Rzeżączka, wywoływana przez Neisseria gonorrhoeae, jest powszechną infekcją przenoszoną drogą płciową, która może zakażać błony śluzowe narządów płciowych, gardła, odbytu oraz oczu. Okres inkubacji wynosi zwykle 2-7 dni (zakres 1-30 dni). U mężczyzn objawy pojawiają się w 90% przypadków i obejmują dyzurię, ropną wydzielinę z cewki moczowej oraz ból jąder, natomiast u kobiet infekcja często przebiega bezobjawowo (do 80%), co utrudnia diagnostykę i sprzyja transmisji. Rzeżączka może manifestować się także jako zapalenie odbytu, gardła czy spojówek, z objawami takimi jak świąd, ból gardła, ropna wydzielina z oczu, a infekcje gardła mogą utrzymywać się mediana 16 tygodni. Bez leczenia infekcja może prowadzić do poważnych powikłań, w tym zapalenia narządów miednicy mniejszej (PID) u kobiet, zapalenia najądrza i prostaty u mężczyzn oraz rozsianej infekcji gonokokowej (DGI) u około 1% pacjentów.

Rzeżączka – przegląd choroby

Rzeżączka (gonorrhoea) to powszechna choroba przenoszona drogą płciową (STI), wywoływana przez bakterię Neisseria gonorrhoeae, określaną także jako gonokok. Bakteria ta może zakażać błony śluzowe narządów płciowych, ust, gardła, odbytu oraz oczu.12 Rzeżączka rozprzestrzenia się poprzez kontakt seksualny z osobą zakażoną (stosunek waginalny, analny lub oralny) lub z matki na dziecko podczas porodu.34 Choroba ta występuje powszechnie u osób aktywnych seksualnie, szczególnie u młodych dorosłych.3

Ta infekcja jest istotnym problemem zdrowia publicznego na całym świecie, powodując znaczną zachorowalność zarówno w krajach rozwiniętych, jak i rozwijających się.1 Zachorowalność na rzeżączkę wzrosła w ciągu ostatnich 20 lat, głównie ze względu na rozwój oporności na antybiotyki.1 Jest to druga najczęściej zgłaszana bakteryjna choroba zakaźna, co podkreśla jej epidemiologiczne znaczenie w kontekście zdrowia publicznego.5

Objawy rzeżączki – informacje ogólne

Rzeżączka często nie powoduje żadnych objawów, zwłaszcza u kobiet. Bezobjawowy przebieg choroby stanowi poważny problem epidemiologiczny, gdyż umożliwia nieświadome przenoszenie infekcji na partnerów seksualnych.67 Według danych epidemiologicznych, nawet 50% zakażonych kobiet i 10-15% mężczyzn nie doświadcza żadnych objawów.89

Okres inkubacji rzeżączki, czyli czas od momentu zakażenia do wystąpienia objawów, wynosi zazwyczaj 2-7 dni, choć może wahać się od 1 do 30 dni.1011 U mężczyzn objawy najczęściej pojawiają się w ciągu 2-5 dni od zakażenia, natomiast u kobiet objawy mogą wystąpić po około 10 dniach, jeśli w ogóle się pojawią.129

Rzeżączka może dawać objawy w miejscu zakażenia, które różnią się w zależności od lokalizacji anatomicznej infekcji (narządy płciowe, odbyt, gardło, oczy) oraz płci pacjenta.1314 Objawy mogą być łagodne i niespecyficzne, co sprawia, że mogą być mylone z innymi infekcjami, takimi jak zapalenie pęcherza moczowego czy infekcja pochwy.15

Objawy rzeżączki u mężczyzn

Mężczyźni częściej niż kobiety doświadczają objawów zakażenia rzeżączką, przy czym około 90% zakażonych mężczyzn rozwija symptomy.16 Objawy te zwykle pojawiają się w ciągu 2-14 dni od zakażenia i mogą obejmować:717

  • Bolesne lub pieczące oddawanie moczu (dyzuria)614
  • Wydzielina z cewki moczowej – biaława, żółta lub zielona, o konsystencji ropnej914
  • Ból lub obrzęk jąder (rzadziej)177
  • Zapalenie i obrzęk napletka1819

W ciągu kilku dni od pojawienia się pierwszych objawów wydzielina z cewki moczowej zwykle staje się bardziej obfita, ropna i czasami może być podbarwiona krwią.11 W przypadku braku leczenia, u mężczyzn rzeżączka może rozprzestrzeniać się i powodować zapalenie najądrza (epididymitis), które objawia się bólem i obrzękiem jąder oraz zapewnia drogę do dalszego rozprzestrzeniania się infekcji.2021

Dodatkowo, rzeżączka może powodować zapalenie prostaty (prostatitis), charakteryzujące się bólem w dolnej części pleców, bolesnym oddawaniem moczu oraz problemami z ejakulacją.1522

