Świąd odbytu (pruritus ani) jest powszechnym objawem, często związanym z wieloma etiologiami, zarówno miejscowymi, jak i ogólnoustrojowymi. Występuje u około 5% populacji, z przewagą mężczyzn w wieku 40-60 lat. Przyczyny miejscowe obejmują czynniki drażniące takie jak zanieczyszczenie kałem, nadmierna higiena, czynniki chemiczne (perfumowane mydła, detergenty, preparaty doodbytnicze), nadmierna wilgotność oraz dieta (kawa, herbata, alkohol, produkty mleczne, owoce cytrusowe, przyprawy). Choroby proktologiczne, takie jak hemoroidy, szczeliny odbytu, przetoki, brodawki, wypadanie odbytnicy i ropnie okołoodbytowe, stanowią istotną grupę przyczyn, występującą u nawet 52% pacjentów. Infekcje grzybicze (do 15% przypadków), owsiki, infekcje bakteryjne i przenoszone drogą płciową również mogą wywoływać świąd. Ponadto, choroby dermatologiczne (łuszczyca, egzema, kontaktowe zapalenie skóry, liszaj płaski) oraz schorzenia ogólnoustrojowe (cukrzyca, choroby wątroby, tarczycy, hematologiczne, zapalne jelit, niewydolność nerek) mogą manifestować się tym objawem.

Etiologia świądu odbytu (pruritus ani)

Świąd odbytu (pruritus ani) jest częstym objawem, charakteryzującym się dokuczliwym odczuciem swędzenia w okolicy odbytu, prowadzącym do nieodpartej potrzeby drapania. Nie jest on samodzielną jednostką chorobową, lecz objawem, który może mieć wiele różnych przyczyn. Świąd może występować jako wtórny (gdy zidentyfikowano konkretną przyczynę) lub pierwotny/idiopatyczny (gdy nie można ustalić jednoznacznej przyczyny). Szacuje się, że około 25-75% przypadków można przypisać konkretnej przyczynie, podczas gdy pozostałe mają charakter idiopatyczny12. Problem ten może dotykać nawet do 5% populacji, częściej mężczyzn niż kobiety (w stosunku 2:1 lub 4:1), głównie w wieku 40-60 lat34.

Miejscowe czynniki drażniące

Jedną z głównych przyczyn świądu odbytu są miejscowe czynniki drażniące5. Wśród nich wyróżnia się:

Zanieczyszczenie kałem – Niedokładne oczyszczanie po defekacji może pozostawiać cząstki kału na skórze, które działają drażniąco6. Bakterie obecne w kale wytwarzają substancje chemiczne, które są silnie drażniące dla skóry okołoodbytowej7. Wilgotność spowodowana pozostałościami kału jest jednym z najczęstszych czynników wywołujących świąd odbytu8.

Nadmierna higiena – Zbyt intensywne mycie okolicy odbytu, szczególnie z użyciem mocnych mydeł, agresywnych ściereczek lub gorącej wody, może powodować podrażnienia skóry9. Nadmierne czyszczenie może usuwać naturalne bariery ochronne skóry, co paradoksalnie pogarsza problem świądu1011.

Czynniki chemiczne – Różne substancje chemiczne mogą powodować podrażnienia lub reakcje alergiczne w okolicy odbytu12. Należą do nich:

  • Perfumowane mydła i żele pod prysznic13
  • Barwniki i zapachy w papierze toaletowym14
  • Środki zmiękczające do tkanin i detergenty15
  • Talk i dezodoranty stosowane w okolicy odbytu16

Nadmierna wilgotność – Utrzymująca się wilgoć w okolicy odbytu, spowodowana nadmiernym poceniem, luźnymi stolcami czy noszeniem obcisłej odzieży, może przyczyniać się do podrażnień skóry1920.

Czynniki dietetyczne

Wiele pokarmów i napojów może powodować lub nasilać świąd odbytu. Dzieje się tak, ponieważ mogą one działać drażniąco podczas przechodzenia przez przewód pokarmowy, a ich pozostałości w kale mogą podrażniać wrażliwą skórę okolicy odbytu2122. Wśród pokarmów najczęściej związanych ze świądem odbytu wyróżnia się:

  • Kawa (zarówno z kofeiną, jak i bezkofeinowa) – może rozluźniać mięśnie odbytu, powodując wyciek kału2324

Schorzenia proktologiczne

Choroby proktologiczne są częstą przyczyną świądu odbytu, w niektórych badaniach stwierdzano je u nawet 52% pacjentów z tym objawem43. Do najważniejszych schorzeń proktologicznych powodujących świąd należą:

Hemoroidy – Zarówno wewnętrzne, jak i zewnętrzne hemoroidy mogą powodować świąd odbytu44. Hemoroidy mogą utrudniać dokładne oczyszczanie po wypróżnieniu, a także wytwarzać śluz lub powodować niewielkie wycieki, które drażnią skórę4546. Są najczęstszym schorzeniem proktologicznym związanym ze świądem odbytu47.

