająca około 90% kobiet w ciąży, 70% nastolatek oraz 40% nastoletnich chłopców, szczególnie aktywnych fizycznie. Mimo braku zagrożenia dla zdrowia fizycznego, rozstępy mają istotny wpływ psychologiczny, zwłaszcza u kobiet. Naturalny proces blaknięcia trwa od 6 do 12 miesięcy, z lepszym rokowaniem w przypadku rozstępów związanych z okresem dojrzewania. Leczenie powinno być rozpoczęte po ustabilizowaniu zmian, z dokumentacją fotograficzną. Terapie takie jak tretynoina, laser barwnikowy pulsacyjny (PDL) oraz peelingi chemiczne wykazują skuteczność w poprawie wyglądu wczesnych, aktywnych rozstępów. Nowoczesne metody, w tym lasery frakcyjne CO2, radiofrekwencja igłowa oraz karboksyterapia, wpływają na remodelację kolagenu, elastyny i unaczynienia skóry, co przekłada się na poprawę estetyczną zmian.

Prognozy dotyczące rozstępów

Rozstępy (striae distensae) to częsta dolegliwość skórna, która dotyka pacjentów w różnym wieku, płci i pochodzeniu etnicznym. Choć z medycznego punktu widzenia nie stanowią zagrożenia dla zdrowia fizycznego, mogą mieć znaczący wpływ psychologiczny na osoby nimi dotknięte, szczególnie kobiety.123 Badania epidemiologiczne wskazują, że rozstępy występują u około 90% kobiet w ciąży, 70% nastolatek i 40% nastoletnich mężczyzn (z których wielu uprawia sport).4

Naturalny przebieg i poprawa rozstępów w czasie

Rozstępy są trudne do skutecznego leczenia, jednak z czasem wiele z nich ulega samoistnej poprawie.5 Nawet bez leczenia stają się mniej widoczne wraz z upływem czasu.6 Generalnie rozstępy potrzebują od sześciu do dwunastu miesięcy, aby zblednąć, choć przy zastosowaniu odpowiednich metod terapeutycznych proces ten może ulec przyspieszeniu.7

Warto zauważyć, że rokowanie jest szczególnie korzystne w przypadku rozstępów związanych z gwałtownym wzrostem w okresie dojrzewania. Te formy rozstępów z czasem stają się mniej widoczne i mają doskonałe rokowanie w porównaniu z innymi typami rozstępów.8 Nastolatki z rozstępami mogą oczekiwać, że z czasem ich zmiany będą mniej widoczne.9

Skuteczność metod leczenia i ich wpływ na rokowanie

Chociaż nie istnieje standardowa metoda leczenia rozstępów, wiele różnych terapii zostało wypróbowanych w celu poprawy ich wyglądu.10 Najlepszy moment na rozpoczęcie leczenia (z odpowiednią dokumentacją fotograficzną) to czas, gdy rozstępy się ustabilizowały.11

Leczenie z wykorzystaniem tretynoiny, lasera barwnikowego impulsowego i peelingów chemicznych znacząco poprawia kliniczny wygląd wczesnych, aktywnych rozstępów.12 Nowsze technologie, takie jak lasery i zabiegi z wykorzystaniem fal radiowych, wykazują obiecujące wyniki, ponieważ próbują modyfikować unaczynienie, kolagen, a także potencjalnie elastynę w skórze.13

Szczególnie obiecujące wyniki wydaje się dawać radiofrekwencja igłowa, ponieważ teoretycznie głębsza penetracja energii może pozwolić na „kurczenie się” rozstępów we wszystkich kierunkach, poprawiając tym samym ich ogólny wygląd.14

Lasery w terapii rozstępów

Lasery stały się w ostatnich latach popularną alternatywą terapeutyczną w leczeniu rozstępów.15 Wyniki leczenia za pomocą lasera frakcyjnego CO2 okazały się lepsze, gdy stosowano go w połączeniu z laserem barwnikowym pulsacyjnym (PDL).16

Chociaż różne rodzaje laserów były wykorzystywane w leczeniu rozstępów, ich wyniki nie są spójne. Frakcyjne lasery ablacyjne i nieablacyjne wykazały jak dotąd najlepsze rezultaty terapeutyczne.17

Innowacyjne metody leczenia rozstępów

Badania kliniczne wykazują, że zarówno karboksyterapia, jak i terapia osoczem bogatopłytkowym (PRP) są bezpieczne i skuteczne w leczeniu rozstępów, przy czym nie zaobserwowano znaczącej różnicy między tymi dwiema metodami.18 Stwierdzono istotną poprawę w przypadku rozstępów białych (striae alba) po obu terapiach, choć nie było znaczącej różnicy między nimi pod względem procentowej poprawy, odpowiedzi (według skali oceny) czy satysfakcji pacjenta.19

Zaleca się przeprowadzenie dodatkowych badań na większej liczbie pacjentów w celu lepszej oceny, a próby łączące obie metody mogą przynieść lepsze rezultaty. Przeprowadzenie większej liczby sesji zarówno karboksyterapii, jak i PRP może przynieść doskonałe wyniki.20

Podejście terapeutyczne i przewidywanie wyników

Aby zaoferować skuteczne leczenie rozstępów, ważne jest przeprowadzenie kompleksowej oceny pacjenta, w tym zebranie odpowiedniego wywiadu, ocena typu rozstępów oraz typu skóry pacjenta.21 Dostępnych jest wiele strategii terapeutycznych, ale jak dotąd żadna pojedyncza metoda nie okazała się skuteczna samodzielnie. Często konieczne są wielokrotne sesje z wykorzystaniem różnych metod terapeutycznych, które działają na skórę na różnych poziomach.22

Wnioski dotyczące rokowań

Rozstępy, choć stanowią problem kosmetyczny, nie wpływają negatywnie na zdrowie fizyczne osób nimi dotkniętych. Naturalna tendencja do blaknięcia z czasem daje podstawy do optymistycznego rokowania, szczególnie w przypadku rozstępów związanych z okresem dojrzewania. Nowoczesne metody terapeutyczne, zwłaszcza lasery frakcyjne, radiofrekwencja igłowa, karboksyterapia i terapia osoczem bogatopłytkowym, oferują obiecujące rezultaty w poprawie wyglądu rozstępów, choć często wymagają łączenia różnych technik i przeprowadzenia wielu sesji zabiegowych.232425
