Kluczowe elementy leczenia podtrzymującego obejmują:456

Pozaustrojowa oksygenacja membranowa (ECMO)
W najcięższych przypadkach HPS, gdy konwencjonalne metody wspomagania oddychania nie są wystarczające, stosuje się pozaustrojową oksygenację membranową (ECMO). Metoda ta polega na ciągłym pompowaniu krwi pacjenta przez urządzenie, które usuwa dwutlenek węgla i dodaje tlen, po czym natleniona krew jest zwracana do organizmu.123
Zastosowanie ECMO we wczesnym stadium dekompensacji układu oddechowego wiąże się z około 80% wskaźnikiem przeżycia pacjentów, mimo zapaści krążeniowo-oddechowej.12 ECMO wspiera niewydolne serce i płuca wystarczająco długo, aby umożliwić ich powrót do prawidłowego funkcjonowania.1
Leczenie farmakologiczne
Obecnie nie ma zatwierdzonego przez FDA specyficznego leczenia przeciwwirusowego dla zakażeń hantawirusem powodujących HPS. Jednak kilka leków przeciwwirusowych było i jest badanych pod kątem potencjalnej skuteczności:12
Rybawiryna
Rybawiryna to analog guanozyny o szerokim spektrum działania przeciwwirusowego. Wykazano jej skuteczność przeciwko niektórym hantawirusom w badaniach in vitro i częściowo in vivo:12
- Skuteczność rybawiryny w leczeniu gorączki krwotocznej z zespołem nerkowym (HFRS) została potwierdzona w badaniach klinicznych w Chinach, gdy leczenie rozpoczęto we wczesnej fazie choroby1
- W modelu zakażenia wirusem Andes u chomików, leczenie dawką zaledwie 5 mg/kg dziennie było skuteczne w zapobieganiu śmiertelnej postaci HPS, gdy terapię podawano od 1 do 10 dnia po zakażeniu1
- Podawanie rybawiryny nawet 14 dni po zakażeniu zapewniało znaczny poziom ochrony przed śmiertelnym HPS1
Mimo obiecujących wyników badań przedklinicznych, dwa badania kliniczne dotyczące leczenia HPS za pomocą rybawiryny wykazały, że lek był nieskuteczny u pacjentów, u których HPS postąpił do fazy krążeniowo-płucnej.1 Obecnie nie zaleca się stosowania rybawiryny w leczeniu HPS.123
Fawipirawir
Fawipirawir (T-705) to inny lek przeciwwirusowy o szerokim spektrum działania, który wykazuje obiecujące wyniki w badaniach nad hantawirusami:12
- Badania wykazały, że fawipirawir silnie hamuje wirusy SNV i ANDV in vitro

Glikokortykosteroidy
Badanie przeprowadzone w ramach kontrolowanego badania klinicznego z placebo w Chile miało na celu ocenę skuteczności dożylnego metyloprednizolonu w zakażeniu HPS. Wyniki nie wykazały znaczącej różnicy w śmiertelności między grupami leczenia, a lek nie zapewnił istotnej klinicznie korzyści pacjentom. Wyniki te nie potwierdzają zasadności stosowania metyloprednizolonu w HPS.1
Leczenie wspomagające układ krążenia
Ze względu na znaczne upośledzenie funkcji serca w przebiegu HPS, istotnym elementem leczenia jest wspomaganie układu krążenia:12
- Utrzymanie średniego ciśnienia tętniczego powyżej 70 mm Hg
- Wsparcie inotropowe, na przykład dobutaminą, aby zwiększyć kurczliwość mięśnia sercowego
- Staranna kontrola bilansu płynów, aby utrzymać optymalne ciśnienie napełniania serca przy jednoczesnym minimalizowaniu obrzęku płuc
Dializa i leczenie nerkozastępcze
W przypadkach, gdy zakażenie hantawirusem prowadzi do zaburzenia funkcji nerek, szczególnie w przypadku gorączki krwotocznej z zespołem nerkowym (HFRS), pacjenci mogą wymagać leczenia nerkozastępczego:12

Znaczenie wczesnego rozpoznania i leczenia

To date, there are no US Food and Drug Administration-approved antiviral drugs available for use against HFRS or HPS. Therefore, the management of severe cases is purely based on supportive care. Maintaining fluid and electrolyte balance is very important in these patients. HFRS patients with severe renal insufficiency may need extracorporeal blood purification (dialysis treatment). In HCPS, mechanical ventilation or even extracorporeal membrane oxygenation may be required. Additional to supportive care, there had been trials of antiviral and immunotherapies done against HFRS and HPS. They are as follows: […] Effectiveness of ribavirin against some Hantaviruses had been proved in vitro and to some extent in vivo. Efficacy of ribavirin therapy against HFRS has been evaluated using a Hantaan virus-infected suckling mouse model with various doses of ribavirin. Treatment with 50 mg of ribavirin/kg per day beginning on day 10 of the viral challenge demonstrated a higher survival rate compared with placebo controls in mice. Ribavirin efficacy against Andes virus-related HPS has been shown in vitro using Vero E6 cells and in vivo using a lethal hamster HPS model. In hamsters, treatment with as little as 5 mg/kg per day was effective in preventing lethal HPS disease when therapy was administered via intraperitoneal injection from day 01 through day 10 post-infection. Ribavirin remained 100% protective when administered via intraperitoneal injections up to 03 days following infection. Also, another study has shown 100 mg/kg and 50 mg/kg concentrations of ribavirin prevented HPS in hamsters without toxicity, and administration of ribavirin at 14 days post-infection also provided a significant level of protection against lethal HPS. These data provide in vivo evidence supporting the potential use of ribavirin as a post-exposure treatment to prevent HPS caused by ANDV. However, two clinical trials involving the treatment of HPS using ribavirin have shown that ribavirin was ineffective for patients having HPS progressed to the cardiopulmonary phase.

A study has been done using a parallel-group, placebo-controlled clinical trial in Chile to see the efficacy of intravenous methylprednisolone for HPS infection. It concludes as there is no significant difference in mortality between treatment groups was observed and it did not provide significant clinical benefit to patients.
