Rokowanie w niedoczynności tarczycy jest generalnie dobre przy wczesnym wdrożeniu terapii substytucyjnej hormonami tarczycy, co pozwala na normalizację długości życia i zachowanie jakości życia pacjentów. Nieleczona niedoczynność tarczycy niesie ryzyko poważnych powikłań, takich jak śpiączka hipometaboliczna (myxedema) i zgon. W kontekście COVID-19, zaburzenia tarczycy, w tym niedoczynność, są związane z gorszym rokowaniem (OR 2,92; 95% CI 2,09-4,08; p=0,001), choć czułość niedoczynności tarczycy w przewidywaniu ciężkiego przebiegu COVID-19 wynosi jedynie 24% (95% CI 12-42%), a swoistość 92% (95% CI 87-96%). U starszych pacjentów z COVID-19 niedoczynność tarczycy może wiązać się z łagodniejszym przebiegiem choroby, a jej wpływ na śmiertelność wewnątrzszpitalną nie jest istotny po korekcie czynników zakłócających (HR=0,69; 95% CI 0,47–1,03; p=0,07).

Niedoczynność tarczycy (hypothyroidism) – Rokowanie (outcome prediction)

Rokowanie w niedoczynności tarczycy jest generalnie dobre pod warunkiem wdrożenia odpowiedniego leczenia, szczególnie jeśli zostanie ono rozpoczęte wcześnie. Przy właściwej terapii substytucyjnej hormonami tarczycy, pacjent z niedoczynnością tarczycy zazwyczaj ma normalną długość życia, z niewielkim lub żadnym obniżeniem jakości życia.1

Podstawowe rokowanie przy leczeniu

Obecnie nie istnieje metoda lecznicza pozwalająca na całkowite wyleczenie niedoczynności tarczycy, jednak można skutecznie kontrolować stan chorobowy za pomocą terapii substytucyjnej hormonami tarczycy. Pacjenci z niedoczynnością tarczycy mają bardzo dobre rokowanie, jeśli otrzymują odpowiednie leczenie.2 Niedoczynność tarczycy jest stanem zdrowotnym, który w większości przypadków charakteryzuje się obiecującym rokowaniem przy właściwym leczeniu.3

Rokowanie przy braku leczenia

Nieleczona niedoczynność tarczycy niesie za sobą wysokie ryzyko powikłań medycznych, w tym śpiączki i śmierci. Bez leczenia niedoczynność tarczycy może stać się poważnym i zagrażającym życiu stanem medycznym.45 Szczególnie niebezpieczne jest rozwinięcie się śpiączki hipometabolicznej (myxedema), która stanowi bezpośrednie zagrożenie życia.6

Niedoczynność tarczycy a COVID-19

Relacja między metabolizmem tarczycy a stanem zapalnym w przebiegu COVID-19 była przedmiotem licznych badań. Metaanalizy wykazały, że zaburzenia tarczycy, niezależnie od ich typu, były związane z gorszym rokowaniem w COVID-19 (OR 2,92 (95% CI 2,09-4,08), P 0,001; I2 = 71%, P 0,001).78

Specyficznie dla niedoczynności tarczycy, diagnostyczna metaanaliza wykazała, że ma ona czułość 0,24 (0,12-0,42), swoistość 0,92 (0,87-0,96) i AUC wynoszące 0,77 w przewidywaniu gorszych wyników COVID-19.9 Oznacza to, że obecność niedoczynności tarczycy przy przyjęciu do szpitala miała czułość 24% i swoistość 92% w przewidywaniu złożonych niekorzystnych wyników u pacjentów z COVID-19.1011

Warto jednak zauważyć, że w badaniu wieloośrodkowym skupiającym się na starszych pacjentach z COVID-19, historia niedoczynności tarczycy nie wydawała się istotnie wpływać na śmiertelność wewnątrzszpitalną. W modelu Coxa niedoczynność tarczycy była związana ze zmniejszoną śmiertelnością wewnątrzszpitalną tylko w modelu jednowymiarowym (HR=0,66, 95% CI 0,45–0,96, p=0,03), natomiast nie zaobserwowano istotnych wyników po skorygowaniu o potencjalne czynniki zakłócające (HR=0,69, 95% CI 0,47–1,03, p=0,07).12

Co ciekawe, niedoczynność tarczycy może być związana z łagodniejszą kliniczną i biochemiczną prezentacją choroby przy przyjęciu na oddział u starszych pacjentów z COVID-19.13

Niedoczynność tarczycy a nowotwory

Niedoczynność tarczycy może wystąpić jako późny efekt leczenia ukierunkowanego na raka piersi, szczególnie po radioterapii. Jednak niewiele wiadomo na temat tego, czy niedoczynność tarczycy wpływa na rokowanie po raku piersi.14

Wyniki dużego badania kohortowego sugerują, że niedoczynność tarczycy obecna w momencie diagnozy lub pojawiająca się w trakcie obserwacji nie jest związana z nawrotem raka piersi ani śmiertelnością z jakiejkolwiek przyczyny. Dla nawrotu raka i śmiertelności z jakiejkolwiek przyczyny, te bliskie zeru wyniki nie uległy modyfikacji po stratyfikacji według statusu menopauzalnego, statusu receptora estrogenowego (ER), chemioterapii lub radioterapii, czy czasu trwania niedoczynności tarczycy przed diagnozą raka piersi.15

Z klinicznego punktu widzenia jest to uspokajające dla pacjentów cierpiących na niedoczynność tarczycy i ich lekarzy, podkreślając, że niedoczynność tarczycy prawdopodobnie nie ma niekorzystnego wpływu na kliniczny przebieg raka piersi ani przeżycie.16

Monitorowanie i kontrola rokowania

Dla optymalizacji rokowania kluczowe znaczenie ma regularne monitorowanie funkcji tarczycy u pacjentów z niedoczynnością tarczycy oraz odpowiednie dostosowywanie dawek leków. Właściwa kontrola stężenia hormonów tarczycy w surowicy krwi pozwala na minimalizację ryzyka powikłań długoterminowych i utrzymanie dobrej jakości życia.17

Pacjenci powinni być świadomi, że niedoczynność tarczycy jest stanem wymagającym długoterminowego, najczęściej dożywotniego leczenia, ale przy prawidłowym postępowaniu terapeutycznym nie powinna ona istotnie wpływać na długość i jakość życia.18
