Afty, czyli płytkie, bolesne owrzodzenia błony śluzowej jamy ustnej, dotyczą około 25% populacji i dzielą się na trzy typy: małe (80-85% przypadków, <1 cm, gojące się w 7-14 dni bez blizn), duże (10-15%, >1 cm, głębsze, gojące się 2-6 tygodni, mogące pozostawić blizny) oraz opryszczkowate (5-10%, liczne zmiany 1-3 mm, gojące się w 7-30 dni bez blizn). Przed pojawieniem się aft występuje faza prodromalna z mrowieniem i zaczerwienieniem, a pełne owrzodzenie trwa zwykle 3-6 dni. Afty lokalizują się na ruchomych, miękkich tkankach jamy ustnej, takich jak wewnętrzna strona policzków, warg, języka, podniebienia miękkiego i dna jamy ustnej. Objawy towarzyszące mogą obejmować ból nasilający się podczas jedzenia, picia i mówienia, a w cięższych przypadkach gorączkę, powiększenie węzłów chłonnych i trudności w połykaniu.

Afty – Charakterystyka

Afty (łac. aphthous ulcers, ang. canker sores) to płytkie, bolesne owrzodzenia występujące na błonie śluzowej jamy ustnej. Są one jednymi z najczęstszych zmian w jamie ustnej, dotykając około 25% populacji1. W przeciwieństwie do opryszczki wargowej, afty nie są zakaźne i nie mogą być przenoszone z osoby na osobę2.

Rodzaje aft

Afty dzielą się na trzy główne typy, różniące się wielkością, głębokością oraz czasem gojenia34:

Afty mniejsze (minor aphthous ulcers)

Są to najczęstsze afty, stanowiące około 80-85% wszystkich przypadków5. Charakteryzują się następującymi cechami:

  • Zwykle są małe (mniej niż 1 cm średnicy, często 2-3 mm)6
  • Mają kształt owalny lub okrągły z czerwoną obwódką7
  • Goją się bez pozostawienia blizny w ciągu 1-2 tygodni8

Afty większe (major aphthous ulcers)

Stanowią około 10-15% przypadków10 i charakteryzują się:

  • Większymi rozmiarami (ponad 1 cm średnicy)11
  • Głębszym umiejscowieniem w tkance12
  • Zwykle okrągłym kształtem z wyraźnymi granicami, ale w przypadku bardzo dużych aft mogą mieć nieregularne krawędzie13
  • Znacznym bólem, często intensywniejszym niż przy aftach mniejszych14
  • Dłuższym czasem gojenia – od 2 do 6 tygodni15
  • Możliwością pozostawienia blizny po wygojeniu16

Afty opryszczkowate (herpetiform aphthous ulcers)

Są najrzadsze, występują u około 5-10% pacjentów z aftami1718, mimo nazwy nie są związane z infekcją wirusem opryszczki19:

  • Mają rozmiar punktowy, często 1-3 mm średnicy20
  • Występują w skupiskach od 10 do 100 zmian, które mogą się łączyć w jedno duże owrzodzenie21
  • Posiadają nieregularne krawędzie22
  • Goją się bez pozostawienia blizny w ciągu 1-2 tygodni23, choć niektóre źródła podają okres 10-14 dni24 lub 7-30 dni25

Objawy aft

Objawy wczesne

Na 1-3 dni przed pojawieniem się właściwej zmiany mogą wystąpić objawy prodromalne26:

  • Uczucie mrowienia lub pieczenia w miejscu, gdzie później pojawi się afta27
  • Uczucie szorstkości lub dyskomfortu w jamie ustnej28
  • Zaczerwienienie i niewielki obrzęk w miejscu przyszłej zmiany29

Pełne rozwinięcie aft

Po stadium prodromalnym, zwykle w ciągu 1-3 dni, rozwija się w pełni uformowana afta3031. Charakteryzuje się ona:

  • Białym, żółtawym lub szarym środkiem32
  • Otaczającą czerwoną, zapalną obwódką33
  • Regularnym, okrągłym lub owalnym kształtem34
  • Wyglądem przypominającym krater35
  • Znacznym bólem, który nasila się podczas jedzenia, picia lub mówienia36

Stadium pełnego rozwoju afty trwa zwykle 3-6 dni, choć w niektórych przypadkach może trwać dłużej37. W tym okresie ból może być najbardziej intensywny38.

Lokalizacja aft

Afty zwykle pojawiają się na ruchomych, miękkich tkankach jamy ustnej39, takich jak:

  • Wewnętrzna strona policzków40
  • Wewnętrzna strona warg41
  • Na języku i pod językiem42
  • Podniebienie miękkie (tylna część podniebienia)43
  • Podstawa dziąseł44
  • Dno jamy ustnej45

Objawy towarzyszące

W większości przypadków afty powodują jedynie miejscowy ból i dyskomfort46. Jednak w ciężkich przypadkach mogą wystąpić dodatkowe objawy47:

  • Gorączka48
  • Ogólne osłabienie i zmęczenie49
  • Powiększone węzły chłonne podżuchwowe50
  • Trudności w połykaniu, zwłaszcza przy aftach umiejscowionych w tylnej części jamy ustnej lub gardle51

Fazy rozwoju aft

Afty przechodzą przez kilka charakterystycznych faz od momentu pojawienia się do całkowitego wygojenia53:

  1. Faza prodromalna: uczucie mrowienia lub pieczenia na 1-3 dni przed pojawieniem się widocznych zmian54
  2. Faza tworzenia: pojawienie się zaczerwienienia i obrzęku55

Czas gojenia

Czas gojenia aft zależy od ich typu59:

  • Afty mniejsze: zwykle goją się w ciągu 7-14 dni60
  • Afty większe: mogą wymagać od 2 do 6 tygodni do całkowitego wygojenia61
  • Afty opryszczkowate: goją się zazwyczaj w ciągu 7-14 dni62

Charakterystyczny przebieg gojenia obejmuje63:

Afty mają tendencję do nawracania68. Częstość nawrotów jest różna dla różnych osób:

Należy pamiętać, że chociaż jest to rzadkie, długo utrzymująca się afta (ponad 3 tygodnie) może być objawem raka jamy ustnej i wymaga oceny przez lekarza8485.

Cechy charakterystyczne aft

Afty są bolesną, ale zwykle nieszkodliwą dolegliwością jamy ustnej86. Większość aft wygląda podobnie i ma charakterystyczne cechy87:

  • Występują wewnątrz jamy ustnej, nie na zewnątrz88
  • Są płytkie, okrągłe lub owalne89
