Choroba von Willebranda (VWD) jest najczęstszym dziedzicznym zaburzeniem krzepnięcia, charakteryzującym się defektem czynnika von Willebranda (vWF) i wtórnym obniżeniem czynnika VIII, co prowadzi do przedłużonego krwawienia. Wyróżnia się trzy typy choroby: typ 1 (łagodny, z obniżonym poziomem vWF), typ 2 (dysfunkcja vWF) oraz typ 3 (ciężki, z niemal całkowitym brakiem vWF i czynnika VIII). Objawy obejmują m.in. częste krwawienia z nosa, dziąseł, obfite miesiączki (menorrhagia), krwawienia po zabiegach chirurgicznych oraz łatwe powstawanie siniaków. Typ 3 cechuje się ciężkimi, spontanicznymi krwawieniami do stawów i mięśni, przypominającymi hemofilię. U kobiet szczególnie istotne są obfite krwawienia menstruacyjne, krwawienia okołoporodowe oraz ryzyko powikłań w okresie okołomenopauzalnym. U dzieci mogą wystąpić krwawienia z kikuta pępowiny, po obrzezaniu czy nakłuciu żyły.

Choroba von Willebranda – objawy i przebieg

Choroba von Willebranda (ang. von Willebrand disease, VWD) jest najczęstszym dziedzicznym zaburzeniem krzepnięcia krwi, które spowalnia proces tworzenia skrzepu, powodując przedłużone krwawienie po urazie. Choroba dotyka zarówno mężczyzn, jak i kobiety, ale to kobiety częściej doświadczają objawów ze względu na krwawienia miesiączkowe, podczas ciąży i porodu.123

Nasilenie objawów

Nasilenie objawów choroby von Willebranda może być różne – od bardzo łagodnych do ciężkich, zagrażających życiu. Wiele osób z chorobą von Willebranda nie zdaje sobie sprawy, że cierpi na to schorzenie, ponieważ objawy są łagodne lub wręcz nieobecne.45

Nasilenie objawów zależy w dużej mierze od poziomu czynnika von Willebranda (oraz w konsekwencji czynnika VIII) we krwi pacjenta. Niektórzy pacjenci mogą mieć tak łagodne objawy, że nigdy nie zostaną zdiagnozowani, chyba że wystąpi u nich poważny problem z krwawieniem, np. po operacji chirurgicznej lub urazie.67

Warto zaznaczyć, że objawy choroby von Willebranda mogą zmieniać się w czasie. Z wiekiem, podczas ciąży, w czasie ćwiczeń fizycznych i stresu objawy krwawienia mogą występować rzadziej.89

Najczęstsze objawy

Najczęstszym objawem choroby von Willebranda jest nieprawidłowe krwawienie. U pacjentów z chorobą von Willebranda mogą wystąpić:1011

  • Nadmierne krwawienie z rany lub po operacji chirurgicznej czy zabiegu stomatologicznym
  • Częste krwawienia z nosa, które nie ustępują w ciągu 10 minut
  • Krwawienia z nosa, które występują bez urazu (spontanicznie), występują często (5 lub więcej razy w roku)
  • Krwawienia z dziąseł
  • Obfite lub długotrwałe krwawienie miesiączkowe
  • Obfite krwawienie podczas porodu i po porodzie
  • Krew w moczu lub kale
  • Łatwe powstawanie siniaków lub siniaków guzowatych

Specyficzne objawy w zależności od typu choroby

Choroba von Willebranda występuje w kilku typach, które różnią się nasileniem objawów:1415

Typ 1 i Typ 2

Typ 1 jest najczęstszą i najłagodniejszą formą choroby. Osoby z tym typem mają nieco niższy poziom czynnika von Willebranda niż normalnie. Objawy są zazwyczaj łagodne i mogą obejmować siniaczenie, krwawienia z nosa i dziąseł, oraz u kobiet – obfite miesiączki. Pacjenci z typem 1 zwykle nie doświadczają spontanicznych krwawień, ale mogą mieć nasilone krwawienie podczas miesiączki, po urazie lub zabiegu chirurgicznym.1617

Typ 2 charakteryzuje się nieprawidłowym działaniem czynnika von Willebranda. Objawy są umiarkowane do średnich, podobne jak w typie 1, ale mogą być bardziej nasilone.1819

U osób z typem 1 lub typem 2 objawy krwawienia są zazwyczaj łagodne do umiarkowanych i obejmują:2021

  • Częste, duże siniaki po niewielkich urazach
  • Częste lub trudne do zatrzymania krwawienia z nosa
  • Przedłużone krwawienie z dziąseł po zabiegu stomatologicznym
  • Obfite lub przedłużone krwawienie miesiączkowe u kobiet
  • Krew w kale z powodu krwawienia w jelitach lub żołądku
  • Krew w moczu z powodu krwawienia w nerkach lub pęcherzu
  • Obfite krwawienie po skaleczeniu lub innym wypadku

