Torbiel Bartholina to łagodna zmiana torbielowata powstająca w wyniku zablokowania przewodu wyprowadzającego gruczołu Bartholina, zlokalizowanego po obu stronach wejścia do pochwy. Występuje u około 2% kobiet, głównie w wieku reprodukcyjnym (20-30 lat). Małe torbiele (0,2-2,5 cm) są często bezobjawowe, natomiast większe mogą powodować dyskomfort, ból podczas chodzenia, siedzenia, stosunku płciowego oraz uczucie pełności w obrębie sromu. Zakażenie torbieli prowadzi do powstania ropnia, charakteryzującego się silnym, pulsującym bólem, obrzękiem, zaczerwienieniem, gorączką, dreszczami oraz ropnym wyciekiem. Ropień może pęknąć samoistnie, co przynosi ulgę, jednak wymaga konsultacji lekarskiej w celu zapobieżenia rozprzestrzenianiu się zakażenia.

Definicja torbieli Bartholina

Torbiel Bartholina to łagodna zmiana torbielowata, która powstaje w wyniku zablokowania przewodu wyprowadzającego gruczołu Bartholina. Gruczoły te są parzyste i znajdują się po obu stronach wejścia do pochwy. Ich funkcją jest produkcja wydzieliny nawilżającej przedsionek pochwy. Kiedy ujście gruczołu zostaje zablokowane, dochodzi do gromadzenia się wydzieliny i powstania torbieli wypełnionej płynem 12. Torbiele Bartholina są jednymi z najczęstszych zmian torbielowatych sromu i występują u około 2% kobiet w ciągu życia, najczęściej w wieku reprodukcyjnym, między 20. a 30. rokiem życia 34.

Objawy torbieli Bartholina

Objawy torbieli Bartholina zależą od jej wielkości oraz obecności ewentualnego zakażenia. W większości przypadków torbiele są bezobjawowe i mogą zostać wykryte przypadkowo podczas badania ginekologicznego lub samobadania 56.

Objawy niezakażonej torbieli

Mała, niezakażona torbiel Bartholina często pozostaje niezauważona i może nie powodować żadnych objawów. Niektóre kobiety mogą zaobserwować:

  • Bezbolesny guzek w okolicy wejścia do pochwy (zazwyczaj jednostronny) 78
  • Asymetrię warg sromowych – jedna strona może wyglądać na powiększoną 910
  • Miękki, okrągły guzek o średnicy od 0,2 do 2,5 cm (wielkości od grochu do piłki golfowej) 1112

W miarę wzrostu torbieli mogą pojawić się dodatkowe objawy 13:

Objawy zakażonej torbieli (ropień)

Jeśli dojdzie do zakażenia torbieli Bartholina, powstaje ropień. Zakażenie może rozwinąć się bardzo szybko, nawet w ciągu kilku dni 2223. Charakterystyczne objawy zakażonej torbieli (ropnia) to:

  • Silny, pulsujący ból w okolicy torbieli 2425
  • Obrzęk, zaczerwienienie i ucieplenie okolicy torbieli 2627
  • Znaczne nasilenie bólu podczas chodzenia, siedzenia i stosunków płciowych 2829
  • Gorączka i dreszcze 3031
  • Wyciek ropny o nieprzyjemnym zapachu 3233
  • Ogólne złe samopoczucie 34
  • W niektórych przypadkach powiększenie węzłów chłonnych pachwinowych 35

W przypadku dużego ropnia ból może być na tyle silny, że uniemożliwia normalne funkcjonowanie, chodzenie, siedzenie czy spanie 3637. Samoistne pęknięcie ropnia może przynieść nagłą ulgę w bólu, co jest charakterystycznym objawem 3839.

Przebieg torbieli Bartholina

Naturalny przebieg

Torbiel Bartholina może mieć różny przebieg kliniczny. W wielu przypadkach małe, niezakażone torbiele mogą pozostać bezobjawowe przez długi czas lub nawet ustąpić samoistnie w ciągu kilku tygodni 4041. Inne torbiele mogą powoli rosnąć z czasem, powodując narastające objawy 42.

Przebieg torbieli Bartholina można podzielić na kilka etapów 43:

  1. Wczesne stadium – niewielki, bezbolesny guzek, często niezauważalny
  2. Stadium rozwinięte – miękki, wypełniony płynem guzek, który może powodować dyskomfort
  3. Stadium zaawansowane – duża torbiel powodująca ból i utrudniająca codzienne czynności
  4. Zakażenie – przekształcenie torbieli w ropień, co wiąże się z gwałtownym nasileniem objawów

Zakażenie torbieli

Zakażenie torbieli Bartholina może rozwinąć się gwałtownie, w ciągu kilku dni 44. W wyniku zakażenia tworzy się ropień, który charakteryzuje się szybkim narastaniem objawów bólowych, obrzękiem, zaczerwienieniem i wzmożonym uciepleniem okolicy 4546.