Szczeliny odbytu – Niewielkie pęknięcia w wyściółce odbytu lub kanału odbytu mogą powodować świąd, ból i dyskomfort4849.

Przetoki odbytnicze – Nieprawidłowe połączenia między jelitem a skórą wokół odbytu, często związane z chorobą Leśniowskiego-Crohna, mogą prowadzić do wycieku drażniących płynów5051.

Brodawki odbytu – Wyrostki brodawkowate odbytu i kłykciny kończyste mogą być źródłem świądu52.

Wypadanie odbytnicy – Wypadanie błony śluzowej odbytu lub odbytnicy może prowadzić do świądu53.

Ropnie okołoodbytowe – Gromadzenie się ropy w okolicy odbytu, częstsze u osób z cukrzycą i otyłością54.

Wydzielina z odbytu – Uważa się, że drażniąca wydzielina z kanału odbytu może powodować świąd55.

Infekcje i pasożyty

Różnego rodzaju infekcje mogą być przyczyną świądu odbytu56. Należą do nich:

Infekcje grzybicze – Kandydoza (wywołana przez Candida albicans) i inne zakażenia grzybicze mogą powodować świąd i podrażnienie okolicy odbytu5758. Według niektórych badań, infekcje grzybicze odpowiadają za do 15% przypadków świądu odbytu59. Nadużywanie antybiotyków może prowadzić do zakażeń grzybiczych6061.

Owsiki – Są częstą przyczyną świądu odbytu, szczególnie u dzieci62. Samice owsików wychodzą z jelita w nocy, aby złożyć jaja w okolicy odbytu, co powoduje intensywny świąd, najsilniejszy w nocy i nad ranem6364. Około 20% wszystkich dzieci w Stanach Zjednoczonych będzie miało owsiki na pewnym etapie życia65.

Infekcje bakteryjne – Zakażenia bakteryjne, takie jak te wywołane przez Staphylococcus aureus lub Streptococcus pyogenes, mogą powodować świąd i podrażnienie66. Szczególnie u dzieci często występuje zakażenie paciorkowcowe okolicy odbytu (Streptococcus grupy A)67.

Infekcje przenoszone drogą płciową (STI) – Różne STI mogą powodować świąd odbytu68, w tym:

  • Opryszczka analna69
  • Rzeżączka odbytnicza70

Choroby skóry

Wiele chorób dermatologicznych może obejmować okolicę odbytu i powodować świąd75. Do najczęstszych należą:

Łuszczyca – Znaleziono ją u nawet 55% pacjentów ze świądem odbytu76. Łuszczyca może powodować czerwone, łuszczące się plamy w okolicy odbytu77.

Wyprysk/Egzema – Atopowe zapalenie skóry może obejmować obszar odbytu i powodować świąd7879.

Kontaktowe zapalenie skóry – Alergiczne kontaktowe zapalenie skóry prowadzi do zaczerwienienia, łuszczącej się skóry i tworzenia się pęcherzyków80. Może być reakcją na różne substancje, w tym mydła, detergenty, perfumy, barwniki, konserwanty, nikiel, kobalt i niektóre pokarmy81.

Łojotokowe zapalenie skóry – Może obejmować okolicę odbytu i powodować łuszczenie się i świąd82.

Liszaj płaski i liszaj twardzinowy – Mogą występować w okolicy odbytu i powodować świąd8384.

Sucha skóra – Proces starzenia może powodować suchość skóry wokół odbytu, co prowadzi do intensywnego świądu85.

Choroby ogólnoustrojowe

Niektóre choroby ogólnoustrojowe mogą mieć świąd odbytu jako jeden z objawów86. Do najczęstszych należą:

Cukrzyca – Podwyższony poziom cukru we krwi może wpływać na zdrowie skóry, prowadząc do suchości i świądu87. Cukrzyca zwiększa również ryzyko infekcji i związanego z nimi świądu88.

Choroby zapalne jelit – Choroba Leśniowskiego-Crohna i wrzodziejące zapalenie jelita grubego mogą prowadzić do świądu odbytu9596.

Niewydolność nerek – Może powodować świąd mocznicowy, w tym w okolicy odbytu97.

Zespół jelita drażliwego (IBS) – Może przyczyniać się do świądu odbytu poprzez zaburzenia funkcji jelit98.

Leki i stosowanie antybiotyków

Niektóre leki mogą powodować lub nasilać świąd odbytu99. Należą do nich:

W rzadkich przypadkach, świąd odbytu może być objawem nowotworu111. Do nowotworów, które mogą manifestować się świądem odbytu, należą:

  • Rak płaskonabłonkowy skóry okolicy odbytu112
  • Choroba Bowena – Rak przedinwazyjny skóry113
  • Pozasutkowa choroba Pageta – Rzadki nowotwór skóry114115

Do połowy pacjentów z chorobą Bowena lub chorobą Pageta okolicy odbytu może mieć świąd jako objaw116.