Typ 3

Typ 3 jest najrzadszą, ale najcięższą formą choroby von Willebranda. W tym typie poziom czynnika von Willebranda i czynnika VIII jest bardzo niski lub całkowicie nieobecny.2324

Pacjenci z typem 3 mogą doświadczać wszystkich objawów występujących w typie 1 i 2, a dodatkowo mogą mieć:2526

  • Ciężkie epizody krwawienia bez wyraźnej przyczyny (krwawienia spontaniczne)
  • Krwawienia do stawów i mięśni, powodujące silny ból i obrzęk
  • Objawy przypominające hemofilię

Objawy u kobiet

Kobiety są bardziej narażone na objawy choroby von Willebranda niż mężczyźni. Szczególnie charakterystyczne dla kobiet z chorobą von Willebranda są obfite krwawienia miesiączkowe (menorrhagia), które mogą być pierwszym objawem choroby.2930

Objawy związane z miesiączką mogą obejmować:3132

  • Skrzepy krwi większe niż 2,5 cm średnicy w krwawieniu miesiączkowym
  • Konieczność zmiany podpasek lub tamponów częściej niż raz na godzinę
  • Konieczność stosowania podwójnej ochrony higienicznej podczas miesiączki
  • Objawy anemii, w tym zmęczenie, osłabienie lub duszność
  • Miesiączki trwające dłużej niż 7 dni

Kobiety z chorobą von Willebranda mogą również doświadczać nadmiernego krwawienia podczas i po porodzie, a nawet do 6 tygodni po porodzie (30% w porównaniu do 2% u kobiet bez tej choroby).3536

Problemy z krwawieniem mogą się nasilić w okresie okołomenopauzalnym, kiedy regulacja hormonalna staje się bardziej chaotyczna, co zwiększa ryzyko nieprzewidywalnych i obfitych krwawień.3738

Objawy u dzieci

U dzieci z chorobą von Willebranda mogą wystąpić specyficzne objawy, takie jak:39

  • Krwawienie z kikuta pępowiny
  • Krwiak głowy (cephalohematoma)
  • Krwiak policzka
  • Krwawienie ze spojówek
  • Krwawienie po obrzezaniu

Dzieci z łagodną postacią choroby von Willebranda mogą nie wykazywać widocznych objawów. Objawy mogą pojawić się dopiero po poważnym urazie lub zabiegu chirurgicznym.4142

Ciężkie epizody krwawienia

W ciężkich przypadkach choroby von Willebranda (głównie typ 3) mogą wystąpić:4344

Długotrwałe lub powtarzające się krwawienia mogą prowadzić do różnych powikłań:4950

  • Niedokrwistość z niedoboru żelaza – występuje, gdy organizm nie ma wystarczającej ilości żelaza do wytworzenia hemoglobiny, co może prowadzić do zmęczenia, osłabienia i duszności
  • Uszkodzenie stawów – powtarzające się krwawienia do stawów mogą prowadzić do zapalenia stawów i przewlekłego bólu
  • Obniżona jakość życia – częste krwawienia, ból i zmęczenie mogą znacząco wpływać na codzienne funkcjonowanie i jakość życia pacjentów

Anemia jako objaw wtórny

Anemia z niedoboru żelaza jest częstym powikłaniem choroby von Willebranda, szczególnie u osób z typem 3 oraz u kobiet z obfitymi miesiączkami. Kiedy dochodzi do częstych lub obfitych krwawień, organizm traci czerwone krwinki, hemoglobinę i żelazo szybciej niż jest w stanie je uzupełnić.5354

Choroba von Willebranda jest schorzeniem przewlekłym, na które nie ma lekarstwa, jednak przy odpowiednim leczeniu większość pacjentów może prowadzić normalne, aktywne życie.6667

Rokowanie zależy od typu i nasilenia choroby:6869

  • Większość osób z typem 1 może prowadzić normalne życie z jedynie łagodnymi problemami z krwawieniem
  • Pacjenci z typem 2 mają zwiększone ryzyko umiarkowanego krwawienia i powikłań, szczególnie w przypadku infekcji, operacji lub ciąży
  • Osoby z typem 3 są narażone na ciężkie krwawienia oraz krwawienia wewnętrzne, wymagają regularnego leczenia i monitorowania

Dla pacjentów leczonych w specjalistycznych ośrodkach hematologicznych, nawet z najcięższą postacią choroby, rokowanie jest korzystne, a oczekiwana długość życia jest zazwyczaj normalna.7172