Objawy rzeżączki u kobiet

U kobiet rzeżączka często przebiega bezobjawowo – nawet do 80% zakażonych kobiet nie doświadcza żadnych objawów lub są one tak łagodne, że pozostają niezauważone.87 Ta bezobjaowość stanowi poważny problem zdrowotny, ponieważ zwiększa ryzyko nieświadomego przenoszenia infekcji oraz rozwoju poważnych powikłań.6

Jeśli objawy występują, mogą być łagodne i często są mylone z zapaleniem pęcherza moczowego lub infekcją pochwy.17 Typowe objawy rzeżączki u kobiet obejmują:76

  • Nietypowa wydzielina z pochwy – wodnista, kremowa lub zielonkawa1014
  • Bolesne lub pieczące oddawanie moczu617
  • Krwawienie międzymiesiączkowe lub po stosunku2317
  • Ból podczas stosunku płciowego (dyspareunia)2425
  • Ból lub dyskomfort w dolnej części brzucha610

W przypadku progresji infekcji i jej rozprzestrzenienia się do górnych dróg rodnych, mogą pojawić się objawy zapalenia narządów miednicy mniejszej (PID), takie jak silny ból brzucha, gorączka, nudności i wymioty.2611 PID stanowi poważne powikłanie rzeżączki u kobiet i może prowadzić do długotrwałego bólu miednicy, niepłodności i zwiększonego ryzyka ciąży ektopowej.627

Objawy zakażenia odbytu i gardła

Rzeżączka może również zainfekować odbyt i gardło, powodując charakterystyczne objawy w tych lokalizacjach. Infekcje te występują zazwyczaj na skutek kontaktu seksualnego (seks analny lub oralny) z osobą zakażoną.6

Objawy zakażenia odbytu (rzeżączkowe zapalenie odbytnicy) mogą obejmować:177

  • Wydzielinę z odbytu1016

Warto zauważyć, że zakażenia odbytu często przebiegają bezobjawowo – według niektórych źródeł nawet 90% przypadków może nie powodować żadnych objawów.298

Infekcja gardła (rzeżączkowe zapalenie gardła) również często przebiega bezobjawowo.287 Jeśli jednak objawy występują, mogą obejmować:1410

  • Ból gardła i trudności w połykaniu3031
  • Obrzęk węzłów chłonnych szyi1030
  • Czerwone lub białe plamy w jamie ustnej30

Badania wykazują, że infekcje gardła mogą utrzymywać się dłużej niż zakażenia narządów płciowych – mediana czasu trwania nieleczonego zakażenia gardła wynosi około 16 tygodni.3232 Infekcje gardła są również trudniejsze do wyleczenia niż zakażenia narządów płciowych i mogą stanowić istotne źródło transmisji w społeczności.5

Zakażenie oczu (zapalenie spojówek)

Rzeżączka może również zainfekować oczy, powodując rzeżączkowe zapalenie spojówek (gonococcal conjunctivitis). Zakażenie to może wystąpić, gdy zainfekowane płyny (np. nasienie lub wydzielina z pochwy) mają kontakt z oczami.2713

Należy podkreślić, że rzeżączkowe zapalenie spojówek wymaga szybkiego leczenia, ponieważ w przeciwnym razie może prowadzić do poważnych powikłań, w tym utraty wzroku.3327 Noworodki mogą zostać zakażone podczas porodu, jeśli matka ma rzeżączkę, co może prowadzić do rzeżączkowego zapalenia spojówek u noworodka.827

Progresja choroby i powikłania

Nieleczona rzeżączka może prowadzić do poważnych powikłań i trwałych problemów zdrowotnych, które różnią się w zależności od płci pacjenta.1734 Powikłania te mogą rozwinąć się nawet bez wyraźnych objawów początkowych, co podkreśla znaczenie wczesnego wykrywania i leczenia.8

Powikłania u kobiet

U kobiet nieleczona rzeżączka może rozprzestrzeniać się do górnych dróg rodnych, powodując zapalenie narządów miednicy mniejszej (PID).617 PID jest poważnym powikłaniem, które dotyka około 10-20% kobiet z nieleczoną rzeżączką.8 Powikłania związane z PID obejmują:1727

  • Tworzenie się tkanki bliznowatej w jajowodach, co może prowadzić do niedrożności1735
  • Niepłodność – uszkodzenie jajowodów może uniemożliwić zajście w ciążę636
  • Zwiększone ryzyko ciąży ektopowej (pozamacicznej)1737

  • Ropnie wewnętrzne (trudne do wyleczenia kieszenie wypełnione ropą)8

Kobiety z PID nie zawsze doświadczają objawów. Gdy objawy występują, mogą być bardzo nasilone i obejmować ból brzucha oraz gorączkę.811

Rzeżączka podczas ciąży stanowi dodatkowe ryzyko. Może prowadzić do poronienia, przedwczesnego porodu, niskiej masy urodzeniowej noworodka oraz zakażenia dziecka podczas porodu.2738 Zakażenie noworodka może prowadzić do zapalenia spojówek, które nieleczone może powodować ślepotę.827

