Zerwanie obrąbka stawu biodrowego
Diagnostyka i diagnoza

Zerwanie obrąbka stawu biodrowego stanowi uszkodzenie pierścienia chrząstki otaczającej panewkę stawu biodrowego, którego diagnostyka jest wyzwaniem ze względu na niespecyficzne objawy i współistnienie innych patologii stawu. Średni czas do prawidłowego rozpoznania wynosi ponad 2 lata, z koniecznością konsultacji u około 4 specjalistów i wykonania średnio 3,4 badań obrazowych. RTG służy do oceny struktury kostnej i wykluczenia innych przyczyn bólu, natomiast MRI standardowe cechuje niska czułość (~30%) i dokładność (36%) w wykrywaniu uszkodzeń obrąbka.

Wyzwania diagnostyczne

Wywiad i badanie fizykalne

  • Test konfliktu stawu biodrowego (test impingement) – wykonywany poprzez zgięcie, przywiedzenie i rotację wewnętrzną stawu biodrowego10
  • Test FABER (Flexion, Abduction, External Rotation) – zgięcie, odwiedzenie i rotacja zewnętrzna11
  • Test FADIR (Flexion, Adduction, Internal Rotation) – zgięcie, przywiedzenie i rotacja wewnętrzna12

Badania obrazowe

  • dGEMRIC (delayed gadolinium-enhanced MRI of cartilage) – specjalistyczna technika MRI umożliwiająca ocenę stanu chrząstki stawowej11

Iniekcje diagnostyczne

  • Umożliwia dokładną ocenę stopnia i lokalizacji uszkodzenia
  • Oferuje możliwość natychmiastowego przejścia do leczenia (artroskopia terapeutyczna)

Ograniczenia artroskopii diagnostycznej

  • Przepuklina sportowa
  • Konflikt udowo-panewkowy (FAI)
  • Dysplazja stawu biodrowego
  • Zapalenie kaletki krętarzowej
  • Zmiany zwyrodnieniowe stawu biodrowego
  • Złamania zmęczeniowe
  • Dolegliwości bólowe pochodzące z dolnego odcinka kręgosłupa

Współistniejące patologie

Zerwanie obrąbka stawu biodrowego rzadko występuje jako izolowane uszkodzenie. Często współistnieje z innymi patologiami w obrębie stawu biodrowego, które mogą wymagać odrębnego leczenia:930

  • Konflikt udowo-panewkowy (FAI) – często będący przyczyną zerwania obrąbka
  • Dysplazja stawu biodrowego
  • Uszkodzenia chrząstki stawowej
  • Niestabilność stawu biodrowego
  • Zapalenie błony maziowej

Identyfikacja tych współistniejących patologii jest istotna, ponieważ skuteczne leczenie zerwania obrąbka stawu biodrowego wymaga często równoczesnego leczenia tych schorzeń.9

Implikacje kliniczne diagnostyki

Prawidłowa i wczesna diagnostyka zerwania obrąbka stawu biodrowego ma istotne znaczenie dla rokowania pacjenta i wyboru optymalnej metody leczenia.31

Znaczenie wczesnej diagnostyki

Wczesna i dokładna diagnoza zerwania obrąbka stawu biodrowego jest kluczowa z kilku powodów:132

  • Nieleczone zerwanie obrąbka może prowadzić do dalszego uszkodzenia chrząstki stawowej
  • Zwiększa ryzyko rozwoju zmian zwyrodnieniowych stawu biodrowego
  • Długotrwały ból i ograniczenie funkcji stawu mogą prowadzić do zaburzeń biomechaniki i wtórnych problemów w innych częściach układu ruchu

  • Lokalizacji i rozmiaru uszkodzenia obrąbka
  • Obecności współistniejących patologii
  • Wieku i poziomu aktywności pacjenta
  • Wcześniejszego leczenia i jego efektów

Opcje leczenia zerwania obrąbka stawu biodrowego obejmują zarówno metody zachowawcze (odpoczynek, modyfikacja aktywności, fizjoterapia, leki przeciwzapalne, iniekcje), jak i leczenie operacyjne (artroskopowa naprawa obrąbka, wycięcie uszkodzonego fragmentu obrąbka, rekonstrukcja obrąbka).1826

Wybór odpowiedniej metody leczenia zależy od dokładnej diagnozy i indywidualnych czynników pacjenta, a właściwe postępowanie może zapobiec długotrwałym powikłaniom i przywrócić prawidłową funkcję stawu biodrowego.34

Podsumowanie diagnostyki

Standardowy algorytm diagnostyczny zerwania obrąbka stawu biodrowego obejmuje:836

  • Dokładny wywiad medyczny i badanie fizykalne, w tym specjalistyczne testy prowokacyjne
  • Podstawowe badania obrazowe (RTG)
  • Zaawansowane badania obrazowe (MRI, MRA)
  • W wybranych przypadkach, iniekcje diagnostyczne
  • W razie potrzeby, artroskopia diagnostyczna

Postępy w technikach diagnostycznych, w szczególności w badaniach obrazowych i artroskopii, znacznie poprawiły możliwości wczesnego i dokładnego rozpoznawania zerwania obrąbka stawu biodrowego.

You might need tests to confirm if you have a labral tear: […] […] Diagnostic injection Your provider injects a pain-relieving anesthetic into your joint. This injection helps diagnose the cause of your hip pain. If your pain improves after the anesthetic, a problem in your joint is likely the source of the pain. If you dont have any pain relief after the injection, it could mean that your hip joint is not the cause of your pain. […] […] X-ray An X-ray gives your medical provider a better look at the bones in your joint. X-ray images can show the cause of your hip pain, like hip impingement or dysplasia. […] […] MRI scan An MRI (magnetic resonance imaging) scan shows more detailed images of the soft tissues in and around your hip joint. These images help your provider determine the location and severity of the tear. An MRI shows labral tears even if you dont have any symptoms. […] […] Arthroscopy Arthroscopic surgery helps to diagnose and treat certain hip conditions. Your provider makes small keyhole incisions (cuts) and inserts a small camera into your joint to examine your hip damage.
