Prognoza dla pacjentów z alergią na nikiel jest generalnie bardzo dobra, gdyż choroba ta rzadko prowadzi do poważnych powikłań, o ile unika się stałej ekspozycji na alergen oraz przerwania ciągłości skóry. Wysypka alergiczna może utrzymywać się od 2 do 4 tygodni po kontakcie z niklem, a nadwrażliwość na ten metal jest trwała przez całe życie. Leczenie jest objawowe i polega głównie na unikaniu kontaktu z niklem, gdyż nie istnieje metoda całkowicie eliminująca alergię. Fototerapia może być stosowana, jednak efekty pojawiają się dopiero po kilku miesiącach regularnych zabiegów. Diagnostyka opiera się na ocenie klinicznej oraz historii ekspozycji na nikiel.

Ogólna prognoza dla pacjentów z alergią na nikiel jest doskonała. Rzadko powoduje ona poważne powikłania, chyba że czynnik wywołujący alergię stale drażni pacjenta lub jeśli u pacjenta dochodzi do przerwania ciągłości skóry (np. rany, zadrapania).1 W większości przypadków choroba ta nie zagraża życiu, jednak stanowi wyzwanie dla pacjentów ze względu na brak możliwości całkowitego wyleczenia.2
Czas trwania objawów
Wysypka spowodowana alergią na nikiel może utrzymywać się od dwóch do czterech tygodni po ekspozycji na alergen.3 Należy jednak podkreślić, że po rozwinięciu nadwrażliwości na nikiel, pacjent pozostaje uczulony na ten metal przez całe życie i musi unikać kontaktu z nim, aby zapobiec nawrotom objawów.4
Leczenie i rokowanie długoterminowe
Nie istnieje metoda leczenia, która całkowicie wyleczyłaby alergię na nikiel. Głównym celem terapii jest unikanie kontaktu z alergenem oraz odpowiednie leczenie objawów, jeśli się pojawią.5 Rozpoznanie alergicznego kontaktowego zapalenia skóry wywołanego niklem zwykle opiera się na wyglądzie skóry oraz niedawnej ekspozycji na przedmioty mogące zawierać nikiel.6
W przypadku zastosowania fototerapii jako metody leczenia reakcji alergicznej na nikiel, należy pamiętać, że efekty takiej terapii mogą być widoczne dopiero po kilku miesiącach regularnych zabiegów.7
Wpływ leczenia ortodontycznego na alergię na nikiel
Interesującym aspektem rokowania w alergii na nikiel jest wpływ leczenia ortodontycznego. Mimo że stopy zawierające nikiel są powszechnie stosowane w aparatach ortodontycznych, a nikiel jest najczęstszym alergenem kontaktowym, badania wykazują, że leczenie ortodontyczne może pełnić rolę ochronną przeciwko nadwrażliwości na nikiel.8
Metaanalizy pokazują, że po uwzględnieniu czynników zakłócających, takich jak płeć i przekłucia (piercingu), leczenie ortodontyczne wiązało się z niższym ryzykiem rozwoju nadwrażliwości (skorygowany iloraz szans 0,60; 95% przedział ufności: 0,40-0,80; p < 0,001).9 Szczególnie silny efekt ochronny obserwuje się, gdy leczenie ortodontyczne poprzedza wykonanie przekłuć ciała.10
Zapobieganie nawrotom
Najlepszą strategią zapobiegania rozwojowi alergii na nikiel jest unikanie długotrwałej ekspozycji na przedmioty zawierające ten metal. Jeśli alergia już się rozwinęła, najskuteczniejszym sposobem zapobiegania reakcjom alergicznym jest całkowite unikanie kontaktu z niklem.11
Należy pamiętać, że nadwrażliwość na nikiel może rozwinąć się zarówno po pierwszej ekspozycji, jak i po wielokrotnym lub długotrwałym kontakcie z alergenem.12 Dlatego ważne jest, aby pacjenci z rozpoznaną alergią na nikiel byli szczególnie ostrożni i konsekwentnie unikali ekspozycji na ten metal.13
Jakość życia pacjentów z alergią na nikiel
Mimo że alergia na nikiel rzadko zagraża życiu, dla wielu pacjentów stanowi ona wyzwanie w codziennym funkcjonowaniu ze względu na powszechność występowania tego metalu w otoczeniu. Pacjenci mogą skutecznie zarządzać swoją alergią poprzez unikanie przedmiotów i pokarmów zawierających nikiel oraz stosowanie odpowiedniego leczenia w przypadku wystąpienia objawów.14
Długoterminowa prognoza dla pacjentów z alergią na nikiel jest dobra, pod warunkiem stosowania się do zaleceń dotyczących unikania ekspozycji na alergen. Umiejętność identyfikacji potencjalnych źródeł niklu oraz wczesne rozpoznawanie objawów reakcji alergicznej mogą znacząco poprawić jakość życia pacjentów z tą chorobą.1516