Powikłania u mężczyzn

U mężczyzn nieleczona rzeżączka może prowadzić do następujących powikłań:1728

  • Zapalenie najądrza (epididymitis) – bolesny stan objawiający się obrzękiem jąder, który w rzadkich przypadkach może prowadzić do niepłodności820
  • Zapalenie prostaty (prostatitis)927
  • Zwężenie cewki moczowej na skutek tworzenia się blizn, co może powodować trudności w oddawaniu moczu2939
  • Rzadziej: niepłodność2840

Rozsiana infekcja gonokokowa (DGI)

W rzadkich przypadkach rzeżączka może rozprzestrzeniać się przez krwiobieg do innych części ciała, powodując rozsianą infekcję gonokokową (DGI).1734 DGI występuje u około 1% osób z rzeżączką i może być stanem zagrażającym życiu.4142

Objawy DGI mogą obejmować:2943

DGI wymaga natychmiastowego leczenia, ponieważ może prowadzić do trwałych uszkodzeń organów i stanów zagrażających życiu.4634

Czynniki zwiększające ryzyko powikłań

  • Współistniejące inne infekcje przenoszone drogą płciową – rzeżączka zwiększa podatność na zakażenie HIV i może utrudniać kontrolę HIV u osób z koinfekcją1528
  • Zaburzenia odporności – osoby z obniżoną odpornością są bardziej narażone na ciężki przebieg i powikłania34

Czasowy przebieg objawów rzeżączki

Przebieg czasowy objawów rzeżączki można podzielić na kilka etapów:106

Okres inkubacji

Okres od zakażenia do pojawienia się pierwszych objawów wynosi zazwyczaj 2-7 dni, choć może wahać się od 1 do 30 dni.1011 W tym czasie bakterie namnażają się w organizmie, ale nie wywołują jeszcze reakcji układu odpornościowego powodującej objawy.6

Wczesne objawy (2-14 dni od zakażenia)

U mężczyzn pierwsze objawy zazwyczaj obejmują dyskomfort w cewce moczowej, a następnie bolesne oddawanie moczu i wydzielinę z prącia.1348 U kobiet, jeśli objawy występują, mogą obejmować zwiększoną wydzielinę z pochwy i dyskomfort podczas oddawania moczu.644

Objawy w tej fazie mogą być łagodne i niespecyficzne, co może prowadzić do ich przeoczenia lub pomylenia z innymi infekcjami.1017

Progresja objawów (bez leczenia)

Bez odpowiedniego leczenia, objawy mogą nasilać się w ciągu kolejnych dni lub tygodni.11 U mężczyzn wydzielina z cewki moczowej staje się bardziej obfita i ropna.11 U kobiet mogą rozwinąć się objawy PID, takie jak ból w dolnej części brzucha i gorączka.11

Późne powikłania (tygodnie do miesięcy)

Bez leczenia, rzeżączka może prowadzić do poważnych powikłań, takich jak PID u kobiet lub zapalenie najądrza u mężczyzn.620 W rzadszych przypadkach może rozwinąć się DGI.3417

Długotrwałe następstwa nieleczonej rzeżączki mogą obejmować niepłodność, przewlekły ból miednicy oraz zwiększone ryzyko ciąży ektopowej u kobiet.635

Fluktuacja objawów

Objawy rzeżączki mogą czasem ustępować i nawracać, co może prowadzić do błędnego przekonania, że infekcja ustąpiła samoistnie.4950 Należy jednak podkreślić, że rzeżączka nie ustępuje samoistnie i wymaga leczenia antybiotykami.5149

Po rozpoczęciu leczenia objawy zazwyczaj ustępują w ciągu jednego do dwóch tygodni.4931 Różne objawy mogą ustępować w różnym tempie – na przykład ból miednicy czy jąder może utrzymywać się do dwóch tygodni.49

Jeżeli objawy utrzymują się dłużej niż 3-5 dni po zakończeniu leczenia, może to wskazywać na nieskuteczność terapii, reinfekcję lub współistniejącą inną infekcję.495 W takim przypadku niezbędna jest ponowna konsultacja lekarska.19

Nietypowe objawy i prezentacje kliniczne

Oprócz typowych objawów, rzeżączka może czasami manifestować się w nietypowy sposób, co może utrudniać diagnozę:3411

  • Zapalenie gardła – może objawiać się jedynie łagodnym bólem gardła lub nie dawać żadnych objawów138
  • Bakteriemia gonokokowa – rozprzestrzenienie się bakterii do krwiobiegu, powodujące gorączkę i ogólne osłabienie1311
  • Zapalenie wsierdzia – rzadkie, ale potencjalnie zagrażające życiu powikłanie, które może objawiać się podwyższoną temperaturą, dreszczami i ogólnym złym samopoczuciem1141
  • Zapalenie błony maziowej (tenosynovitis) – objawiające się bólem i obrzękiem ścięgien1141

Rzeżączka u dzieci jest zazwyczaj objawem wykorzystywania seksualnego i wymaga natychmiastowej interwencji medycznej oraz socjalnej.13