Czynniki psychologiczne

Czynniki psychologiczne mogą również odgrywać rolę w powstawaniu i utrzymywaniu się świądu odbytu117:

  • Lęk i stres – Mogą nasilać świąd odbytu, szczególnie w nocy118119.
  • Zaburzenia psychiczne – Mogą przyczyniać się do świądu odbytu u niektórych osób120.
  • Nadwrażliwość na bodźce – Stany lękowe mogą powodować nadmierną czujność mózgu na odczucia cielesne, które w innym przypadku mogłyby być ignorowane121.

Błędne koło świądu

Niezależnie od pierwotnej przyczyny, świąd odbytu może prowadzić do powstania „błędnego koła świądu” (itch-scratch-itch cycle), w którym drapanie powoduje większy świąd122123. Drapanie zachęca do uwalniania mediatorów zapalnych, które nasilają zaczerwienienie, intensyfikują świąd i zwiększają obszar zajęty przez suchą skórę, powodując efekt kuli śnieżnej124. Jest to częste zjawisko u pacjentów ze świądem odbytu, które może utrudniać leczenie i powodować nawroty125.

Inne przyczyny świądu odbytu

Oprócz wymienionych głównych kategorii, istnieją inne czynniki, które mogą przyczyniać się do świądu odbytu:

Nietrzymanie kału i moczu

Nietrzymanie kału (inkontynencja) i moczu, szczególnie u małych dzieci i osób starszych, może predysponować do podrażnień miejscowych i wtórnych infekcji grzybiczych126127. Przewlekła biegunka również może prowadzić do podrażnień skóry okolicy odbytu128.

Czynniki anatomiczne

Niektóre cechy anatomiczne mogą predysponować do świądu odbytu129, takie jak:

  • Nadmierna ilość włosów w okolicy odbytu (hipertrichoza okołoodbytowa)
  • Nadmierna potliwość w okolicy odbytu (hiperhidroza okołoodbytowa)
  • Głęboko osadzony odbyt (tzw. odbyt w kształcie lejka)

Ubrania i aktywność fizyczna

Obcisła odzież i bielizna, szczególnie wykonana z materiałów, które nie przepuszczają powietrza, może prowadzić do nadmiernego pocenia i podrażnień skóry130131. Intensywna aktywność fizyczna powodująca nadmierne pocenie się również może przyczyniać się do świądu odbytu132.

Praktyki seksualne

Niektóre praktyki seksualne, takie jak seks analny, mogą prowadzić do podrażnień i świądu odbytu133. Mogą również zwiększać ryzyko infekcji przenoszonych drogą płciową, które z kolei mogą powodować świąd134.

Świąd odbytu idiopatyczny

W wielu przypadkach, mimo dokładnej diagnostyki, nie udaje się zidentyfikować konkretnej przyczyny świądu odbytu – jest to tzw. świąd idiopatyczny135. Szacuje się, że nawet do 90% przypadków świądu odbytu może być idiopatycznych136. Świąd idiopatyczny rozpoznaje się po wykluczeniu wszystkich innych możliwych przyczyn137.

Niektórzy eksperci sugerują, że świąd idiopatyczny może być wynikiem kombinacji różnych czynników, takich jak dieta, higiena, indywidualna wrażliwość skóry i predyspozycje genetyczne138. Nawet w przypadkach idiopatycznych, często można zidentyfikować czynniki zaostrzające świąd, takie jak spożywanie kofeiny, czekolady, owoców cytrusowych czy pomidorów139.

Podsumowanie przyczyn świądu odbytu

Świąd odbytu jest objawem, który może mieć wiele różnych przyczyn, od prostych problemów związanych z higieną, przez choroby skóry i infekcje, aż po poważne schorzenia ogólnoustrojowe. W wielu przypadkach nie udaje się zidentyfikować jednoznacznej przyczyny. Właściwa diagnoza i leczenie zależą od dokładnego wywiadu lekarskiego, badania fizykalnego oraz, w razie potrzeby, badań dodatkowych140.

Niezależnie od przyczyny, świąd odbytu może prowadzić do błędnego koła drapania i nasilania się świądu, co utrudnia leczenie. Z tego powodu istotne jest zidentyfikowanie i leczenie przyczyny podstawowej oraz właściwe postępowanie objawowe, obejmujące odpowiednią higienę i unikanie czynników drażniących141.

Warto pamiętać, że w rzadkich przypadkach świąd odbytu może być objawem poważniejszych chorób, takich jak nowotwory, dlatego uporczywy świąd nieustępujący po zastosowaniu podstawowych środków wymaga konsultacji lekarskiej142.