Zakażenie może być spowodowane przez różne bakterie, w tym bakterie przenoszone drogą płciową, a także przez florę fizjologiczną skóry i pochwy 4748. W niektórych przypadkach obserwuje się zakażenie bakteriami MRSA (methicylino-oporny Staphylococcus aureus) 49.

Samoistne pęknięcie

W przypadku ropnia torbieli Bartholina może dojść do jego samoistnego pęknięcia. Objawia się to nagłym zmniejszeniem bólu i wyciekiem krwistej lub ropnej wydzieliny 5051. Po samoistnym pęknięciu ropnia objawy zazwyczaj szybko ustępują, a dyskomfort może utrzymywać się jeszcze przez kilka dni 52.

Samoistne pęknięcie ropnia może prowadzić do wyleczenia w ciągu 3-4 dni, jednak zaleca się konsultację lekarską, aby zapobiec rozprzestrzenieniu się zakażenia 5354.

Czas trwania torbieli

Czas trwania torbieli Bartholina jest bardzo zróżnicowany i zależy od jej wielkości, obecności zakażenia oraz zastosowanego leczenia 55. Małe, niezakażone torbiele mogą ustąpić samoistnie w ciągu kilku tygodni po zastosowaniu metod domowych, takich jak ciepłe kąpiele 5657.

W przypadku ropnia, po jego skutecznym drenażu, poprawa następuje zazwyczaj w ciągu 24 godzin 5859. Całkowity czas gojenia po zabiegach chirurgicznych może wynosić od kilku dni do kilku tygodni, w zależności od rodzaju i rozległości zabiegu 60.

Nawroty

Torbiele Bartholina mają tendencję do nawrotów. U niektórych kobiet torbiele mogą powracać wielokrotnie, nawet po latach od pierwszego epizodu 6162. Częstość nawrotów zależy od zastosowanej metody leczenia. Proste nacięcie i drenaż wiążą się z wyższym ryzykiem nawrotu niż bardziej zaawansowane metody chirurgiczne, takie jak marsupializacja (wytworzenie stałego otworu w torbieli) 6364.

W przypadku częstych nawrotów torbieli lub ropni może być konieczne całkowite usunięcie gruczołu Bartholina 6566.

Różnicowanie torbieli Bartholina

Należy pamiętać, że nie każdy guzek w okolicy wejścia do pochwy jest torbielą Bartholina. W diagnostyce różnicowej należy uwzględnić 67:

  • Torbiele przewodów Skene’a
  • Torbiele pochwy
  • Tłuszczaki
  • Włókniaki
  • Rak sromu (rzadko, ale należy wykluczyć, szczególnie u kobiet po menopauzie) 68

U kobiet po 40. roku życia lub po menopauzie, u których pojawia się guzek w okolicy gruczołu Bartholina, zaleca się wykonanie biopsji w celu wykluczenia procesu nowotworowego 69.

Powikłania torbieli Bartholina

Choć torbiele Bartholina są zazwyczaj łagodne, nieleczone mogą prowadzić do powikłań 70:

  • Rozwój ropnia z silnym bólem i gorączką 71
  • Zapalenie tkanki łącznej (cellulitis) otaczającej ropień 7273
  • Rozprzestrzenienie się zakażenia na okoliczne tkanki 74
  • W skrajnych przypadkach, nieleczone zakażenie może przedostać się do krwiobiegu, prowadząc do posocznicy 7576
  • Nawracające zakażenia mogą prowadzić do bliznowacenia, co zwiększa ryzyko wystąpienia przyszłych torbieli lub ropni 77
  • Długotrwały dyskomfort i obniżenie jakości życia, szczególnie w przypadku nawracających torbieli 78

Kiedy zgłosić się do lekarza

Należy skonsultować się z lekarzem w przypadku 7980:

Obraz kliniczny torbieli Bartholina – podsumowanie

Torbiel Bartholina to stosunkowo częsta przypadłość, dotykająca około 2% kobiet, głównie w wieku rozrodczym. Przebieg kliniczny jest zróżnicowany – od bezobjawowych, małych torbieli, które mogą ustąpić samoistnie, po duże, bolesne ropnie wymagające interwencji medycznej 8788.

Charakterystyczne dla torbieli Bartholina jest jednostronne występowanie (zwykle tylko po jednej stronie wejścia do pochwy) oraz możliwość przekształcenia się torbieli w ropień w przypadku zakażenia 8990.
